About this file
This is a Past Performance Questionnaire template used by the U.S. Department of Veterans Affairs, Network Contracting Office (NCO) 02, for evaluating contractor performance on federal contracts. The questionnaire is designed for official use only and applies to solicitation 36C24226Q0940.
The questionnaire collects contractor information including name, address, and point of contact, along with contract details such as contract number, type, award date, period of performance, total dollar amount including options, and scope of services. Respondents must provide this information to avoid rendering the proposal unresponsive. The evaluation uses a six-point rating scale: Exceptional (E), Very Good (VG), Satisfactory (S), Neutral (N), Marginal (M), and Unsatisfactory (U). Ratings of Exceptional, Marginal, or Unsatisfactory require written explanations. The questionnaire assesses contractor performance across four primary categories: Quality of Service (five evaluation areas), Timeliness of Performance (four areas), Business Relations (six areas), and Management of Key Personnel (five areas). All completed questionnaires must be returned no later than the solicitation submission deadline via email to Tiffany Vazquez-Simon, Contract Specialist at the FDR Hospital – Montrose Campus in Montrose, New York. The form concludes with a recommendation section asking whether the respondent would recommend the contractor for similar requirements in the future.
