About this file

This document is a Past Performance Questionnaire for Reference Laboratory Testing Services, issued as part of an RFQ process. The form is designed to capture detailed performance feedback from previous customers, with rating categories including transmission/connectivity to IT systems, IT support quality, startup time, test menus, turn-around times, specimen evaluation, critical result metrics, specimen pick-up time, and continuing education materials. The questionnaire uses a four-tier rating system (Superior, Satisfactory, Unsatisfactory, and Neutral) to evaluate vendor performance, and includes sections for identifying contract details, assessing strengths and weaknesses, and determining whether the customer would award another contract to the vendor. The form is intended to provide comprehensive insight into a vendor's past performance to support future contract award decisions in reference laboratory testing services.
