About this file
This document contains a past performance questionnaire for a federal contractor. The questionnaire requests information about a contractor's previous work on a U.S. Fish and Wildlife Service contract to replace liners in fish pond 4, including replacing asbestos-cement drain lines with a new drain line from existing kettles to the termination point. Respondents are asked to rate the contractor's past performance on factors such as managerial experience, personnel quality, quality control, problem identification and resolution, compliance with contract terms, responsiveness to changes, submittal timeliness, resolution of punch-list items and warranty calls, and overall customer satisfaction. The questionnaire also inquires whether the contractor has ever received cure notices, show cause notices, payment suspensions or claim denials, and whether the respondent would award the contractor another contract. The respondent is asked to provide identifying information and additional comments.
