Performance Work Statement for Davis-Monthan 355 CS Furniture Move
355th Communications Squadron Davis-Monthan AFB, AZ
Objective. Provide moving services for the 355th Communications Squadron at building 1540.
1.0. Description of Services. The contractor shall provide all tools, materials, supplies, and equipment transportation, personnel, labor, supervision, and management necessary to perform moving services, at building 1540 to storage boxes, to the Defense Reutilization Marketing Office (DRMO), and potential disposal on Davis-Monthan Air Force Base (DMAFB), See Appendix 1 for estimated items by Room number in accordance with all applicable laws, regulations, standards, instructions, permits, and commercial practices.
1.1. Schedule. Upon contract award, the contractor shall submit a schedule of each phase of the work. The Contractor shall move and assemble all items that need to be moved to building 632. After, the contractor can move all other items to the storage boxes, DRMO, and potential disposal.
1.2. General Information
1.2.1. Contract Manager. The contractor shall designate a primary Contract Manager (CM) who will be responsible for contractor-performed services. The CM shall be always available by phone while contractor or subcontractor employees are at the worksite. Upon contract award, and before commencing work, the contractor shall submit the name and telephone number of the CM, in writing, to the Contracting Officer and the 355 CS point of contact. The CM shall have full authority to act for the contractor on all matters relating to daily execution of the contract.
1.2.2. Contractor Qualifications. All contractor employees shall be certified and qualified to perform moving services. The contractor shall provide proof of employee certification/qualification within 1 business day upon the Government’s request. The contractor is responsible for all code compliance and proper servicing.
1.2.3. English Language. All contractor employees must be able to read, write, speak, and understand English fluently.
1.2.4. Pass and Identification Items. The contractor shall ensure that all necessary pass and identification items required for contract performance are obtained for contractor employees and non-Government owned vehicles.
1.2.5. Traffic Laws. Contractor employees shall comply with all base traffic regulations. Contractor employees are subject to random vehicle speed control checks. Contractor personnel cited for speeding on the installation may suffer loss of base driving privileges, debarment from the base, or other administrative action. The use of seat belts is mandatory for all vehicle occupants. The use of cell phones is prohibited while driving on base, unless the phone is a hands-free cell phone.
1.2.6. Random Personnel and Vehicle Searches. Contractor personnel are subject to random personnel and vehicle searches. If contractor personnel refuse to be searched, they will be denied entry to the base, and may result in loss of base driving privileges, debarment from the base, or other administrative action.

1.2.7. Weapons, Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their contractor owned vehicle or privately owned vehicle while on Davis-Monthan AFB.
1.2.8. Physical Security. Contractor employees shall safeguard all Government property and controlled forms provided for contractor use. At the end of each work period, the contractor shall secure all Government facilities, equipment, and materials. The contractor shall establish and implement methods of ensuring all Government lock combinations, are not lost, misplaced, or used by unauthorized persons. The contractor shall prohibit the use of lock combinations, issued by the Government, by any persons other than the contractor’s employees. The contractor shall also prohibit and the opening of locked areas by contractor employees to permit entrance of persons other than contractor employees engaged in performance of contract work requirements in those areas.
1.2.9. Contractor Personnel Travel on the Installation. All contractor personnel will limit their travel on the installation only to specific areas required for performance of services, specified break and meal areas, or travel directly to and from these locations.
1.2.10. Duty Hours. 355 CS normal duty hours of operation are 7:30 a.m. to 4:30 p.m., Monday through Friday, excluding holidays. Contractor employees shall perform routine maintenance service during normal duty hours of 7:30 a.m. – 4:30 p.m., Monday through Friday, unless otherwise specified, due to mission requirements.
1.2.11. Federal Holidays. The Federal Government observes the following holidays. (Note: If the holiday falls on a Saturday then the holiday will be observed as recognized by the Federal Government). The contractor shall not normally be required to provide routine services on holidays, or the actual day set aside for observation.

1.2.12. Contractor Employee Appearance. Contractor employees shall present a professional, conservative, and neat appearance. Contractor personnel shall report for duty in a professional manner, in appropriate attire befitting the work environment, and have complied with socially acceptable standards of personal hygiene. While performing services, contractor employees shall be neat and clean (free of visible dirt and stains), well-groomed and appropriately dressed. The contractor employee's clothing shall fit correctly to provide a professional, modest appearance, in keeping with normally accepted commercial standards of dress for the work being performed. Contractor personnel shall be easily recognizable while performing services on DMAFB. This shall be accomplished through the wear of distinctive clothing, overcoats, or hats, bearing the company name or logo. The coloring or design of the items selected should be such that identifies personnel easily and quickly for reasons of safety and personal protection.
1.2.13. Communication. Contractor employees shall maintain open, professional, and respectful communication with Government personnel (military and civilian) and other contractor employees. The COR will report valid complaints in writing to the CO for action. Valid complaints will be considered a performance failure. The contractor shall resolve all substantiated complaints immediately upon notification of the complaint. Contractor employees are required to identify themselves as a contractor at all times while on the job, (e.g. in the workplace, when attending meetings, when answering Government telephones, in e-mails, or when making phone calls on behalf of the Government).
1.2.14. Safety. The contractor shall comply with all applicable laws and safety regulations. Compliance with OSHA and other applicable laws and regulations for the protection of employees is exclusively the obligation of the contractor, and the Government will assume no liability or responsibility for the contractor's compliance or noncompliance with such responsibilities.
1.2.14.1. Fire Prevention and Safety. Contractor employees shall comply with OSHA, Davis-Monthan AFB Fire Protection Program, Air Force Occupational Safety, and the Fire Prevention and Health Program.
1.2.14.2. Smoking in Air Force Facilities. The Air Force has placed restrictions on the smoking of tobacco products in Air Force facilities. Air Force Instruction AFI 48-104, Tobacco Free Living, outlines the procedures used by the commander to control smoking in our facilities. Contractor employees and visitors are subject to the same restrictions as Government personnel. Smoking is permitted only in designated smoking areas.
1.2.15. Work Areas. The contractor shall, always, keep work areas free from accumulations of waste material, rubbish, tools, equipment, and materials. Upon completion of the job or when away from the work site, the contractor shall leave the work area and premises in a clean, neat, safe and workmanlike condition. The contractor is responsible for removal and proper disposal of all job-related waste material at a permitted site. Any hazardous waste will be disposed of in accordance with regulatory requirements.
1.2.16. Controlled Unclassified Information (CUI). CUI is unclassified information requiring safeguarding and dissemination controls, consistent with applicable law, regulation, or government-wide policy. In accordance with Department of the Air Force Instruction (DAFI) 16-1403, Controlled Unclassified Information (CUI), the contractor shall protect CUI, regardless of its form, always in a manner that minimizes the risk of unauthorized disclosure while allowing for access by authorized holders. Contractor personnel working with CUI shall be careful not to expose CUI to unauthorized users or others who do not have a lawful government purpose to see it. When not under continuous monitoring or physical control, the contractor shall store hardcopy CUI documents in a locked desk, file cabinet, or similar means, where only authorized personnel have access. The following are examples of CUI:
· Personally Identifiable Information (PII)
· Sensitive Personally Identifiable Information (SPII)
· Proprietary Business Information (PBI)
· Unclassified Controlled Technical Information (UCTI)
· Sensitive but Unclassified (SBU) Information
1.2.17. Reporting Requirements. Contractor employees shall immediately report to an appropriate DMAFB authority any information or circumstances of which they are aware that may pose a threat to the security of Department of Defense personnel, contractor personnel, resources, and classified or unclassified defense information.
2.0. GOVERNMENT SERVICES
2.1. Emergency Services. The Government will provide security forces services (call 228-4444, for emergencies only), and fire protection services (call 228-3333). There are no emergency medical services available on Davis-Monthan AFB, therefore contractor personnel shall call 520-228-3333 for medical emergencies.
