About this file

This document is a Weekly Payroll Records Report & Statement of Compliance form issued by the Massachusetts Department of Labor Standards (DLS), in accordance with Massachusetts General Law c. 149, §27B. The form requires contractors and subcontractors working on public works projects to maintain and submit accurate weekly payroll records to the awarding authority, with documentation to be preserved for three years from the contract's completion date.

The form mandates that every contractor and subcontractor submit a certified copy of their weekly payroll records within 15 days of completing their portion of work, including a Statement of Compliance form. This statement must be signed by an authorized officer who supervised wage payments, certifying that all workers (mechanics, apprentices, teamsters, chauffeurs, and laborers) have been paid in accordance with the predetermined wage rates established under specific sections of Massachusetts General Laws. The payroll records can be submitted via regular mail or email, and the awarding authority is responsible for maintaining these records for three years.
