About this file
This is a Price Appendix Frequently Asked Questions (FAQ) document that provides instructions for completing pricing submission templates for two contract vehicle types: Blanket Purchase Agreements (BPA) and Authorization, Delivery, and Payment (ADP) agreements.
For BPA submissions, vendors must complete yellow-highlighted cells in NDC (National Drug Code) pricing tables, with all other cells locked from editing. Package Size entries in column G auto-populate remaining cells, and Price per Unit (column H) is auto-calculated using the formula: Price per NDC divided by Package Size. If a vendor does not wish to submit a quote for a condition set, the Price per NDC field in column F will highlight red to signal non-participation to the P&T committee. The template provides tables for up to 10 NDCs; vendors should only complete tables for their actual NDC submissions. Vendors with more than 10 NDCs must contact the Points of Contact (POCs) listed in the RFQ document to request an expanded spreadsheet.
For ADP submissions, vendors must complete yellow-highlighted cells, with subsequent rows auto-populating after the first entry in each relevant field. The first non-FAMP price entered in column F and the first Current Annual Federal Ceiling Price entered in column H auto-populate down the table. A 0.0000% Additional Offered Refund in column G will highlight red to indicate non-participation in that formulary category. Columns I and J auto-calculate using the formula: Total Calculated Refund = ((non-FAMP-FCP) + (non-FAMP*Y%)). Similar to BPA submissions, vendors may submit up to 10 NDCs and should contact POCs for expanded templates if submitting more. Both sections emphasize reviewing Formulary Notes at the spreadsheet top to understand P&T committee decision-making processes and instruct vendors to contact POCs with questions or concerns.
