About this file

This document is a Past Performance Questionnaire Template used for evaluating contractor performance on federal contracts.

The template is designed for use in Solicitation 1305M326Q0325 for DNA Next Generation Sequencing Testing services requested by NOAA's Atlantic Coastal Fisheries Oceanography Program (ACGO) on behalf of the Northeast Fisheries Science Center (NEFSC). The questionnaire is completed in four sections: Sections I and II are completed by the offeror and major subcontractors, then provided to a government assessor who had prior experience with the contractor. The assessor verifies the information and completes Sections III and IV. Offerors must submit copies of Sections I and II with their proposals by the solicitation closing date. Section II requires validation of contract information including contractor name and address, contract number, contract type, initial and final contract values, period of performance, project description, contracting role (prime or subcontractor), and termination history. Section III captures assessor information including name, title, contact information, and the assessor's role in the contract award or administration. Section IV requires an overall performance rating based on contract requirements fulfillment, quality and timeliness of delivery, cost management, communications, and customer satisfaction. Five rating options are provided: Exceptional (exceeds most requirements), Very Good (exceeds some requirements), Satisfactory (meets requirements), Marginal (meets some requirements), and Unsatisfactory (does not meet requirements), with an additional N/A option for non-recent or non-relevant information.
