About this file
This is a Standard Form 30 (SF 30) amendment to a solicitation/contract issued by the Naval Supply Fleet Logistics Center Yokosuka on April 29, 2026, effective immediately. The amendment modifies solicitation N6264926QJ010, dated April 27, 2026, and does not extend the offer receipt deadline. The contracting officer is Jon R. Miller, and the issuing activity is NAVSUP FLT LOG CTR YOKOSUKA (PSC 473 BOX 11, FPO AP 96349-1500). The amendment does not require contractor signature and return.
The amendment revises maintenance requirements in the Performance Work Statement (PWS) for the rental of one 15,000-pound forklift at Naval Air Facility Atsugi Air Operations, Japan. The primary change deletes and replaces Section C, Paragraph 4.2.5, requiring the contractor to perform monthly safety and operational inspections of the forklift, coordinated in advance with the Government Technical Point of Contact (TPOC) to minimize operational impact. The contractor must maintain inspection logs and provide them to the Government TPOC upon request. The base contract period is six months from the date of award with one optional one-month extension. The forklift must be delivered to U.S. Air Terminal Building 206 at NAF Atsugi and meet specified equipment requirements including a diesel engine, 15,000-pound load capacity, integrated side-shifter attachment, and oil-cooled wet disc brakes. The contractor is responsible for delivery, removal, maintenance, and repairs, with on-site repair technician response required by the end of the next business day following an equipment failure notification and temporary replacement units provided at no additional cost if repairs exceed three business days.
