II. Scope of Work
Project Title: Housing, Expungement and Recovery through Treatment & Support Services (HEARTSS) Program
The Scope of Work is a general guide and is not a complete list of all work necessary to complete the project. Successful proposals will address the following core elements for each of the Program components for all served populations, unless otherwise specified below.
The funding for these services was awarded through the BSCC Proposition 47 Cohort 4, Grant Program, which supports programs for housing assistance, substance use disorder and mental health treatment, job training, and civil legal services in California. The Department of Homelessness and Supportive Housing is the lead agency for cohort 4. Services provided through cohort 4 will augment the City’s prior Proposition 47 cohort grant programs with a greater focus on housing, problem solving, and cultural congruence. The overall goal of the program is to reduce incarceration and recidivism by advancing stability through housing and recovery.
The purpose of the grant is to provide Housing, Expungement and Recovery through Treatment & Support Services (HEARTSS) program services to criminal justice-involved adults with substance use disorder and/or mental health issues, including but not limited to Latine and Spanish monolingual clients. The project will achieve its goals by connecting individuals with culturally responsive, trauma-informed housing assistance, treatment, diversion, expungement services and peer navigation support delivered by city agencies/departments and CBOs rooted in community. This may involve assessing needs and providing referrals to supportive services

• Increase access to culturally responsive housing options for justice-involved individuals;
The grant is one component of a broader citywide effort across multiple Departments and community-based agencies to deliver services for justice-involved individuals (i.e., people who have been arrested, released, or diverted from jail). been arrested, released, or diverted from jail) more intentionally and holistically. The program aims to bridge the gap between vulnerable reentry populations and the homelessness response system (HRS) by incorporating housing support into the spectrum of available services, including as part of jail discharge and reentry planning. Additional requirements include:
• Provide services that meet the needs of participants who primarily speak languages other than English.
• Coordinate care with all HEARTSS Program Partners to support participant needs.
