About this file
The document is a Past Performance Evaluation Questionnaire designed to assess a contractor's performance across multiple dimensions for a federal contract. The questionnaire provides a comprehensive rating system using an adjectival scale from Unacceptable (U) to Outstanding (O), evaluating key performance areas including quality of service, recruitment/retention, timeliness/schedule, business relationships, customer satisfaction, and overall contract performance. Specific evaluation criteria include the contractor's ability to provide services across multiple sites, handle contingency support, recruit qualified personnel, maintain effective communication, and meet contractual requirements.
The questionnaire is associated with Solicitation Number 36C24225Q0699 for H166--NYH Industrial Hygiene Services, which is set aside for Service-Disabled Veteran-Owned Small Businesses (SDVOSBs) through the Department of Veterans Affairs Veterans Health Administration Veterans Integrated Service Network 2. The form allows the client to provide detailed feedback on the contractor's performance, including whether they would award another contract, if the contract was terminated for default, and if any cure notices or suspensions were issued. The document is intended to be used as part of the past performance assessment in a federal contract award process.
