About this file
This is a Price Appendix FAQ document providing instructions for completing pricing spreadsheets for a federal Blanket Purchase Agreement (BPA) and an Abbreviated Dosing Plan (ADP) contract opportunity.
For the BPA section, vendors must complete yellow cells only while all other cells remain locked. Upon entering the Package Size in column G for each National Drug Code (NDC), the spreadsheet auto-populates remaining cells. The Price per Unit (column H) is auto-calculated using the formula: Price per NDC divided by Package Size. If a vendor does not wish to submit a quote for a condition set, the Price per NDC cell in column F will highlight red to signal non-participation to the Pharmacy and Therapeutics (P&T) committee. Vendors should review Formulary Notes at the spreadsheet top to understand P&T decision-making processes. The template accommodates 10 NDCs; vendors submit quotes only for their applicable NDCs. Requests for additional NDC tables or questions regarding completion should be directed to the Points of Contact (POCs) listed in the RFQ document.
For the ADP section, vendors similarly fill yellow cells only and review Formulary Notes for context. Upon entering the first non-FAMP price in column F, subsequent rows auto-populate with that price. The first Current Annual Federal Ceiling Price entered in column H auto-populates remaining rows. A 0.0000% Additional Offered Refund in column G highlights red, indicating non-participation. Columns I and J auto-calculate using the formula: ((non-FAMP-FCP) + (non-FAMP×Y%)). Like the BPA section, the template provides 10 NDC tables; vendors submit only applicable tables and should contact POCs for additional tables or completion questions.
