About this file
This is a Single Source Justification document for a procurement conducted under the Department of Veterans Affairs (VA) Simplified Acquisition Threshold (SAT) procedures.
The James A. Haley Veterans' Hospital, located within VA VISN 8, NCO 08 (2237 number: 673-26-3-023-0602), is procuring a ThinPrep T5000 Processor for automated slide preparation. The equipment includes automated capping and de-capping of ThinPrep vials, chain of custody verification, and direct slide output into stainer-ready racks. This device is essential for Pap smear preparation and cytologic screening for cervical cancer, atypical cells, and precursor lesions as defined by The Bethesda System. The VA asserts that no other automated processor can meet the required level of quality and throughput. The sole source justification cites two primary factors: an emergency condition created when the assigned Contracting Officer left the organization without sufficient time to execute a longer-term procurement solution, and the highly specialized and certified sole source nature of the requirement. The justification emphasizes that delay in award presents unacceptable risk to clinical operations and direct patient care for veterans. Hologic has certified in writing that they are the sole proprietary manufacturer and domestic distributor of the required ThinPrep systems. Market research was conducted via SAM.gov, confirming these requirements as sole sourced to Hologic. The Contracting Officer has certified the justification as accurate and complete in accordance with RFO FAR 12.102(a).

Justification for Single Source Awards IAW RFO FAR 12
Acquisition Plan Action ID: 36C248-26-AP-3075
1. Contracting Activity: Department of Veterans Affairs, VISN 8, NCO 08, James A.
Haley Veterans’ Hospital. The 2237 number is 673-26-3-023-0602.
2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount: ThinPrep T5000 Processor, fully automated slide preparation, including automated capping and de-capping of ThinPrep vials, chain of custody verification and slides directly output into stainer-ready racks. No other automated processor can meet the level of quality and throughput as the ThinPrep® T5000 processor.
The estimated amount is:
3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used:
An urgent and compelling need exists and following the ordering procedures would result in unacceptable delays: The Contracting Officer assigned to this requirement left the organization and subsequently there was not enough time to execute a longer-term solution. This created an emergency need for immediate support to this requirement. This has an immediate and significant impact on operations and presents unacceptable risk.
IMMEDIATE IMPACT TO OPERATIONS: The Thinprep systems are the only conventional method of Pap smear preparation for use in screening for the presence of atypical cells, cervical cancer, or its precursor lesions (Low-grade Squamous Intraepithelial Lesions, High-grade Squamous Intraepithelial Lesions), as well as all other cytologic categories as defined by The Bethesda System for Reporting Cervical/Vaginal Cytologic diagnoses. Failure to implement this critical support in the most expeditious manner results in unnecessary risk to life, health, and safety for the care of patients. This prevents an unacceptable risk to direct patient care, diagnosis, and treatment.
This purchase is limited to only one source due to the highly specialized and certified sole source nature of the requirement items. This procurement is also conducted as an emergency as delaying an award presents unacceptable risk to the clinical operations of the James A. Haley Veterans hospital affecting the lives and health of veterans receiving care from the hospital.
