About this file
This is a Past Performance Questionnaire designed to evaluate contractor performance for federal contract award consideration. The questionnaire is specifically tied to Request for Quote (RFQ) W91364-21-Q-0011 and targets offerors in the Perishable Prepared Food Manufacturing industry (NAICS code 311991). Offerors must submit information on a maximum of four current and relevant past performance contracts, where "current" is defined as performance within the three years preceding the solicitation release date, and "relevant" means work of similar scope, magnitude, and complexity to the Statement of Work requirements.
The questionnaire requires evaluation across five primary performance areas: conformance of supplies and services to contract specifications; effectiveness of project management and subcontractor control; timeliness of performance and deliverables; small business utilization in accordance with FAR 52.219-8; and commitment to customer satisfaction. Evaluations are scored using a four-tier rating scale (Substantial Confidence, Satisfactory Confidence, Limited Confidence, and No Confidence), with detailed definitions provided for each rating level. The questionnaire is completed in two parts: Part I captures contract identification details completed by the offeror, and Part II requires evaluation responses with mandatory rationales from the point of contact. The Government reserves the right to contact references independently to verify contractor performance and seek additional information regarding the identified evaluation topics.

Performance Assessment Questionnaire
Please provide your candid responses. The information that you provide will be used in the awarding of federal contracts. Therefore, it is important that your information be as factual, accurate and complete as possible to preclude the need for follow-up by the evaluators. If you do not have knowledge of or experience with the company in question, please forward this Questionnaire to the person who does and notify the Contracting Officer.
In accordance with FAR Provision 52.212-2, Basis for Award, offerors should identify current and relevant past performance under North American Industry Classification System (NAICS) code 311991, Perishable Prepared Food Manufacturing, to assist the Government in ascertaining probability of successful completion of the requirements of the Statement of Work (SOW) for Request for Quote (RFQ) W91364-21-Q-0011.
As part of the past performance evaluation, it is requested that offerers provide contract information about its current and relevant for evaluation. The term “Current” means performance occurring within the last three (3) years for the period beginning three years prior to the solicitation release date. “Relevant” – means similar scope, magnitude of effort, and complexity of work to the current Statement of Work (SOW). A maximum of four current and relevant contracts will be reviewed.
In reviewing past performance, the Government will make attempts to contact the above identified contacts to ask for a rating and rational to the following topics:
1. Conformance of supplies / services to contract requirements.
2. Effectiveness of Project Management, including use of subcontractors.
3. Timeliness of performance and deliverables.
4. Commitment to customer satisfaction.
5. Any other relevant performance information.
Rating Definitions:
Substantial Confidence: Performance meets contractual requirements and exceeds many requirements that benefit the end user. Work was accomplished with few, if any, minor problems for which corrective actions taken by the contractor were highly effective. The offeror has been highly successfully in performing the required effort.
Satisfactory Confidence: Performance meets contractual requirements and exceeds some requirements that benefit the end user. Work was accomplished with some minor problems for which corrective actions taken by the contractor were effective. The offeror has successfully performed the required effort.
Limited Confidence: Performance does not meet some contractual requirements. Serious problems with contractor performance were experienced for which the contractor has either not yet identified corrective actions or the corrective actions taken appear only marginally effective. The offeror has had little success performing the required effort.
No Confidence: Performance does not meet most contractual requirements. Serious problems with contractor performance were experienced for which the corrective actions were ineffective. The offeror has not successfully performed the required effort.
