Acquisition Plan Action ID: 36C248-24-AP-4780

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:
The Lakleand VA Community Based Outpatient Clinic (CBOC) has a requirement for Reichert Tono- Pen AVIA Handheld Tonometers and accessories (BRAND NAME ONLY) to include a Black Illuminated Phoroptor and Handheld Pachymeter. The equipment will be used to provide optometry care to veterans in the local community.
Estimated cost is $56,520.00
3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used:
The Reichert AVIA Tono-Pen, Reichert Black Illuminated Phoroptor and Reichert Handheld Pachymeter currently outfits the following VA Tampa Helthcare Network Optometry Clinics: James A. Haley Veterans’ Hospital and New Port Richey VA Clinic. Optometry Clinicians rotate to the Lakleand VA CBOC frequently to ensure personnel coverage and continuing standardization of equipment at each facility is necessary to ensure and enhance patient safety and the quality of care to Veteran patients. This enables a streamlined effort by Optometry Clinicians utilizing the same equipment to reduce patient misdiagnosis. Additionally, the Biomedical staff throughout the Tampa VA Healthcare network are familiar with repair of Recheirt ophthalmic instruments, which prevents long lead times in equipment repair and increases repair accuracy.
4. Description of market research conducted and results or statement why it was not conducted:
Market research revealed four (4) contracts awarded to three (3) different SDVOSB Vendors for Tono-Pen AVIA Handheld Tonometers since 2021. Additional research by the Program Manager revealed a total of 48 Reichert brand ophthalmic instruments procured for the Tampa VA Healthcare Network since 2009. IAW VAAR 819.7006 there is reasonable expectation that quotations will be received from two or more eligible VIP-listed SDVOSB vendors and the VA Rule of Two has been satisfied. Therefore, this requirement will be solicited Open Market as an SDVOSB set-aside
5. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
