PEFORMANCE WORK STATEMENT

FOR

PUMP AND CLEAN OIL WATER SEPARATORS

1.0. DESCRIPTION OF SERVICES.

SCOPE OF WORK: The Contractor shall provide all personnel, equipment, tools, vehicles, materials, supervision, permits/licenses, and any other special items or services necessary to perform operations involved in annual/tri annual or emergency cleaning/skimming of oil water separators (OWS) or holding tanks. All work shall be accomplished at Shaw Air Force Base, Sumter, South Carolina and shall be performed in accordance with the terms and conditions of this Performance Work Statement (PWS).

1.1. PRINCIPLE FEATURES: The scope of work to be accomplished, includes but is not limited to the following:

1.1.1. Service “OWS” or Service “Holding Tanks” listed on Appendix A, as per Site/Facility Location, Task and Frequency Chart, at the frequencies listed. The Contractor shall perform either a Skimming or a Clean & Recharge on the indicated tanks as specified on appendix “Site/ Facility Location and Task Frequency Chart”. Upon completion of service to OWS/Holding Tanks, it shall be left free of oil or solid debris on bottom. OWS tanks shall have oil free water not lower than chamber separation baffles. If servicing a Holding Tank, it shall contain no more than 25% of total tank capacity upon completion of service. If water is needed to restore an OWS to minimum operational levels, it shall be supplied at no cost to contractor. (See Section 3.)

1.1.2. Skimming shall include removal of waste oil from the liquid surface and removal of any obvious solid debris from waste water inlet or outlet then restoring water level to “minimum operational level”.

1.1.3. Clean and Recharge consists of removal of waste oil/water from OWS and removal of sludge from bottom as well as any visible or obvious obstruction from inlet and outlets of a separator, cleaning of residue from walls, covers and recesses of separators/interceptors and cleaning of all associated strainers, leaving each compartment empty of waste including scum, grease film, etc. This may be accomplished by pressure washing compartment or another acceptable method the Contractor may want to propose with the Contracting Officer’s approval. Contractor shall also be responsible for restoring water in separators to minimum operational level after cleaning to ensure the separator continues to operate effectively. Water removed from the OWS for the purpose of cleaning may be reused in OWS after cleaning is accomplished providing that water is free of oil/solid waste.

1.1.4. The depth of water and the depth of oil shall be measured to the nearest inch and recorded for submittal to the Contracting Officer’s Representative (COR) for each tank/separator each time a unit is serviced. Check sludge volume to ensure that it is no more than 20% of the tank’s capacity. Record sludge volume and submit report to COR for each tank serviced. In addition, grit chambers shall be serviced to allow proper flow into separator. Oil holding chambers shall be checked to

1512459583A Cross-Out ensure oil does not exceed 75% of chamber volume. Record oil holding chamber volumes and submit to COR. Check effluent side of separators to ensure it does not contain any petroleum products. Record visual inspection of effluent chamber and verify no petroleum products are present and condition of separator and submit information to COR.

1.1.5. The elevation of the water/oil interface and the total liquid level, referenced to the bottom of the tank/separator, will be measured using a gauging stick and water finding paste. Units with multiple compartments should have multiple measurements if the compartments are accessible for measurement.

2. SERVICES SUMMARY.

3. GOVERNMENT FURNISHED PROPERTY AND SERVICES. Water for the purpose of refilling or otherwise bringing operational level up to minimum required conditions shall be provided at no cost, contractor may use fill point (two and one half inch valve) located between building 708 and hangar 1200 as this is the only authorized fill location for on base contractors, fill point is located beside Fire Dept. on Shaw AFB. (Building 708)

3.1. Information Management: All records, files, documents, regardless of media (paper, electronic, etc.), as described in the PWS, that are the responsibility of the contractor are the property of the Government and shall remain so upon termination or completion of the contract.

The contractor shall keep these items current and maintain and dispose of them in accordance with the requirements established in Air Force Instruction (AFI) 33-322, Records Management Program, Paragraph 10, Air Force Manual (AFMAN) 33-363, paragraph 6.4, Air Force Manual (AFMAN) 33-364, paragraph 2.13, and the Air Force Records Disposition Schedule available on the Air Force Portal link. Records shall be turned over to the Government upon completion of the contract unless otherwise stated. All records are subject to the Freedom of Information and Privacy Act. The Shaw AFB Records Management Office (20 CS/SCOKR), 895-1630, will provide assistance and training to the contractor to ensure compliance with Federal record keeping requirements.

4. GENERAL INFORMATION

4.1. HOURS OF OPERATION: The Contractor’s workweek shall be 0730 to 1630 Monday through Friday, Federal holidays excluded. The Contracting Officer must approve in advance any deviation from these hours.

4.2 SCHEDULING OF WORK: The Contractor shall submit a detailed work schedule for approval prior to the start of any work, it must show the dates the work shall be accomplished. The Contracting Officer must approve any deviations from the approved schedule 48 hours in advance.

Contractor shall contact 20 CES/CEOES at 803-895-9714/9712, hours prior to commencement of any work under this contract. No collect calls will be accepted.

4.3. The Contractor shall adjust his work schedule around the following recognized federal holidays:

4.4. SPILL. In case of a spill on Shaw AFB of any materials involved in this contract, the following shall take place:

4.4.1. Report. The Contractor shall immediately notify the COR. The Contractor should be prepared to give the following information:

• Name

• Location of spill.

• Number of injured persons and nature of injuries (if applicable).

• Substance spilled.

• Source of spill.

• Approximate quantity of spill and rate material is currently spilling.

• Extent spill has traveled.

• Water which might be affected.

• Cause and time of spill.

• Any action underway to confine or clean up the spill.

4.5. CLEANUP. The Contractor is solely responsible for immediate (same day) containment, cleanup, and disposal of any spills occurring in the performance of this contract. Costs incurred by the government due to the Contractors’ failure to properly clean, and dispose of any spill will be deducted from any payments due the Contractor. The Contractor shall have basic spill response equipment on the truck at all times while servicing the oil/water separators.

4.6. EMERGENCY SERVICE CALLS. Normally all work will be in accordance with the Contractor’s submitted work schedule however unexpected services may be required in the event of natural disasters or other unscheduled events or repairs to pipes or other infrastructure involving oil/water separators. In the event that servicing is required outside the Contractor’s submitted work schedule, the Contractor shall begin the emergency service within 12 hours of notification; each emergency service call shall not exceed 2000 gallons pumped. On-call service shall be priced separately on the bid schedule.

4.7. TYPE AND FREQUENCY OF SERVICE: The OWS/Holding Tanks shall be serviced with the type service and frequencies indicated in Appendix “A”

4.8. SALVAGE AND DISPOSAL

4.8.1. All waste materials generated by pumping and/or cleaning of tanks/oil separators etc. shall be handled, transported, stored, recycled, and disposed of off base by the Contractor. Disposal and recycled methods shall be in accordance with all applicable Federal, State, or local laws, ordinances, regulations, court orders, or other types of rules or rulings having the effect of laws.

Reports:

Contractor shall submit a report after each Skimming and or Cleaning along with disposal tickets to the COR no later than 5 business days after disposal identifying each separator holding tank pumped or cleaned and the quantity (Gallons) of waste and oil removed from each and certify that disposal was in accordance with Federal, State and EPA environmental laws and regulations.

4.8.2. The Contractor shall obtain all licenses and pay all fees required for any portion of the contracted operation.

4.8.3. Oil or waste pumped or skimmed from OWS/Tanks may be transported to the Wash Rack at building 344 or other on-base facilities approved by the Contracting Officer and be allowed to settle in the truck for a minimum of 30 minutes, after settlement is complete, water at the bottom of the tank may be drained off into the oil water separator. When the water has been drained off, the remaining liquid waste shall be managed in accordance with 40 CFR 279. The Contractor is responsible for ensuring all separators, which have been pumped and cleaned, are properly recharged with re-circulated/oil free water before performance will be considered satisfactory.

4.8.4. The operational level in the oil/water separator at the Wash Rack, Bldg 344 (or other on-base facility used for the water drain-off phase) shall be checked by the contractor prior to and after each day’s work when oil separator/tanks are being serviced. If the oil liquid level in the tank is greater than 6 inches, the tank shall immediately be skimmed. The extra skimming requirements of the oil separator tank used for the water drain-off phase (if required) shall be included in the total contract price, as this work will not be paid on a unit price basis.

4.8.5. The Contractor shall be required to inform the Contracting Office of the means of managing the material pumped from the oil-water separators prior to the commencement of any work (pumping, sludge disposal). The material has been classified as used oil (non-hazardous waste) and is subject to the requirements of 40 CFR 279, Standards for the Management of Used Oil.

4.9. SUBMITTALS REQUIRED: All the following required data and permits shall be submitted by the Contractor and be approved by the Contractor Officer prior to the start of work. Items to be submitted are as follows:

4.9.1. A detailed work schedule calendar showing the dates that the waste separators and/ or tanks are to be serviced. As a minimum the annual calendar shall indicate the following information:

Example: If a unit calls for TRI-ANNUAL (3) pump outs, and ANNUAL (1) pump out and clean. That requires four separate operations, the pump out and clean does not count as one of the pump outs. SEMI-ANNUAL (1) pump outs shall be accomplished six months before or after the Annual clean out, depending when the Annual is scheduled.

4.9.2. The Contractor shall provide the Government a copy of any Environmental Protection Agency (EPA) Reports that the Contractor is required to file and maintain in accordance with Environmental Protection Agency Standards and this Performance Work Statement.

4.9.3.1 Transporter Information

4.9.4. The Contractor shall develop and provide the Government a report that lists the facilities serviced and the extent to which they were serviced on a particular day. The report shall include, but not be limit, to the following:

4.9.4.1 a. CLIN number

b. Building number

c. Type of service; i.e. skimming or clean.

d. Service frequency

e. Amount of water and amount of waste to nearest 1”.

f. Number of units measured and price per unit to measure.

g. Price per unit to service and total price of service provided.

h. Inspection data as listed in section 5.2

4.9.4.2. The Contractor shall report to the Service Contracts Section Bldg. 252, prior to starting each day’s required service under this contract. The Contractor shall fill out and turn in a Record of Disposal or Record of Recycling to the Service Contract Section. If grit chambers, grates, or oil water separators are cleaned and sludge is removed, contractor shall submit a report on the quantities of sludge and sediment removed location of disposal site, and the method of disposal.

4.9.4.3. The Government reserves the right to accompany the operator to observe the requirements of this Performance Work Statement.

4.10. PERSONNEL:

4.10.1. Contractor must be a trained specialist in handling/recycling waste petroleum products and hazardous wastes and follow all safety regulations pertaining to entry into a confined space. If that contractor must enter an OWS that falls under OSHA 1910 Confined Space definition, the contractor must follow rules set governing the entry of a confined space in addition, the contractor must notify Shaw AFB Fire Dept. as well as COR prior to entering and immediately after exiting the confined space.

4.10.2 Contractor personnel shall present a neat appearance and be easily recognized. This may be accomplished by wearing distinctive clothing bearing the name of the company or by wearing appropriate badges, which contain the company name and employee name.

4.10.3. EMPLOYEE TRAINING. The Government at its’ discretion, will conduct flight line access/driver familiarization training for all employees requiring entry within the buildings and flight line area. Only personnel trained in oil/water maintenance procedures shall be allowed to perform the work indicated in this Performance Work Statement (PWS). The Contractor shall maintain records of individual’s training and certification as well as provide proof of insurance and VIN number of any vehicle used for OWS services on Shaw AFB.

5.0. SECURITY REQUIREMENTS FOR UNCLASSIFIED CONTRACTS

5.1. Listing of Employees. The contractor shall maintain a current listing of employees. The list shall include the employee's name, social security number and type of investigation if contract work involves unescorted entry to Air Force restricted areas or other sensitive areas designated by the installation commander. The list shall be provided to the Program Manager and sponsoring agencies Security Manager. An updated listing shall be provided when an employee's status or information changes.

5.2. Security Training. The contractor ensures all contractor employees receive initial and recurring security education training from the sponsoring agencies security manager. Training must be conducted IAW DODM 5200.01, Volume 3, Information Security Program, and AFI 31- 401, "Information Security Program Management". Contractor personnel who work in Air Force controlled/restricted areas must be trained IAW AFI 31- 101, The Air Force Installation Security Program.

5.3. Pass and Identification Items. The contractor shall ensure the pass and identification items required for contract performance are obtained for employees.

5.3.1. Background Checks. All contractor employees shall submit their name, social security number, date of birth and driver’s license (or valid identification card) number to the COR or Contracting Officer. The required information is needed to perform a criminal background check accomplished by Shaw Air Force Base Security Forces prior to the employee being issued base access credentials. Failure to submit requested Contractor Listing of Employees as indicated can result in denial or delay of issuing base access credentials. Processing criminal background checks takes, at a minimum, 7-10 days from time of submission.

5.4. Retrieving Identification Media. The contractor is responsible for retrieving all government-issued identification media from employees who depart for any reason before the contract expires;

e.g. terminated for cause, retirement, etc. The contractor shall return retrieved items to the Security Forces Pass and Registration Office.

5.5. Traffic Laws. The contractor and its employees shall comply with base traffic regulations.

5.6. Weapons, Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their contractor-owned vehicle or privately-owned vehicle while on Shaw Air Force Base.

5.7. Controlled Unclassified Information (CUI). The contractor shall comply with DoD Instruction 5200.48, Controlled Unclassified Information; DoD 5400-7-R, Chapter 4, DoD Freedom of Information Act (FOIA) Program, requirements. This regulation sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding FOUO material.

5.8. Reporting Requirements. Contractor personnel shall report to an appropriate authority any information or circumstances of which they are aware may pose a threat to the security of DOD personnel, contractor personnel, resources, and classified or unclassified defense information.

Contractor employees shall be briefed by their immediate supervisor upon initial on-base assignment.

5.9. Physical Security. The contractor shall be responsible for safeguarding all government property and controlled forms provided for contractor use. At the end of each work period, all government facilities, equipment, and materials shall be secured.

5.10. Controlled/Restricted Areas. The contractor shall implement local base procedures for entry to Air Force controlled/restricted areas where contractor personnel will work. An AF Form 2586, Unescorted Entry Authorization Certificate, must be completed and signed by the sponsoring agencies Security Manager before a Restricted Area Badge will be issued. Contractor employees must have a completed National Agency Check with written inquiries (NACI) investigation before receiving a Restricted Area Badge.

5.11. Key Control. The contractor shall establish and implement methods of making sure all keys issued to the contractor by the Government are not lost or misplaced and are not used by unauthorized persons. The contractor shall not duplicate any keys issued by the government.

5.11.1. The contractor shall immediately report to the Contracting Officer’s Representative (COR) or Program Manager any occurrences of lost or duplicated keys.

5.11.2. In the event keys, other than master keys, are lost or duplicated, the contractor may be required, upon written direction of the Contracting Officer, to re-key or replace the affected lock or locks without cost to the government. The government may, however, at its option, replace the affected lock or locks or perform re-keying and deduct the cost of such from the monthly payment due the contractor. In the event a master key is lost or duplicated, all locks and keys for that system shall be replaced by the government and the total cost deducted from the monthly payment due the contractor.

5.11.3. The contractor shall prohibit the use of keys, issued by the government, by any persons other than the contractor’s employees and the opening of locked areas by contractor employees to permit entrance of persons other than contractor employees engaged in performance of contract work requirements in those areas.

The following hazardous material and environmental requirements are required in Shaw AFB Statement of Works (SOW’s) and Performance Work Statements (PWS’s) when contract work involves hazardous materials or environmental issues (Refer to attached Appendices C and D).

Contractor is subject to all requirements within the Shaw AFB Hazardous waste management plan 32- 7002C, Hazardous material management plan 32-7002A, Federal, State, and Local regulations, Shaw Operating Instructions/ regulations and Air Force Regulations present and future versions.

Appendix “B”

DEFINITIONS

Designated Facility: A waste treatment, storage, or disposal facility (TSDF) which has received an EPA permit in accordance with the requirements of 40 CFR 270 and 40 CFR 124 and that has been designated on the manifest by the generator pursuant to 40 CFR 262.50.

Disposal Facility: A facility or part of a facility at which waste is intentionally placed into or on any land or water, and at which waste will remain after closure.

EPA Identification Number: A number assigned by EPA to each generator, transporter and treatment, storage or disposal facility.

Generator: Any person, by site, whose act or process produces hazardous waste identified or listed in 40 CFR 261, or whose act first causes a hazardous waste to become subject to regulation.

Hazardous Waste: A hazardous waste as defined by 40 CFR 261.3.

Manifest: The shipping document EPA Form 8700-22 and, if necessary, EPA Form 8700-22A, originated and signed by the generator in accordance with the instructions included in the Appendix of 40 CFR 262.

Manifest Document Number: The U.S. EPA twelve digit identification number assigned to the generator plus a unique five digit document number assigned to the Manifest by the generator for recording and reporting purposes.

Transporter: The Contractor or a person engaged in the off-site transportation of hazardous waste by air, rail, highway, or water.

Volatile Liquids: Waste material to be removed from separators and tanks which includes biodegradable soaps and water, various oils, engine oils, mineral base solvents such as PD-680 and small quantities of jet fuel such as JP4.
