JUSTIFICATION FOR OTHER THAN FULL AND OPEN COMPETITION
This is a Justification for Other Than Full and Open Competition document (Control Number VA-26-00024506) issued by the Department of Veterans Affairs, Office of Procurement, Acquisition, and Logistics, Technology Acquisition Center. The VA seeks to award a firm-fixed price contract for a Veterans Integrated Services Network (VISN) wide software license for Imagine Systems Inc Radiology (ISI Rad) client software application. The estimated total contract value is $358,000.00 with a base year and two 12-month option periods. The action will be issued as an open market solicitation via SAM.gov's Contract Opportunities page.
The justification for sole-source procurement is based on the unique technical requirements that only ISI Rad can satisfy. The software must replace the existing VistARad application, whose image manipulation toolkit will no longer be available after December 31, 2022. ISI Rad is the only Commercial Off-The-Shelf (COTS) solution that integrates with both VistA and the Indian Health Service (IHS) Resource Patient Management System (RPMS), recognizes and manipulates DICOM images using C++ language, and performs required verification, validation, and patching without development effort. The VA's technical experts conducted continuous market research from 2013 through November 2025, evaluating alternative products including Laurel Bridge DICOM Connectivity Framework and DCM4CHE toolkits, determining that no alternatives meet all functional requirements. Failure to procure ISI Rad would require either over $1 million and more than one year of internal development or procurement of an alternative solution, both of which would severely disrupt radiology operations and delay patient care. The procurement is authorized under 41 U.S.C. 3304(a)(1) and FAR Deviation 6.103-1.

JUSTIFICATION FOR OTHER THAN FULL AND OPEN COMPETITION
1. Contracting Activity: Department of Veterans Affairs (VA) Office of Procurement, Acquisition, and Logistics Technology Acquisition Center (TAC) 23 Christopher Way Eatontown, NJ 07724
2. Description of Action: This proposed action is for the award of a firm-fixed price contract for a Veterans Integrated Services Network (VISN) wide software license for brand name Imagine Systems Inc Radiology (ISI Rad) client software application. This action will be issued open market via Contract Opportunities at SAM.gov.
3. Description of Supplies or Services: VA requires a Commercial Off The Shelf (COTS) application that can meet all functional requirements currently met through use of the Government’s Veterans’ Hospital Information System Technology and Architecture Imaging system (VistARad) application and integrates with VistA. Specifically, the required COTS application must have the ability to recognize and manipulate Digital Imaging and Communications in Medicine (DICOM) images using C++ language in order to ensure integration with VistA. This ensures viewing capability to maintain workflows for storage, querying and retrieving. VA also requires that the application integrates with Indian Health Service (IHS) Resource Patient Management System (RPMS) in order to ensure interoperability for patients in the IHS community (Veteran and non-Veteran). The proposed action is to obtain a single VISN wide software license for ISI Rad client application in order to support continued Veterans Health Administration (VHA) radiology operations for diagnostic interpretations of medical imaging procedures. The period of performance is a base year with two 12-month options periods. The total estimated price inclusive of all option periods is $358,000.00.
4. Statutory Authority: The statutory authority permitting other than full and open competition is 41 U.S.C. 3304(a)(1) as implemented by the Federal Acquisition Regulation (FAR) Deviation 6.103-1 entitled, “Only One Responsible Source and No Other Supplies or Services Will Satisfy Agency Requirements.”
5. Rationale Supporting Use of Authority Cited Above: The image manipulation toolkit used in the VistARad application, developed and owned by the Government, will no longer be available after December 31, 2022. The Government, after reviewing similar image toolkits and similar software, has determined that ISI Rad application is the only Commercial Off The Shelf (COTS) solution that can meet all of VA’s functional requirements. Specifically, ISI Rad is the only software that integrates and interoperates with VistA and the IHS RPMS Radiology software and has ability to recognize and manipulate DICOM images using C++ language, which allows for integration with VistA without any development effort. Without the procurement of ISI Rad as a replacement for VistARad, there will not be a diagnostic tool in place for VHA Radiologists within VISN 7. This would severely impact VA’s mission to provide primary diagnostic interpretations of medical imaging procedures for Veterans and the IHS community, thus delaying patient care and negatively impacting patient safety. If ISI Rad is not
ISI Rad License Control Number VA-26-00024506 procured, VA would have to procure an alternative software or internally develop a non- COTS solution, which is estimated to require over a year of development and over $1M to duplicate functionality and interfaces that exist in the COTS ISI Rad application. This estimate of duplicative effort is based on similar clinical development initiatives.
Furthermore, existing clinical workflows for exam review by VHA Clinicians, Radiology Technicians and others would not occur, also negatively impacting the timely delivery of Veteran patient care. This acquisition will enable the user community in VHA to procure a replacement application that is the only commercially available solution.
6. Efforts to Obtain Competition: Market research was conducted, details of which are in the market research section of this document. This effort did not yield any additional sources that could meet the Government’s requirements. There is no competition anticipated for this acquisition. Additionally, the proposed action will be synopsized on the Contracting Opportunities Page in accordance with FAR 5.101.
Any proposals that are received shall be evaluated.
7. Actions to Increase Competition: The Government will continue to conduct market research to ascertain if there are changes in the marketplace that would enable future actions to be fully competed.
8. Market Research: The Government’s technical experts conducted market research by reviewing replacement image software toolkits as well as similar software. This market research is an ongoing process starting in 2013 and continuously being conducted, as recently as November 2025. Specifically, VA medical imaging Subject Matter Experts regularly review industry trade publications and conduct web-based research to ascertain if alternative brand name software products are available. Other similar software reviewed included the Laurel Bridge DICOM® Connectivity Framework (DCF) and DCM4CHE DICOM toolkits as well as Picture Archiving and Communication Systems (PACs). Based on all of these market research efforts, the Government’s technical experts have determined that only ISI Rad brand name software can meet all of VA’s functional requirements. No alternative products provide interoperability with VA’s VistA application or the IHS RPMS, and none provide DICOM imaging capability.
The alternative products would require development in order to integrate with VistA due to the type of coding language used. Further, no other company can do the verification, validation and patching required to ensure the application can be used within the VA VistA and IHS RPMS environments using the ISI name space in VistA or avoid the duplication of time and effort as outlined in Section 5 of this justification.
9. Other Facts: None.
