The service unit comprises three facilities: Northern Navajo Medical Center (a 24/7 hospital-based facility) and two ambulatory care facilities (Four Corners Regional Health Center and DZ Health Center) operating Monday-Friday, 8:00 AM to 4:30 PM.

The contractor shall conduct focused chart reviews for inpatient and outpatient medical records across fourteen clinical departments including Emergency Medicine, Pediatrics, Internal Medicine, Rheumatology, Family Medicine, General Surgery, Obstetrics and Gynecology, Orthopedics, Podiatry, Wound Care, Optometry, Neurology, Pathology, Radiology, Rehabilitation (Physical Therapy and Occupational Therapy), and Psychiatry. Deliverables include approximately 500-600 medical chart reviews annually, with routine quarterly provider reviews ensuring Standard of Care compliance. Medical records will be delivered electronically (preferred) or by paper, covering three contract years: May 2026 to May 2027, May 2027 to May 2028, and May 2028 to May 2029. The contractor must designate board-certified physicians with no conflicts of interest, submit completed reviews within 21 days of receipt (or sooner for urgent reviews marked for one-week completion), and destroy or return all documents within 120 days after completion. Quality assurance includes 100% compliance with Standards of Care, professional non-inflammatory language in all reports, and Board Certification verification for all assigned staff. Performance evaluation will utilize the Contractor's Performance Assessment Report (CPAR), with payment contingent upon satisfactory service delivery and no compensation for incomplete work. The contractor serves as a Business Associate under HIPAA and must indemnify the hospital against claims arising from negligence.
