SUBJECT: PAST PERFORMANCE QUESTIONNAIRE IN RESPONSE TO NRC REQUEST FOR QUOTATION NO. NRC-HQ-60-15-R-0001, ENTITLED, “TECHNICAL SUPPORT FOR THE RADIOLOGICAL ASSESSMENT SYSTEM FOR CONSEQUENCE ANALYSIS (RASCAL) COMPUTER CODE AND EMERGENCY RESPONSE ASSESSMENT TOOLS FOR THE NRC OPERATIONS CENTER”

The U.S. Nuclear Regulatory Commission (NRC) is currently conducting an evaluation for award of a task order to provide the NRC with the subject services.

The NRC’s primary source of information for evaluation of past performance will come from the past performance references that the Offeror has provided to us. The NRC evaluation panel must ensure that the Offeror has both relevant and successful past performance in performing work of a comparable nature and complexity to the NRC’s requirement.

Accordingly, I ask that you complete the attached Past Performance Questionnaire and return it directly to me via email at richard.robinsonii@nrc.gov no later than 5:00 PM, DC local time, on Wednesday February 6, 2015.
