PAST PERFORMANCE QUESTIONNAIRE

INSTRUCTIONS FOR COMPLETING PAST PERFORMANCE QUESTIONNAIRE

1. The Department of the Army requires past performance information as part of an evaluation process for a current solicitation. On the following page, "offeror" refers to the firm using your activity as a reference. Please complete this questionnaire based on the following guidance. Handwritten responses are sufficient. Your input is important to us and responses are needed for inclusion in our evaluation.

2. Indicate, based on the description below, the offeror’s performance for your activity. Assessments should reflect only performance for which the offeror is liable. The following is a definition of the rating levels.

Exceptional:

Indicates the offeror’s performance within the area of evaluation clearly fully meets or exceeds contractual requirements and does so in a way that provides very low risk, and high probability of success.

Good:

Indicates the offeror’s performance is of high quality in most respects, good probability of success; generally exceeds minimum objectives/requirements; some improvement possible.

Acceptable:

Indicates no problems exist in area of evaluation, or has only minor problems for which solutions are at hand.

Marginal:

Fails to marginally meet some requirements; Indicates the area of evaluation contains an existing problem for which there is doubt whether the identified solution is adequate, but the problem appears to be within the offeror's ability to solve.

Unsatsfactory:

Indicates a serious problem exists in area of evaluation, which may be outside the offeror’s ability to solve. The offeror is in danger of not being able to satisfy contractual requirements and timely recovery is not likely.

3. Please circle a rating for each area.

4. Please provide narrative explanations for your answers. Space for your remarks is provided after each area. Ensure your narrative remarks are consistent with the assigned rating. If more space is needed, continue on plain bond paper.

5. In addition to completing the attached questionnaire, please provide comments on any other programs for which your activity has contracts with this offeror. You are urged to supplement your own knowledge of the offeror’s performance with the judgment of others in your organization.

6. This solicitation requires the offeror to submit evaluation as a part of their proposal; therefore, once completed, this Past Performance Questionnaire should be emailed directly to:

Jessica Wilkerson, Contracting Officer Phone: 919-549-4336 Email: Jessica.wilkerson2@us.army.mil
