About this file

The 25R6511 Base Access Request Form is a document detailing instructions for contractors interested in attending a pre-proposal site visit (PPSV) for a design-build medical clinic construction project in Huy Giap, Cao Bang Province, Vietnam. The file specifies that contractors may submit up to four personnel names (two primary and two alternates), but only two participants plus one driver and one vehicle will be permitted to attend the site visit.

Detailed submission requirements include providing a personnel roster in an Excel spreadsheet, combining all participant identification documents into a single, bookmarked PDF file, and submitting evidence of active SAM registration. The site visit is scheduled for June 2025, with the Base Access Request deadline set for May 19, 2025, at 11:00 am Thailand Time. The project involves constructing a two-story, approximately 525 square meter medical clinic with a project magnitude between US$500,000 and US$1,000,000, featuring reinforced concrete construction, energy-efficient systems, and compliance with Vietnamese Ministry of Health standards. Submissions must be sent to the Site Access Coordinator, Ms. Kannikar Pakhunthot, with specific email requirements for submission.
