About this file
This is a Prevailing Wage Weekly Statement of Compliance form issued by the Massachusetts Department of Labor Standards (DLS) in accordance with Massachusetts General Law Chapter 149, Section 27B, applicable to public works projects in the state. The document serves as a mandatory compliance certification requirement for all contractors and subcontractors working on public works projects subject to prevailing wage laws. Contractors and subcontractors must submit certified weekly payroll records and this completed statement form to the awarding authority either by regular mail or email on a weekly basis and within 15 days after completion of their portion of work. The statement must be executed by an authorized officer who supervised wage payments and must certify that all mechanics, apprentices, teamsters, chauffeurs, and laborers employed on the project have been paid in accordance with prevailing wage rates determined under Massachusetts General Laws Chapters 149, Sections 26 and 27.
All contractors and subcontractors are required to maintain accurate payroll records for all persons employed on the project and preserve these records for three years from the contract completion date. The awarding authority is similarly obligated to preserve collected certified payroll records for three years following project completion. Payroll forms containing all legally required information are available from the DLS at mass.gov/dols/pw. This compliance documentation is a standard requirement for the Brockton Multi Service Center Bi-Annual Cleanup of Grounds project and ensures adherence to prevailing wage standards for all workers employed under the contract.
