About this file

This is a Past Performance Questionnaire template used by the U.S. Department of Veterans Affairs, Network Contracting Office (NCO) 02 to evaluate contractor performance on current or previous contracts.

The questionnaire is designed to assess contractor performance across four primary evaluation categories: Quality of Service (5 criteria), Timeliness of Performance (4 criteria), Business Relations (10 criteria), and Management of Key Personnel (5 criteria). Respondents must rate contractors using a six-point scale: Exceptional (E), Very Good (VG), Satisfactory (S), Neutral (N), Marginal (M), and Unsatisfactory (U). The questionnaire requires completion of contractor identification information, including the contractor name, address, and point of contact, along with specific contract data such as contract number, contract type, award date, period of performance, total contract dollar amount including options, and a brief scope description detailing services performed to determine relevancy and recency. Respondents may select "N/A" if unable to provide ratings and are required to provide written explanations for any Exceptional, Marginal, or Unsatisfactory ratings. Completed questionnaires must be returned via email to Tiffany Vazquez-Simon, Contract Specialist, at the FDR Hospital – Montrose Campus in Montrose, New York, no later than the solicitation submission due date. The template concludes with a final recommendation question asking whether the respondent would recommend the contractor for similar requirements in the future, along with signature blocks for the respondent's name, title, signature, and phone number.
