About this file
This is a Past Performance Information Sheet template designed for quoters to document and evaluate their relevant contract experience. The form requires submission of up to three past performance references, with each reference requiring detailed contract information including the contract number, procuring agency, contract value, period of performance, and reference point of contact details with name, phone number, and email.
For each reference, quoters must provide a detailed description of services performed, document any issues encountered during contract performance (including the number, type, frequency, and duration of issues and their impact on quality, delivery, or cost), describe corrective actions taken and their outcomes, and assess the relevance of the past performance to the current solicitation. The relevance assessment uses four categories: Very Relevant (essentially the same scope and magnitude of effort and complexities); Relevant (similar scope and magnitude); Somewhat Relevant (some of the scope and magnitude); or Not Relevant (little or none of the scope and magnitude). The template defines key evaluation criteria as scope (range of operations covered), magnitude (dollar value), and complexity (degree of difficulty). This form serves as a structured mechanism for evaluating a quoter's capability and track record in delivering similar services to the federal government.

The definitions for “Relevant”, ”Not Relevant” “scope”, “size” and “complexity” are as follows:
• Very Relevant: Present/past performance effort involved essentially the same scope and magnitude of effort and complexities this solicitation requires.
• Relevant: Present/past performance effort involved similar scope and magnitude of effort and complexities this solicitation requires.
• Somewhat Relevant: Present/past performance effort involved some of the scope and magnitude of effort and complexities this solicitation requires.
• Not Relevant: Present/past performance effort involved little or none of the scope and magnitude of effort and complexities this solicitation requires.
Scope: Range of operations covered.
Magnitude: Dollar value.
Complexity: Degree of difficulty.
