About this file

This document is a Request for Quotation (RFQ) issued by the West Virginia Department of Health and Human Services, Bureau for Behavioral Health and Health Facilities, on behalf of Lakin Hospital, for a one-time purchase of food service cookware. The RFQ lists 50 specific cookware items required, including stock pots, pans, cutting boards, tongs, measuring cups and spoons, steam table pans, and various kitchen utensils, along with detailed specifications for each item. The contract items must be delivered within 30 working days after receiving a purchase order, and the contract will be awarded to the vendor providing the lowest overall grand total cost.

The RFQ outlines payment terms, delivery requirements, processes for returning unacceptable items, and remedies for vendor default. It does not mention any set-asides, incumbents, or funding details, but states the contract is intended to provide Lakin Hospital with a purchase price for the required cookware items.
