About this file
This document is a Price Appendix FAQ providing detailed instructions for vendors to complete pricing spreadsheets for the Department of Defense Pharmacy Uniform Formulary opportunity. It contains two main sections covering the Blanket Purchase Agreement (BPA) and Additional Discount Program (ADP) pricing submissions.
The BPA section instructs vendors to fill in yellow cells for NDC pricing information, with Package Size entered in column G and Price per NDC in column F. The system automatically calculates Price per Unit in column H using the formula Price per NDC/Package Size. For the ADP section, vendors must input non-FAMP prices (column F) and Current Annual Federal Ceiling Prices (column H), with automatic calculation of Total Calculated Refund based on the formula ((non-FAMP-FCP) + (non-FAMP*Y%)). Both sections provide 10 NDC tables by default, with additional tables available upon request from the POCs. The document emphasizes reviewing Formulary Notes for understanding P&T committee decisions and contacting POCs with any questions about completing the pricing tables.
