About this file

This is a Past Performance Questionnaire template for the Santa Cruz, CA Hazardous Waste Removal Services contract (Solicitation Number 1305M326Q0489) issued by the Department of Commerce National Oceanic and Atmospheric Administration (NOAA). The questionnaire is designed to collect performance history and evaluation data on contractors and major subcontractors who have previously performed similar work on relevant contracts.

The document contains four sections: Section I collects solicitation data identifying the project as Santa Cruz Lab Hazardous Waste Removal Services; Section II requests the offeror to provide current or historical contract information including contractor name and address, contract number, contract type, contract value, period of performance, and termination history, which is then verified by an assessor; Section III identifies the assessor's role in contract administration (such as Procuring Contracting Officer, Administrative Contracting Officer, Contracting Officer's Representative, or other relevant positions) and their period of involvement; and Section IV requires an overall performance rating using a five-point scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, or Not Applicable) based on contract requirement performance, quality and timeliness of delivery, cost management, communications, and customer satisfaction. Completed questionnaires must be submitted to the contracting point of contact (jenna.taulman@noaa.gov) by the solicitation closing date.
