About this file

The document is a Past Performance Questionnaire (PPQ) from the U.S. Department of the Interior, National Park Service for evaluating a contractor's performance history. The form is designed to gather detailed feedback on a company's past project performance, with Section A to be completed by the offeror and Section B to be completed by the project owner or evaluator. The questionnaire requires submission of 3-5 past performance projects completed within the last five years, with ratings ranging from Outstanding (O) to Unacceptable (U) across nine specific evaluation categories including client relationship, corporate management, quality control, and performance schedule.

The associated federal contract opportunity is Solicitation Number 140P6425Q0065 for a "Window Restoration - Proof of Concept" project with the Department of the Interior National Park Service Midwest Region. The solicitation's most recent amendment extends the close date to 7-23-25 at 12:00 PM Eastern Time. The PPQ provides a structured mechanism for potential contractors to demonstrate their previous performance capabilities, which will likely be a key factor in the evaluation and potential award of the window restoration project.
