About this file

The Price Appendix FAQ is a detailed guide for submitting pricing information for a Department of Defense Pharmacy Uniform Formulary Blanket Purchase Agreement/Uniform Formulary Additional Discount Program. The document provides instructions for completing pricing spreadsheets for pharmaceutical agents, with three distinct pricing approaches: Blanket Purchase Agreement (BPA), Additional Discount Program (ADP) non-FAMP Discount, and ADP Wholesale Acquisition Cost (WAC) Discount.

Key instructions include filling out yellow cells in provided spreadsheets, with auto-population and auto-calculation features for various pricing columns. Vendors can submit pricing for up to 10 National Drug Codes (NDCs) per spreadsheet, with options to request additional tables for more NDCs. The document emphasizes reading Formulary Notes carefully, highlights that a 0.0000% discount will indicate non-participation in a formulary category, and provides contact information for vendors with questions. The associated solicitation involves reviewing newly approved drugs across multiple therapeutic categories for the May 2025 Pharmacy and Therapeutics Committee meeting.
