About this file
This document is a past performance questionnaire (PPQ) template for a federal contractor to evaluate past work completed for the Department of Veterans Affairs (VA) Technology Acquisition Center Austin. The VA is considering awarding the contractor the Electronic Health Record Modernization Infrastructure Upgrades Design Project at the Southeast Louisiana Veterans Health Care System in New Orleans. The PPQ requests information on a previous contract to assess the contractor's ability to perform the new project, which involves design work. It prompts the evaluator to provide adjectival ratings and comments on factors such as quality, schedule, communication, management, cost control, and subcontract oversight from the past project. The evaluator is also asked if they would hire or work with the contractor again in the future.
