About this file
This document is a Price Appendix FAQ for a Department of Defense (DoD) Pharmacy Uniform Formulary Blanket Purchase Agreement (BPA) and Uniform Formulary Additional Discount Program (ADP) solicitation. The FAQ provides detailed instructions for vendors submitting pricing quotes for pharmaceutical agents across two pricing sections: BPA and ADP. For both sections, vendors are instructed to fill in yellow cells on a spreadsheet, with specific guidance including reading formulary notes, auto-population of certain cells, calculation methods for pricing, and submission limits of up to 10 National Drug Codes (NDCs) per submission.
Key instructions include reading formulary notes to understand the Pharmacy and Therapeutics (P&T) committee's decision-making process, completing only yellow cells while other cells remain locked, and understanding auto-calculation features for pricing. If vendors need to submit more than 10 NDCs, they are directed to contact the Points of Contact (POCs) on the Request for Quotation (RFQ) document. The document covers specific details about how to indicate non-participation in a formulary category, such as highlighting certain cells red, and provides formulas for calculating refunds and unit pricing. Vendors with questions or concerns are instructed to contact the POCs outlined in the RFQ document.
