About this file

This is a Past Performance Questionnaire (PPQ) template for the Holistic Health and Fitness (H2F) Performance Team services contract (W81K0027RA002) being conducted by the Medical Acquisition Support Office (MASO) for the U.S. Army Reserve. The questionnaire is designed to be completed by past performance references to validate a contractor's experience delivering similar services.

The PPQ requests information about the contractor's prior performance across multiple dimensions, including contract identification, service descriptions, performance locations, contract duration, annual and total contract values, labor categories and Full-Time Equivalents (FTEs), and any instances of unsatisfactory performance resulting in additional costs. References must rate the contractor on two primary evaluation criteria using a four-point scale (Excellent, Satisfactory, Marginal, Unsatisfactory): Quality of Service based on compliance with the Performance Work Statement, and Business/Customer Relations based on effective management, cooperation, flexibility, and customer focus. Ratings other than "Satisfactory" require narrative explanations supporting the assessment. Completed questionnaires should be returned to nathaniel.w.trulove.civ@army.mil and salameya.paulouskaya2.civ@health.mil. The underlying solicitation seeks 25 FTEs comprising Registered Dietitians (5), Cognitive Performance Specialists (5), Strength and Conditioning Coaches (10), and Athletic Trainers (5) to provide non-personal services supporting Army Reserve soldier readiness through holistic health and fitness programming at multiple locations nationwide.
