PERFORMANCE WORK STATEMENT

PERFORMANCE WORK STATEMENT
Base Linen (Excluding Hospital)
I. DESCRIPTION OF SERVICES – BASE LINEN (EXCLUDING HOSPITAL)
1. Scope: This is a non-personal services contract to provide base linen service. The contractor shall provide all personnel, equipment, tools, materials, vehicles, supervision and other items and service necessary to perform laundry and dry cleaning service for Wright Patterson Inn at Wright Patterson Air Force Base (WPAFB). The Government will not exercise any supervision or control over the contractor. The contractor shall perform to the standards in the contract as well as all local, state, federal, health, occupational safety and professional industry standards.
1.1 Pick Up: Weighing/counting of articles, will be done by Government personnel prior to pick-up. The contractor’s representative shall verify weight at time of pick up. Signature on the receipt constitutes agreement with the Government’s weight or count. A copy of the pickup ticket will be retained by the Government for computing variance in weight when clean/dry laundry is delivered.
1.2 Bulk Laundry: Weighing of bulk laundry will be done by the Government prior to pick up by the contractor. The contractor shall pick up and deliver bulk laundry on weekdays to Building 258, Area A between the hours of 6:30 am and 8:00 am. Service shall be daily with a two (2) workday turn around; e.g. items picked up Monday shall be returned Wednesday, items picked up on Friday will be returned on Tuesday. Contractor’s representative shall verify weight at time of pick up. Signature on the receipt constitutes agreement with the Government’s weight. A copy of the pickup ticket will be retained by the Government for computing variance in weight when clean/dry laundry is delivered. Variance in bulk weight for laundry returned shall not exceed 16.64% from bulk weight recorded at pick up. The 16.64% variance shall be calculated by taking a monthly average. Laundering shall be accomplished in accordance with state and local Department of Health regulation and in compliance with any special labeling requirements. All bulk laundry items requiring re-laundering shall be returned by the contractor within 24 hours.
1.3 Piece Laundry and Dry Cleaning: All piece laundry and dry cleaning will be counted by the government prior to pick up. Pickup and delivery shall be made on weekdays at Building 258, Area A between the hours of 6:30am and 8:00am. All Honor Guard and Dress uniforms and other specialty clothing articles identified with a double asterisk – page Workload &Pricing Tables) of the contract shall be performed as next day service (24 hours). For example, articles picked up Monday morning shall be returned Tuesday morning. Items not requiring 24-hour service shall be picked up at the same time as the military uniform items and be returned with four (4) work days. Contractor personnel shall verify count and provide the Government with the returned count and original copy of the ticket. Items with a double asterisk shall be returned in 24 hours. All items shall be accounted for. No percent of variance shall be allowed on these items.
The Contractor shall develop a list of services performed along with cost for each service on the Pricing Attachment prior to award of contract.
Individual Piece Laundry and Dry Cleaning shall be ordered on an as needed basis. The payment method shall be the Government Purchase Card (GPC). Contractor shall submit a monthly report to the COR and Contracting Officer (CO) containing the following information: all payments processed for each transaction, the services provided, the number and type of articles, and dollar amount of each. The monthly report shall be submitted no later than 5 (five) business days after the last day of the month.
1.4 Packaging: Packaging of laundry and dry cleaning for delivery shall be as indicated in Exhibits 1, 4, 5, 6 and 7 of the contract with materials furnished by the contractor. Bulk linen shall be separated in carts by items; e.g. bath towels in a cart, mats in another.
1.5 Unserviceable Articles: All unserviceable articles and non-reparable articles shall be marked as such and returned in a separate bundle by the contractor to the Contracting Officer Representative (COR) after laundering or dry cleaning.
1.6 State or Local Department of Health Requirements: Compliance with any certification or labeling requirement arising from State or Local Department of Health Regulation shall be the responsibility of the contractor.
1.7 Special Laundering and Dry Cleaning Tasks: The following items listed below is subparagraphs require special handling (laundry / dry cleaning separately). To include as applicable: correct ticket, stain proof hanger and protective covering.
1.7.1 Draperies: Must be resized to original size after cleaning and prior to packaging.
1.7.2 Mops: Shall be washed and returned in plastic bags.
1.7.3 Honor Guard / Band Military Dress Uniform Items: Clean, dry free of lint, odor, spots, stains, press wrinkle free, center crease where creases are required (double creases not allowed). Hang in delivery carts so as to prevent wrinkling in transit.
1.7.4 Chem Suit: Dry Clean only. Chem suits consist of pants and jacket shall be cleaned according to instructions.
1.8 Repairs
1.8.1 Non-Reimbursable Repairs: Except for articles determined to be unserviceable per definition in Appendix A, the contractor shall repair all minor rips, tears, open seams and holes using thread and materials of the same quality, compatible color, color fasteners and texture as that used in the manufacture of the articles. The contractor shall replace all broken or missing buttons with buttons of matching color and similar appearance and quality. All such repairs shall be performed at no additional cost to the Government. NOTE: Linens and bath item repairs shall only be required upon request.
1.9 Laboratory Testing: During the period of the contract the COR will (not less than once each quarter) supply test pieces (swatch of material) to the contractor for processing in order to determine average tensile strength loss, whiteness retention and other properties during washing. Laundering of the test pieces may be witnessed by the contracting officer’s representative. When the one washing is complete, the test piece shall be returned to the COR and mailed by the COR to the International Fabricare Institute for evaluation. In the event the test results indicate tensile strength loss in excess of the percentages shown below, or appearance ratings are fair or poor, as shown below, the contractor shall take remedial action (adjust washroom formulas) and the test shall be repeated until results are within the tolerated limits.

1.10 Workmanship and Sanitation: Delivered finished articles shall conform to the accepted industry standards of quality of cleanliness, finish and appearance. All work performed shall be done under sanitary conditions.
1.10.1 Re-Performance of Laundering/Dry Cleaning/Repair: Re-performance shall be at no additional cost to the Government. Items shall be identified by the COR to the contractor’s representative at the next scheduled pickup after the unacceptable condition is discovered. The contractor shall identify and pack items separately after the unacceptable work has been re-accomplished.
1.11 Damaged Linen: When it has been determined by the Government that linen has been damaged or distorted due to contractor error the contractor shall make reimbursement at 100% of current replacement cost (no depreciations allowed).
1.12 Missing and Lost Items: Any articles missing from a delivery shall be delivered by the contractor in the next regularly scheduled delivery. Missing articles needed within 24 hours by the Government will be delivered before 1600 hours on the same day at no additional cost to the Government. Lost articles such as uniforms pieces will be paid for in full by the contractor.
1.13 Emergency Services: Example: civil disturbances, natural disasters and military wartime contingency operations.
1.14 Vehicle Interior Requirements: Soiled items and clean items shall not be transported in the same vehicle at the same time. All clean items shall be covered during transit to the activities (to prevent contamination). After transportation of dirty/soiled laundry the vehicle shall be vacuumed and sanitized with EPA approved Detergent Germicidal Agent before loading the clean articles.
1.15 Contaminated Linens
1.15.1 The contractor shall comply with all local health regulations and /or state laws pertaining to the transportation and handling of all contaminated linen.
1.15.2 The contaminated linens shall be placed in water soluble bags and a red plastic bag by the Government and shall be processed without individual handling by the contractor. Washing temperature shall be a minimum of 160 degrees Fahrenheit. Water soluble bags will be furnished by the Government at no cost to the contractor.
1.15.3 The contractor shall have facilities available whereby goods contaminated with infectious disease can be stored and received separately from the goods in the main plant and working area of the plant. Contractor shall be remove and completely destroy all contaminants in the linens during the laundry process.
1.16 The contractor shall ensure that all employees who handle soiled linen (including those involved in collecting, transporting, sorting and washing) receive adequate training regarding risk of exposure. All contractor employees shall follow all OSHA requirements and procedures, which shall include as a minimum the use of protective apparel such as glove and gowns or aprons to reduce the risk of contamination of exposed areas of skin and soiling of clothing. The contractor employees shall follow frequent hand washing procedures. The contractor is responsible for the health and well-being of their employees.
1.17 Recognized Holidays: Contractor shall not be required to conduct pickups or deliveries on the following federal holidays:

1.18 Base Closure Notification Procedures: The base could be closed because of but not limited to adverse weather, a base or local disaster, security problems or other events that would necessitate the closing of the base. After an official decision to close Wright Patterson AFB has been made by the Base Commander, the following radio and television stations will be notified of the closure:

1.19 Contractor shall receive no other form of notification of the base closure from the Government. The contractor is responsible for the notification of employees.
1.20 Quality Control: The contractor shall develop and maintain a quality control program to ensure laundry and dry cleaning services are performed in accordance with commonly accepted commercial practices and services identified in this PWS. The contractor shall develop and implement procedures to identify, prevent and ensure non-performance and continual repeat of defective services does not occur. As a minimum the contractor shall develop quality control procedures addressing the areas identified in “Part II” Services Summary.
1.21 Quality Control Plan: The contractor shall submit a QCP no later than any pre-service orientation. The QCP shall demonstrate how the contractor intends to ensure quality performance during the contract. The contractor shall maintain the QCP throughout the life of the contract. The contractor shall provide any updates to the plan to the contracting officer for acceptance with a copy to the COR when changes occur. The QCP shall include the following:
1.21.1 Inspection System: Written procedure for the inspection system includes the title/name of the individual responsible for the QCP and who shall perform the inspections. The location of inspection records and key control logs shall be made available to the government at all times during the term of the contract.
1.21.2 Deficiency Prevention: An inspection form describing the method for identifying and preventing defects in the quality of service performed.
1.21.3 Inspection Record: The inspection record provides the following information:
a. Date, time and location (building number) of the inspection
b. Title and signature block for the person who performed the inspection.
c. Ratings of acceptable or unacceptable for all work performance inspected.
d. Areas (room number, section, etc.) found to be deficient and corrective action taken.
e. Total number of observations and defects.
f. Type of inspection (100%, random, periodic, etc.)
g. Frequency (weekly, monthly, daily, etc.) of inspection.
1.22 Meetings: The contractor shall attend all meetings convened by the contracting activity or contract administration office. The CO, COR and other Government personnel periodically will meet with the contractor to discuss the contractor’s performance, and the contractor shall apprise the Government of problems. Appropriate action shall be taken by the contractor to resolve outstanding issues. The meetings shall be at no additional cost to the government.
1.23 REPORTS. The contractor shall report ALL labor hours (including subcontractor labor hours) as stated in Exhibit 2.

III. GOVERNMENT FURNISHED FACILITY, UTILITIES AND SERVICES
3.1 Facility: None. The contractor shall pick up and deliver only on base.
3.2 Equipment: The Government and contractor shall utilize scales for weighting of linen in building 258 Area A at Wright Patterson AFB.
3.3 Emergency Medical Services: The Government will provide emergency medical treatment and emergency patient transportation service for contractor personnel who are injured or become critically ill while making deliveries to Wright Patterson AFB building 258 Area A during the performance of this contract. The contractor shall reimburse the Government for the cost of medical treatment and patient transportation service at the current inpatient or outpatient treatment rate, as appropriate.
3.4 Physical Security: The contractor shall safeguard all government property for contractor use. The contractor shall conform to the provisions of AFI 31-209, The Resources Protection Program, for safeguarding the Government property.
3.5 The contractor shall record and report within one hour to the Contracting Officer or designed government representative, all facts relating to each instance or damage to government property or injury to either contractor or government personnel.
Hospital Linen
I. DESCRIPTION OF SERVICES/GENERAL INFORMATION
1.1 SCOPE
The contractor shall provide all personnel, equipment, tools, Laundry/Dry Cleaning tickets, materials, supervision and other items and services necessary to perform Laundry/Dry Cleaning Service. Performance shall be done in a manner to ensure all linen from Wright Patterson Medical Center will be processed and washed separately from all other facilities linen. The Medical Center's linen will not be mixed with other facilities linen at any time during the processing cycle. The contractor shall perform to the standards in this contract. The contractor shall meet all professional industry standards, health regulations, state laws, and Occupational Safety and Health (OSHA) requirements.
The Contractor shall develop a list of services performed along with cost for each service on the Pricing Attachment prior to award of contract.

1.2 SPECIFIC TASKS
1.2.1 Pick-up. The contractor shall pick-up soiled laundry and dry cleaning and deliver clean laundry and dry-cleaning Monday through Saturday BLDG 830, Area A, between 0700 hours and 1000 hours. Twenty-four (24) hour turn-around service is required on all laundry items. Forty-eight (48) hour turn-around service is required for all dry cleaning. The Medical Center's linen will not be mixed with other facilities' linen at any time during the processing cycle.
1.2.2 Weighing. Weighing of all bulk laundry shall be done by contractor personnel in the presence of a government representative prior to pick-up. The contractor shall verify weight by signing a soiled linen sheet for the items being picked up and such signature constitutes this agreement to the government's weight unless specific exception is taken. The soiled linen sheet will be retained by the government for computing variance in weight when clean/dry laundry is delivered.
1.2.3 Delivery. The contractor shall record the number of pounds/pieces for all items delivered on the delivery ticket and the QAP/Linen Officer shall verify this amount. The contractor shall provide the tickets and furnish the government the original and one copy. Difference in bulk weight for laundry returned shall not exceed 10% from bulk weight recorded on the prior day pick-up. No percentage variance shall be allowed for dry cleaned items.
1.3 PACKAGING
1.3.1 Bulk Laundry Items. Packaging of finished items shall be placed in a clean linen cart lined with unused plastic liner or a clean reusable liner. Bulk linen shall be measured in weight. All items shall be shrink wrapped in clear plastic as bundles, marked by type of item, amounts, and size as specified in Appendix A.
1.3.2 Dry Cleaning. All dry cleaned items returned from the laundry service shall have the correct laundry ticket number on a stain proof hanger under a protective covering. Uniforms with the same laundry/ticket number may be placed in a single protective covering.
1.4 UNSERVICEABLE ARTICLES
After laundering or dry cleaning, all unserviceable articles will be separated by the contractor. These articles shall be bundled and marked unserviceable. Final determination of whether these articles stay in the system or go to salvage shall be made by the Government Quality Assurance Personnel/ Linen Officer (QAP).
1.5 STATE OR LOCAL DEPARTMENT OF HEALTH REQUIREMENTS.
Compliance with any certification or labeling requirements arising from state or local department of health regulations is the responsibility of the contractor. The contractor shall follow the standards set forth in The Healthcare Laundry Accreditation Council publication, Accreditation Standards for Processing Reusable Textiles for Use in Healthcare Facilities.
1.6 VEHICLE/CARTS INTERIOR AND EXTERIOR SANITATION REQUIREMENTS
Sanitation of contractor provided vehicle and carts shall include interior and exterior cleaning. Vehicle interior must be metal including floor to allow for proper sanitation. After transporting dirty/soiled laundry, the vehicle and carts interiors shall be vacuumed and sanitized. The aseptic technique required daily for sanitizing is a pressurized steam apparatus or manual cleaning with an EPA registered and approved tuberculocidal detergent germicidal. After vehicles and carts have been sanitized, they shall be free from dirt, dust, litter, debris, dry soil and odor before loading the clean articles. Soiled items and clean items shall not be transported in the same vehicle at the same time. All clean items shall be covered during transit to prevent contamination.
1.7 WORKMANSHIP AND SANITATION
1.7.1 Laundry. The contractor shall ensure that proper cleaning procedures are followed; including personnel wearing protective clothing (etc., lab coat) during finish process, proper supplies are used and washing formulas are selected to enable extraction of various stains. Delivered finished articles shall be clean, free of stains and odor, free of lint and soap residue and other foreign matter. All work performed shall conform to industry standards, under dust and vermin free conditions. Failure to perform work under these conditions could result in harm to hospitalized military members by increasing the chance for hospital born infection. The Government QAP/Linen Officer shall be notified of any linen held for rewash as it will affect the 10% allowable weight variance as stated in
1.2.3. Notification shall be annotated by the contractor on the daily delivery sheet.
1.7.2 Polyester/Cotton Uniforms and Drapery. Identify and remove spots and stains from uniforms, (extra starching as requested). Polyester/cotton uniforms and drapery dry cleaning shall be free of wrinkles and shrinking of fabric. All uniform pants require a straight centered crease. Uniforms shall not appear yellow or damaged from cleaning.
1.8 REPERFORMANCE OF LAUNDERING/DRY CLEANING
1.8.1. Any items found to have been unacceptably cleaned shall be re-cleaned at no additional cost to the government including clean items that were dropped during processing. All items identified by the QAP/Linen Officer for re-performance shall be picked up on the next scheduled pick-up day and returned within 24 hours. The contractor shall identify and pack these items separately as required in Appendix A after re-performed.
1.9 CONTAMINATED LINENS
1.9.1 The contractor must comply with all local health regulations, state laws, and Occupational Safety and Health Administration (OSHA) requirements pertaining to the transporting of all linens.handling
1.9.2 The contractor shall treat all used linens as if they are contaminated. The Medical Center does not separate contaminated linens from other linens.
1.9.3 The contractor shall ensure that his employees who handle soiled linen (including those involved in collecting, transporting, sorting and washing) receiving adequate training regarding risk of exposure. The contractor shall ensure that his employees follow all (OSHA) requirements and procedures which shall include the use of protective apparel such as glove and gowns or aprons to reduce the risk of contamination of exposed areas of skin and soiling of clothing. The contractor shall also ensure his employees follow frequent hand washing procedures. The contractor is responsible for the health requirements for protection of their employees.
1.10 LAUNDERING INFANT'S CLOTHING
1.10.1 Infant's clothing, infant shirts, cotton infant blankets, and children's pajamas shall be laundered separately from all other items. The finished products will be evaluated by the QAP/Linen Officer to ensure articles are fresh smelling, soft in touch, and free of lent. These items shall not be laundered with any soap, detergent, or cleaning agent containing the chemical "Trichlorcarbanilide". The contractor shall furnish a chemical analysis from the manufacturer to the QAP/Linen Officer for all soaps, detergents, or cleaning agents that do not have the chemical contents on the label prior to use.
1.11 LABORATORY TESTING
1.11.1 The contractor shall obtain washroom test services from an approved laboratory at no additional cost to the Government. National Association of Institutional Linen Management (NAILM), 2130 Lexington Road, Suite H, Richmond, KY 40475 and International Fabricare Institute (IFI), 1251 Tech Road, Silver Spring, MD 20904, are two known acceptable laboratories. The contractor shall provide the name and address of the proposed laboratory to the Contracting Officer, as a submittal with bid package.
1.11.2 Twice yearly in the months of Oct and Apr, perform washroom tests for tensile strength, whiteness retention, yellowness, blood stain removal, soil removal and bleach effectiveness, in accordance with the laboratory instructions for light, moderate and heavy soil loads. The tests shall be conducted on contractor provided TEST SWATCHES that have been placed in the same wash cycle as the items picked up at the Medical Center. Upon completion, the swatch is sent to the lab for evaluation. Two (2) copies of the test results shall be provided to the QAP/Linen Officer not later than five (5) workdays from the date of receipt by the contractor from the laboratory.
1.11.3 In the event laboratory test results indicate tensile strength losses and appearance ratings in excess of the values shown in the standards chart below, the contractor shall take appropriate corrective action within five (5) working days and retest. This process shall be repeated until all values are within prescribed tolerances. The Government and the contractor will receive a copy of all test results.

III. GOVERNMENT FURNISHED FACILITY, UTILITIES AND SERVICES
3.1 Facilities: None.
3.2 Equipment: The government shall make available scales for weighing linen on the back loading dock of Bldg 830, located in Area A at WPAFB.
3.3 Emergency Medical Service: The Government will provide emergency medical treatment and emergency patient transportation service for contractor personnel who are injured or become critically ill during the performance of this contract while on the WPAFB installation grounds. The contractor shall reimburse the Government for the cost of medical treatment and patient transportation service at the current inpatient or outpatient treatment rate, as appropriate.
3.4 Record, and report promptly (within one hour), to the Contracting Officer or designated government representative, all available facts relating to each instance of damage to government property or injury to either contractor or government personnel.
IV. CONTRACTOR FURNISHED FACILITY, UTILITIES AND SERVICES
4.1.1 The contractor will provide all vehicles and carts required. Approximately 30 or more carts with a 400-800 pound capacity are required. Each cart must be marked with its empty weight and also be identified for WPAFB MEDICAL CENTER USE ONLY. The contractor shall provide a linen cart for each linen collection area of the Medical Center.
4.1.2 The contractor’s facility shall be equipped and ventilated so as to prevent dissemination of contaminants. The ventilation system shall meet state and federal requirements. The contractor's facility shall be subject to inspection of sanitary conditions at any time by a representative of the government.
4.1.3 The contractor shall provide all racks and hangers for dry cleaning articles; the contractor shall provide heavy duty hangers to hold the cubicle curtains during delivery and storage at the Medical Center. The contractor shall also provide protective covering for laundry/dry cleaning and any other material required to perform the services of this contract including laundry/dry cleaning delivery tickets.
4.1.4 The contractor shall provide a person (Point of Contact) who shall be responsible for the performance of the work. The name of this person, and an alternate or alternates, who shall act for the contractor when the manager is absent, shall be designated in writing to the contracting officer.
4.3 CONTRACTOR EMPLOYEES:
4.3.1 The contractor shall not employ persons for work on this contract if such employee is identified to the contractor by the contracting officer as a potential threat to the health, safety, security, general well-being or operational mission of the installation and its population.
4.3.2. Contractor personnel shall present a neat appearance and be easily recognized as contractor employees. This may be accomplished by wearing distinctive clothing bearing the name of the company or by wearing appropriate badges, which contain the company name and employee name. The contractor should specify that all his employees shall wear a clean uniform every day without rips or tears. The contractor should specify that all his employees must be clean by daily bathing to ensure clean area is not contaminated by employees.
4.3.3 The contractor shall not employ any person who is an employee of the US Government if employing that person would create a conflict of interest. Additionally, the contractor shall not employ any person who is an employee of the Department of the Air Force, either military or civilian, unless such person seeks and receives approval in accordance with Department of Defense, Joint Ethics Publications.
4.3.4 The contractor is prohibited from employing off-duty government QAP/Linen Officer who are surveying any contracts/subcontracts awarded to the contractor.
4.4 SECURITY REQUIREMENTS:
The contractor shall complete an "Application for Identification Card (DD Form 1172) for each employee of the contractor requiring access to Wright-Patterson Air Force Base. The requests shall be submitted to Pass and Registration (Building 10286 WPAFB, OH). The government shall provide a completed "Identification Credential (AFMC Form 87)", which shall be issued, displayed and surrendered as directed in AFI 31-209.
4.5 QUALITY CONTROL PLAN
4.5.1 The contractor shall establish and maintain a complete Quality Control Plan to ensure the requirements of this contract are met. An updated quality control plan must be provided to the Contracting Officer not later than the pre-performance conference and as changes occur. The quality control program is subject to revisions whenever it does not accomplish the objectives.
4.5.2 The program shall include, but not be limited to the following:
4.5.2.1 Inspection System. A description of the inspection system to cover all services in the PWS. This will include the frequency of inspections and the title of the individual(s) who shall perform the inspection.
4.5.2.2 Deficiency Prevention. A method for identifying and preventing defects in the quality of service performed before the level of performance becomes unacceptable.
4.5.2.3 Inspection File Records of all inspections conducted by the contractor and necessary corrective action taken. Documentation shall be made available to the government during the term of this contract. The inspection and corrective action records shall be maintained at a location designated in the Quality Control Plan. The inspection records format shall include; date, time and location of inspection, signature block of the person who performed the inspection, rating of acceptable or unacceptable, area designated for deficiencies noted corrective action take, and total number of inspections.
4.5.3 The contractor shall be required to revise his Quality Control Plan (at no cost to the government) should the plan fail to control the quality of services provided.
4.6 MEETINGS:
The contractor shall attend all meetings convened by the contracting activity or contract administration office in accordance with Federal Acquisition Regulation Subpart 42.5. The CO, QAP/Linen Officer, and other government personnel, periodically will meet with the contractor to discuss the contractor’s performance, and the contractor shall apprise the government of problems. Appropriate action shall be taken by the contractor to resolve outstanding issues. The meetings shall be at no additional cost to the government.
4.7 HOURS OF OPERATION:
4.7.1 The contractor shall perform the services required under this contract during the following hours: Normal Hours. Pick-up and delivery of bulk laundry will be made Monday through Saturday Bldg 830, Area A, between 0700 hours and 1000 hours. Service will be daily with a 24 hour turn around; bulk weight, e.g., items picked up on Monday shall be returned Tuesday. Laundry items shall be picked up and delivered to Bldg. 675 Area B on the First Wednesday of every month or as needed.
4.7.2 The contractor is not required to provide services on the following days: New Year's Day, Martin Luther King Day, Presidents' Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans' Day, Thanksgiving and Christmas Day. NOTE: In the event a holiday occurs on a scheduled pick up or delivery day, pickup and delivery shall be accomplished on the first workday following the holiday.
4.7.3 Unforeseen base closures because of adverse weather does not apply to the Medical Center. Medical Center personnel are mission essential and patient care is 24 hours. Therefore, there can be no delay in pick-up or delivery of laundry due to inclement weather.
4.10 RECORDS:
The contractor shall be responsible for creating, maintaining and disposing of only those government required records that are specifically cited in this SOW. If requested by the government, the contractor shall provide the original record or a reproducible copy of any such record within one (1) working day of receipt of the request.
4.11 ENVIRONMENTAL CONTROLS:
4.11.1 Compliance with Laws and Regulations. The contractor shall comply with all applicable Interstate, Federal, State and Local laws, regulations, and requirements regarding environmental protection. In the event environmental laws/regulations change during the term of this contract, the contractor is required to comply with all changes laws that come into effect during contract performance.
4.11.2 Notification of Spills. If the contractor spills or releases any hazardous substance into the environment within the boundaries of Wright Patterson AFB, the contractor or its agent shall immediately report the incident by calling the Environmental Management Office at 937-257-7297. The contractor shall cleanup any spill to the satisfaction of the base Environmental Management Office (EM) and in accordance with all applicable federal and state laws and regulations. The liability for the spill and any associated clean-up costs shall rest solely with the contractor and/or its agent. In the case of imminent health and safety dangers, the government reserves the right to take immediate action to contain a spill or to otherwise minimize the threat to public health and safety. The contractor shall reimburse the government for the costs incurred upon receipt of a bill from the government. In the event of a spill, the contractor may request support from the base spill team on a cost reimbursable basis.
4.11.3 Infectious Waste. In the event that any items, defined as infectious waste by Ohio Administrative Code (OAC) 3745-27-01(B)(15), are found commingled with the Medical Center's soiled linen, the contractor shall safely place the item in a sharps container. The Medical Center will reclaim and process sharps containers for proper disposal and issue a new container semiannually or as needed. The contractor will contact Facility Management /Housekeeping QAP at 937-257-9076, for all issues related to infectious waste.
4.11.4 Material Storage and Use. The contractor shall follow manufacturer's guidelines and professional recommendations for control of humidity, temperature, cleanliness and materials handling. This includes hazardous materials.
4.12 SAFETY REQUIREMENTS:
4.12.1 The contractor shall conform and comply with safety requirements and provisions contained in the contract for all activities related to the accomplishment of the work.
4.12.2 Take such additional immediate precautions as the contracting officer may reasonable require for safety and mishap prevention purposes.
4.12.3 Include a clause in all subcontracts to require subcontractors to comply with the safety provisions of this contract as applicable.
4.12.4 In the event of an accident/mishap, take reasonable and prudent action to establish control to the accident/mishap scene, prevent further damage to persons or property and preserve evidence until released by the accident/mishap investigative authority through the Contracting Officer.
4.12.5 If the Government elects to conduct an investigation of the accident/mishap, the contractor shall cooperate fully and assist Government personnel in the conduct of investigation until the investigation is completed.
4.12.6 Include a clause in each applicable subcontract requiring the subcontractor's cooperation and assistance in accident/mishap reporting and investigation.
4.13 CONTINUATION OF ESSENTIAL DEPARTMENT OF DEFENSE (DOD) CONTRACTOR SERVICE DURING CRISIS.
4.13.1 All basic services to be performed under this contract have been determined to be essential for performance during crisis and, according to Department of Defense Instruction (DODI) 1100.22 and Air Force implementation thereof, it is determined that the contractor shall be required to perform during crisis. The Contracting Officer or the designated representative shall verbally advise the contractor of the effort required.
4.13.2 Continuation of Contractor Essential Services. In order to ensure continuation of essential contractor services under all circumstances, Base Landry Service is hereby designated as Mission-Essential. There may be times when real world conditions dictate some, or all, contractor services elevated to designation as Mission- Essential; examples (snow removal, hurevac support, elevated FPCONs, etc.). Required support for Wing Plan(s) execution are designated as essential contractor service as defined in Defense Acquisition Regulation Systems (DFARS) 252.237-7023, Continuation of Essential Contractor Services. Contractor shall be responsible for performing those services identified as essential contractor services during crisis situations as directed by the CO per DFARS regulation.
4.13.3 Continuation of Contractor Services During Base Exercises, Local Emergencies or Contingencies, and Implementation of Local Contingency Plans. It is determined contractor shall perform during and/or in support of base exercises, local emergencies, or contingencies and implementation of local contingency plans. Contracting Officer shall notify contractor in the event contractor is relieved from performing any services during base exercises, local emergencies, contingencies or implementation of local contingency plans.
4.13.4 Deployment, Disaster Preparedness, Exercises, Support Plans and Other Contingencies. Contractor shall support activation and exercise of contingency plans and surge requirements. Plans and annexes shall be reviewed and updated as required.
Contractor shall adhere to priorities established by COR when activation or exercise of multiple contingency plans generate conflicting support requirements. Contractor shall not curtail normal daily requirements without prior approval of COR.
