About this file
This Past Performance Questionnaire is for a National Park Service contract (Solicitation #140P5425R0014) to replace fuel system lines and components at Dry Tortugas National Park. The document is designed to collect detailed performance feedback from a previous contractor reference, with a submission deadline of 1:00 p.m. ET on August 04, 2025, to Contracting Officer William Vazquez via email.
The questionnaire includes comprehensive evaluation sections rating the contractor's performance across multiple dimensions: overall performance ability, timeliness, ability to resolve issues, quality of work, and an overall performance rating. Evaluations use a six-point scale ranging from Exceptional to Unsatisfactory, with space for narrative comments. The form requires the reference to provide specific details about the contract, including contract number, value, period of performance, and a brief description of items provided, allowing for a holistic assessment of the contractor's past performance.
