About this file

This is a Price Appendix FAQ document providing instructions for completing pricing spreadsheets in response to a Request for Quotation (RFQ) for pharmaceutical products through a federal contract.

The document contains separate submission instructions for two pricing categories: BPA (Blanket Purchase Agreement) and ADP (Additional Discount Program). For both categories, respondents must complete yellow-highlighted cells in provided spreadsheet tables while all other cells remain locked. Respondents should review Formulary Notes at the top of the spreadsheet to understand the Pharmacy and Therapeutics (P&T) committee's decision-making process. For BPA submissions, respondents enter Package Size (column G) which auto-populates remaining cells, and Price per NDC (column F) which auto-calculates Price per Unit (column H). Failure to submit a quote for a condition set results in red highlighting in the Price per NDC cell. For ADP submissions, respondents input non-FAMP price (column F) and Current Annual Federal Ceiling Price (column H), which auto-populate subsequent rows, and Additional offered refund percentage (column G), with red highlighting indicating no quote submission. Columns I and J auto-calculate using the formula: Total Calculated Refund = ((non-FAMP-FCP) + (non-FAMP*Y%)). The spreadsheets accommodate up to 10 NDCs (National Drug Codes); respondents needing additional NDC tables should contact the RFQ Points of Contact with their requirements. Questions or concerns about table completion should be directed to the POCs identified in the RFQ document.
