About this file

This document is a Single Source Justification for a Simplified Acquisition Procedure (SAP) for the Department of Veterans Affairs, VISN 2 Albany Stratton VHA Medical Center. The procurement is for a VIP Glenoid Reaming System set, estimated at $19,104.16, to support orthopedic surgical procedures specifically for total and reverse shoulder arthroplasty. The justification emphasizes the unique characteristics of the Arthrex system, which uses proprietary preoperative planning software and hardware that are not available from third-party vendors and are critical for precise surgical implant placement.

Market research was conducted across multiple platforms (GSA, FPDS, SBA) to validate the single-source requirement, revealing no equivalent alternative products compatible with existing Arthrex surgical platforms at the Albany VAMC. The document details that the VIP Glenoid Targeter allows surgeons to execute a preoperative virtual plan for glenoid component placement by transferring the planned guide-pin trajectory into the patient's anatomy intraoperatively. The acquisition is justified under FAR 12.102 for a single-source award over the micro-purchase threshold but not exceeding the Simplified Acquisition Threshold (SAT) of $350,000, with an effective date of 11/15/2024.

VHAPG Part 813.106 Simplified Acquisition Procedures: Soliciting from a Single Source Attachment 1: Single Source Justification for SAP under the SAT

Justification for Single Source Awards IAW FAR 12.102 For

Over Micro-Purchase Threshold but Not Exceeding the SAT ($350K)

Acquisition Plan Action ID: VIP Glenoid Reaming System set- 36C24226AP0916

1. Contracting Activity: Department of Veterans Affairs, VISN 2 Albany Stratton VHA Medical Center

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:

The Albany VA Medical Center requires a Glenoid Reamer System to support orthopedic surgical procedures, specifically total and reverse shoulder arthroplasty.

This system is necessary to maintain continuity of patient care, ensure compatibility with existing Arthrex implants, and meet current standards of surgical practice.

The facility currently performs a high volume of shoulder arthroplasty cases. Existing Arthrex implant systems are in use, and the Arthrex Glenoid Reamer System is the only instrument set compatible with these implants. The reamer allows precise preparation of the glenoid surface, which is critical for accurate implant placement, stability, and long-term surgical success. Without this system, surgeons cannot safely or effectively perform these procedures with the implants already in inventory.

The VIP Glenoid Targeter is a surgical instrument that allows the surgeon to execute a preoperative virtual plan for glenoid component placement by transferring the planned guide-pin trajectory into the patient’s anatomy intra-operatively. It supports accurate downstream glenoid preparation and implant insertion for the specific Arthrex systems named in its indications. Estimated dollar amount is $19,104.16.

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: The VIP system uses patient CT data and proprietary preoperative planning software (OrthoVis/VIP Web Portal) to define optimal glenoid implant orientation, positioning, and guide-pin trajectory. The Targeter has adjustable “legs” (feet) that engage the glenoid surface or patient-specific anatomy, aligning the instrument in the exact planned orientation. Once positioned, a central guide pin is inserted through the targeter into the glenoid — that pin then serves as the starting point for glenoid preparation (reaming, implant insertion).

This system incorporates patented Arthrex hardware and software that are not available from third party vendors. The facility already uses compatible Arthrex equipment for surgical procedures making for a seamless integration.

4. Description of market research conducted and results or statement why it was not conducted: No equivalent or substitute products exists that provide the same level of compatibility, integration and functionality with Arthrex surgical platforms. These applications and software are already in place and running in the Albany VAMC for surgical procedures.

GSA was searched for “Glenoid Reaming Set,” with no results returned.

GSA was searched for the term “VIP Glenoid Reamer Set,” and no results were populated.

GSA was searched for the term “Glenoid Reamer,” and returned 3 pages of results and one manufacturer: FX Shoulder Solutions. Their software is not compatible with existing software and implants currently in use in the VA Albany system.

FPDS was searched for “Glenoid Reaming Set,” and one vendor and past contract was located with Arthrex.

SBA was searched for the term “VIP Glenoid Reaming Set,” and 4 small business results were returned in conjunction with NAICS code 339113. None of these results are compatible with Arthrex systems already in place at the Albany VAMC.

SBA was searched for the term, “Glenoid,” and 0 small businesses were populated.

5. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief. .
