About this file
This is a Northern Ireland Fair Employment Practices Disclosure form required by the City of Cleveland under Codified Ordinance Sec. 181.36 for contractors and subcontractors with more than fifty percent interest in proposed contracts. The form requires contractors to disclose whether they or their controlling shareholders, subsidiaries, or parent corporations engage in business or trading for profit in Northern Ireland. Contractors must complete the disclosure prior to contract award and certify either that they have no business interests in Northern Ireland, or that they are taking lawful and good faith steps to engage in fair employment practices consistent with the MacBride Principles for Fair Employment in Northern Ireland. Written questions regarding the form must be submitted by July 15, 2026, at 4:00 p.m., with an addendum containing all questions and answers posted by July 22, 2026. Proposals are due July 29, 2026, by noon.
False statements on this disclosure form constitute default of contract and expose contractors and subcontractors to remedies for default as specified in their contracts. Any contractor or subcontractor found to have made a false statement on the disclosure will be automatically excluded from bidding for any goods or services for use by the City of Cleveland for a period of two years. The form requires the signature of an authorized representative of the contractor or subcontractor submitting the disclosure. Contractors may obtain a copy of the MacBride Principles from the Office of the Commissioner of Purchases and Supplies if additional guidance is needed regarding compliance with fair employment standards.
