Performance Based Statement of Work
Quality Assurance Surveillance Plan
GENERAL
Provide janitorial services in accordance with the specifications and commercial practices in a manner that will maintain a satisfactory facility condition and present a clean, neat and professional appearance.
LOCATION
Snoqualmie Ranger District, North Bend Compound, 902 S.E. North Bend Way, Bldg. #1, North Bend, Washington.
WORK AREA DESCRIPTION
The work area consists of 5 buildings within the North Bend District compound:
Front Office: Bldg. No. 2031, approximately 3156 square feet
Recreation Office: Bldg. No. 2033, approximately 1200 square feet
Fire/ Law Enforcement Office: Bldg. No. 2032, approximately 1100 square feet
Radio Shop: Bldg. No. 2231, approximately 689 square feet
Conference Center: Bldg. No. 2230, approximately 2500 square feet
Square footage refers to net assignable and related use space, including all restrooms and inside storage areas.

WORK SCHEDULE
Work under this contract shall be accomplished outside of regular office working hours. Regular office working hours are from 6:00 AM to 6:00 PM Monday through Friday; and weekends May through October from 6:00 AM to 6:00 PM Saturday and Sunday. Not all areas will be in use during the weekend period.
CLOSURES AND HOLIDAYS
The Contractor will not be required to perform work on National Holidays, nor the day the Holiday is observed. In the event that a schedule coincides with a National Holiday, the services normally scheduled for that day shall be performed either the day before or the day after the holiday.

In the event services are not provided or required because the facility is closed due to inclement weather, unanticipated holidays declared by the President, failure of the Congress to appropriate funds, etc., the CO shall:
1) Direct the Contractor to perform the work contractually specified over a number of immediately subsequent day(s) equal to the duration of the shutdown, or
2) Forego the work and reduce payment to the Contractor for work not performed. The deduction rate in dollars per day shall be equal to the “per month” fixed price divided by 8 days per month and then multiplied by the number of days services were not provided. Appropriate adjustments will be made by the CO in the event services are provided for portions of days, or
3) Reschedule the work on day(s) satisfactory to both parties.
GOVERNMENT-FURNISHED PROPERTY
The following supplies will be furnished by the Government and provided to the Contractor as needed: Toilet tissue; toilet seat covers; paper towels; plastic garbage bags; hand soap; and light bulbs. The items will be stored and accessible both in the janitorial closets in the Main Office Building No. 2031 and the Conference Center Building No. 2230.
Keys to the offices will be signed out to the Contractor and shall be returned upon termination of the contract. The Contractor shall notify the COR or on-site administrator when Government-furnished supplies are low.
CONTRACTOR FURNISHED PROPERTY AND SERVICES
Provide all labor, supervision, transportation, equipment (including vacuum cleaner), cleaning supplies, i.e., furniture polish, glass cleaner, floor wax, etc. (except for those items provided by the Government), and incidentals necessary to perform the described work. The Contractor will be permitted to store minimal Contractor furnished supplies and equipment in the janitorial closets in the Main Office Building No. 2031 and the Conference Center Building No. 2230 in a manner not hindering entrance to these spaces. The Government shall not be liable for missing or damaged Contractor furnished equipment or supplies stored at the Government facility.
REQUIRED BACKGROUND CHECK AND FINGERPRINTING REQUIREMENTS
This work requires entry into federal government facilities, the successful contractor shall undergo a background check and fingerprinting. (See Attachment C Clauses)
WORK STANDARDS
1. The contractor shall exercise care in work methods to prevent damage to any office equipment, windows, etc.
2. All work under this contract shall be performed in a professional manner. The Contracting Officer may, in writing, require the contractor to remove from work any employee the contracting Officer deems incompetent, careless, or otherwise objectionable for theft, possession and/or removal of materials, supplies, equipment, or any government-owned property, or for unsatisfactory performance.
3. The Contractor shall be responsible for maintaining satisfactory standards of employee competency, conduct, appearance, and integrity, and shall be responsible for taking such disciplinary action with respect to their employees as may be necessary.
4. The contractor shall prohibit their employees from disturbing papers on desks, opening desk drawers or cabinets, or using telephones or office equipment provided for official Government use.
TECHNICAL SPECIFICATIONS
A. Services to be Performed Three Times a Week: (Tuesday, Thursday and Saturday or Sunday)
1. Empty all wastebaskets. Collect all waste paper and trash and place in garbage dumpsters located outside the northwest corner of Bldg. No. 2231, Carpenter Shop.
2. Empty all blue commercial recycling containers (approximately 23 – 30 gallons). Collect all recycling materials and place in the recycling dumpster across from the garbage dumpster located outside the northwest corner of Bldg. No. 2231, Carpenter Shop.
3. Vacuum all floors, including corridors and entrances to the extent necessary to remove litter from floors, and dust from under desks. Spot clean soiled areas on carpet with appropriate cleaner as necessary.
4. Dust all visible services of furniture, fixtures, equipment, walls, window sills and blinds. Use treated dust cloth for all dusting.
5. Wet mop or scrub restroom floors. Wash and disinfect toilets and urinals including fixtures.
Clean toilet paper, paper towel and other dispensers. Clean all sinks, fixtures, restroom walls and stalls with cleaners appropriate for the type of surface being cleaned.
6. Fill toilet paper, toilet seat covers, paper towel and hand soap dispensers in restrooms.
7. Remove finger marks and smudges from walls, woodwork and glass surfaces including inside surfaces of windows and screen doors.
8. Clean and disinfect kitchen sink and countertops in both front office and conference room.
B. Services to be Performed Weekly:
1. Keep walls, ceilings and air vents free of cobwebs.
2. Remove old wax from linoleum floors by mopping or scrubbing with a neutral cleanser, rinse thoroughly and apply one coat of slip resistant water emulsion wax.
C. Services to be Performed Quarterly (at equal three month periods)
1. Wash windows, inside and out, (including storm windows and window tracks), leaving no streaks or unwashed places. Wipe water spots from sills and frames. Lay drop cloths as required to protect adjacent surfaces, fixtures, and furniture. Wash windows at approximately equal intervals of time.
Notify the COR when above has been accomplished for the quarter period.
CONTRACTOR QUALITY CONTROL
A. Contractor Quality Control Plan
The contractor shall develop and implement procedures to identify and prevent defective services.
At a minimum, the contractor shall develop quality control procedures for addressing the performance objectives outlined in the Service Delivery Summary (Table 1).
B. Contractor Quality Control Inspection
In accordance with FAR 52.212-4, Contract Terms and Conditions, the Contractor shall ensure that performance meets contract specifications prior to requesting the Government inspect for acceptance and payment. Upon completion of monthly services, the Contractor shall contact the COR or on-site administrator to set a date for inspection of completed work.
C. Measurement and Payment
Payment shall be made on a monthly basis at the unit bid price per month. If janitorial services begin or end midmonth, payment for that period shall be prorated based on a 30-day month.
D. Inspection and Acceptance
1. Services performed under this contract shall be subject to inspection and acceptance by the Government.
2. The Contracting Officer will furnish contractor with written notice of any item not meeting contract requirements. If deficiency is not corrected immediately, contractor may be considered in default of the contract.
GOVERNMENT QUALITY ASSURANCE SURVEILANCE PLAN (Q.A.S.P)
A. Government Quality Assurance Inspection
The Government will, at its convenience, make periodic inspections of the work to ensure contract compliance and to confirm that the Contractor is performing quality control in accordance with their own established quality control procedures.
B. Acceptance
Acceptance of the work will occur after the above inspections have taken place and the Contracting Officer has determined that the performance objectives have been met. When inspections indicate that performance objectives have not been met, the work will be deemed unsatisfactory and payment will not be made until the contractor has corrected the deficiencies.
C. Quality Assurance Standards
The contractor service requirements are summarized below into performance objectives with measurable thresholds that briefly describe the minimum acceptable levels of service required for each requirement.
Oversight: The contractor is responsible for management and quality control actions to meet the terms of the contract. The role of the government is to ensure these actions are sufficient and consistently implemented. The COR will evaluate the services required by each task to ensure compliance.
Standard: 95% of all facilities are without customer complaints for the month or service period. For the purpose of these 3 service categories, a facility may never have more than two customer complaints.

Performance Objective Performance Threshold Basic Cleaning Services.
Floors, baseboards, corners and wall edges are free of dirt, dust and debris. Trash is empty. Trash containers are free of odors and visible dirt. Trash is emptied into outdoor trash collection container. Glass and mirrors have no traces of film, dirt, smudges, or water. Carpets are free of dirt, debris, litter and other foreign matter and spot cleaned as necessary. Carpets are free of stains and discoloration. Dust is not visible.
95% of all facilities are without customer complaints for the month. Total complaints are 2 or fewer.
Basic Restrooms Cleaning Services.
Restrooms are disinfected and free of dirt, deposits, streaks and odors. Toilets and urinals are disinfected and free of scale, stains, scum and other deposits. Floors are free of litter, dirt, dust and debris. Supplies are adequate until next service. All paper dispensers are filled.
95% of all facilities with rest rooms/locker rooms are without customer complaints for the month. Total complaints are 2 or fewer.
Periodic Cleaning Services.
Entrance door, walls, woodwork and glass surfaces are free of all hand and finger marks. Ceilings and air vents are free of cobwebs. All visible surfaces are free of dust. Old wax is removed from linoleum floors in bathrooms. Windows, sills and frames are free of film, dirt, smudges, water, and other foreign matter.
95% of all facilities receiving periodic services are without customer complaints for the reporting period. Total complaints are 2 or fewer.
COMMENCE WORK AND PERIOD OF PERFORMANCE
1. The Contractor shall be required to commence work under this contract within three calendar days after receipt of the Notice to Proceed by the Contractor.
2. The contract shall become effective February 2025, and shall continue in effect until December 31st 2025 for the base year, unless terminated in accordance with other provisions contained herein. If option year awards are exercised, the contract time shall run from January 1st through December 31st of the following calendar year for each option renewal period.
3. The Contractor shall be required to commence work under this contract within three calendar days after receipt of the Notice to Proceed by the Contractor.
PRE-WORK CONFERENCE
The Contractor shall meet with the Government to discuss contract terms and work performance requirements, Work Progress Schedule, and Fire Prevention and Suppression plans. Reference clause AGAR 452.215-73. Post Award Conference.
The Contractor designated representative or foreman (whomever the Contractor will have on the site) shall be present at the prework conference.
DESIGNATION OF CONTRACTING OFFICER'S REPRESENTATIVE (COR)
A. The Contracting Officer will designate an individual as Contracting Officer's Representative (COR) at time of award.
B. The COR is responsible for administering the performance of work under this contract. In no event, however, will any understanding, agreement, modification, change order, or other matter deviating from the terms of this contract be effective or binding upon the Government unless formalized by a proper contractual documents executed by the Contracting Officer prior to completion of the contract.
C. The Contracting Officer should be informed as soon as possible of any actions or inactions by the Contractor or the Government which will change the required delivery or completion times stated in the contract, and the contract will be modified accordingly. (This does not apply to individual assignments which were issued by the COR provided they do not affect the delivery schedule or performance period stated in the contract.)
D. On all matters that pertain to the contract terms the Contractor must communicate with the Contracting Officer. Whenever, in the opinion of the Contractor, the COR requests effort outside the scope of the contract, the Contractor should so advise the COR. If the COR persists and there still exists a disagreement as to proper contractual coverage, the Contracting Officer should be notified immediately, preferably in writing if time permits.
Proceeding with work without proper contractual coverage could result in nonpayment or necessitate submittal of a contract claim.
E. The COR's major duties and responsibilities are contained in the Forest Service Handbook of Contract Administration. The COR is delegated full authority under this contract except for the following actions which are reserved for the Contracting Officer:
1. Approve Change Orders and Modifications
2. Take action to terminate the contract for default or Government convenience.
3. Make contract adjustments under the Differing Site Conditions clause.
4. Grant extensions of contract time.
5. Approve assignment of claims.
6. Make final decisions under the Disputes clause.
7. Make final acceptance under the contract.
8. Make final decisions under the Suspension of Work clause.
9. Make equitable adjustments.
10. Authorize change in amount of Payment Retention.
11. Approve subcontractors.
12. Approve payments with deductions or final payment.
13. Approve use and possession prior to completion.
14. Enforce the warranty provisions.
PERMITS AND RESPONSIBILITIES
1. The Contractor shall be responsible for all damages to persons or property that occurs as a result of the Contractor’s fault or negligence. The Contractor shall take proper safety and health precautions to protect the work, the workers, the public, and the property of others during the performance of the work.
2. After each period of cleaning or other service, the Contractor shall inspect the area for fire hazards, turning off electric fans and unnecessary lights. Outside doors and windows shall be closed and locked. The Contracting Officer will designate any lights, which are to remain on for security.
USE OF PREMISES:
The Contractor shall comply with the regulations governing the operation of premises, which are occupied and shall perform the contract in such a manner as not to interrupt or interfere with the conduct of Government business. Only the Contractor or the Contractor’s designated representative will be permitted on the premises during the performance of the services for this contract. No children will be allowed to accompany custodians while in the performance of their work.
