About this file
This is a Past Performance Information Sheet template used in federal contract solicitations to evaluate a quoter's previous contract experience and performance history.
The template provides three reference sections (Reference #1, #2, and #3) for quoters to document past contracts. For each reference, quoters must provide contract identification details including contract number, procuring agency or organization, contract value, period of performance, contract type (prime or subcontractor), and reference point of contact information (name, phone, email). Quoters are required to provide a detailed description of services performed under each contract, describe any issues encountered during performance including the number, type, frequency, and duration of issues and their impact on quality, delivery, or cost, and explain any corrective actions taken and their outcomes. Additionally, quoters must assess the relevance of each past performance to the current solicitation using four defined categories: Very Relevant (essentially the same scope and magnitude of effort and complexities), Relevant (similar scope and magnitude of effort and complexities), Somewhat Relevant (some of the scope and magnitude of effort and complexities), or Not Relevant (little or none of the scope and magnitude of effort and complexities). The template defines key evaluation criteria as scope (range of operations covered), magnitude (dollar value), and complexity (degree of difficulty).

The definitions for “Relevant”, ”Not Relevant” “scope”, “size” and “complexity” are as follows:
• Very Relevant: Present/past performance effort involved essentially the same scope and magnitude of effort and complexities this solicitation requires.
• Relevant: Present/past performance effort involved similar scope and magnitude of effort and complexities this solicitation requires.
• Somewhat Relevant: Present/past performance effort involved some of the scope and magnitude of effort and complexities this solicitation requires.
• Not Relevant: Present/past performance effort involved little or none of the scope and magnitude of effort and complexities this solicitation requires.
Scope: Range of operations covered.
Magnitude: Dollar value.
Complexity: Degree of difficulty.
