About this file

This is a Mold Assessment Report prepared by Barclay Assessment Services for the U.S. Department of Veterans Affairs regarding the St. John Outpatient Clinic located at 4004 Airline Highway, Reserve, Louisiana.

The assessment, conducted on October 16, 2025, identified significant mold contamination in four rooms (128, 129, 132, and 141), with airborne spore concentrations reaching up to 80 times higher than outdoor reference levels. Source samples confirmed the presence of Chaetomium, Stachybotrys, Aspergillus/Penicillium, Trichoderma, and Cladosporium species—all indicative of water-damaged materials and active mold growth. Ambient measurements revealed relative humidity levels ranging from 47% to 58% and temperatures from 68°F to 79°F across various areas. The assessment documented four HEPA-filtered air scrubbers on site, though only one was operational at the time of inspection, supplemented by plastic sheeting barriers at select entryways and HVAC vents.

The report recommends prompt implementation of engineering controls including roof leak repairs and inspection for additional hidden water sources, coupled with immediate administrative controls requiring staff relocation from Room 141, access restriction to affected rooms, and posting of biohazard warning signs. Comprehensive remediation following IICRC S520 standards is specified, with post-remediation verification sampling required, along with installation of critical barriers with zipper doors at impacted room entryways and sealing of HVAC vents and returns until remediation is complete. The assessment was prepared for Erik Hadwin of the U.S. Department of Veterans Affairs under Project ID 811124 and Purchase Order 629-6Q0312.

Mold Assessment Report – St. John Outpatient Clinic

Executive summary

The mold assessment at St. John Outpatient Clinic confirmed significant contamination in four rooms (128, 129, 132, and 141). Elevated spore counts were detected indoors at concentrations up to 80 times higher than outdoor reference levels. Species identified (Chaetomium, Stachybotrys, Aspergillus/Penicillium, Trichoderma) are consistent with moisture damage and active mold growth. Prompt engineering controls, remediation and strict access controls are recommended.

Background On October 16, 2025, Barclay Assessment Services performed an indoor mold assessment at the St. John Outpatient Clinic, requested Erik Hadwin with the U.S.

Department of Veterans Affairs. The purpose was to inspect for mold, sample suspect materials, evaluate control measures, and provide recommendations.

Summary of findings

The following is a summary of the findings:

• Mold growth was confirmed in certain rooms on drywall above ceilings, wall and base cabinets, contents and furnishings.

• Source samples confirmed Chaetomium, Trichoderma, Stachybotrys, Cladosporium, Aspergillus, and Aspergillus-Penicillium like mold, all associated with water-damaged materials.

• Airborne Aspergillus/Penicillium spore concentrations in Room 128 and 132 were well above the outdoor reference levels, as was Trichoderma in Room

132.

• Rooms 128, 132, and 141 showed significantly elevated mold levels compared to unaffected spaces.

Discussion of control measures

At the time of assessment, four HEPA-filtered air scrubbers were present but only one was in operation, limiting effectiveness. Plastic sheeting barriers were installed at select entryways and HVAC vents. While these steps reduced potential cross-contamination, they were not adequate given the observed mold growth and airborne concentrations.

Recommendati ons

Additional control measures are needed to be promptly employed as follows:

1. Engineering and Preventative controls:

• Repair roof leaks to eliminate ongoing moisture intrusion.

• Inspect for additional hidden water sources (e.g. Plumbing, HVAC).

2. Administrative controls

• Relocate staff, if any, from Room 141 immediately

• Restrict access to Rooms 128, 129, 132, and 141.

• Post biohazard warning signs at entryways.

3. Remediation (Removal)

• Develop a comprehensive remediation plan following IICRC S520.

• Plan should define scope of work, removal methods, PPE, and clearance criteria.

• Conduct post-remediation verification sampling to confirm effectiveness.

4. Isolation

• Install critical barriers with zipper doors at all impacted room entryways.

• Seal HVAC vents and returns until remediation is complete.

Methods The following describes the methods used to assess mold during the mold inspection. Sampling for fungal contaminants was done in accordance with methods derived from “Bioaerosols: Assessment and Control”, published by the ACGIH, 1999. The air and source samples were analyzed via light microscopy at 600X magnification. The air samples were analyzed over 100% of the trace. These results are reported as total, meaning they include both viable and non-viable fungal spores.

However, please note that the scope of the inspection was limited and not exhaustive or intrusive, and included only the specified accessible and observable areas.

Consequently, sources of mold or water intrusion may be present that were not detected or identified.
