SECTION SF 30 BLOCK 14 CONTINUATION PAGE
SUMMARY OF CHANGES
SECTION SF 30 - BLOCK 14 CONTINUATION PAGE
The following have been modified:
QUESTIONS AND ANSWERS
Question 1: Is this solicitation opened to Service-Disabled Veteran Owned Businesses only OR Service-Disabled Veteran Owned Businesses AND Small Businesses?
Answer 1: Per Section K.1 of the solicitation, “In accordance with FAR 52.204-8, the offeror must complete the Online Representations and Certifications Application (ORCA) to include representation as a Service Disabled Veteran Owned Small Business (SDVOSB)”
Question 2: In response to Amendment 2, where the government has specified what Wage Determinations to be used, will the government please supply the SCA Occupation codes to be used for each position on the solicitation?
Answer 2:
12072 – Licensed Practical Nurse II
12312 – Registered Nurse II
12314 – Registered Nurse III
Question 3: We realize that the deadline has passed for questions on the above solicitation, but we wanted to point out an inconsistency with the hours on the Statement of Work for Lot A (RN Labor and Delivery) and the Pricing Sheet for Lot A. Section 4.4 of the SOW indicates that the contractor shall provide 252 hours per week, Sunday through Saturday. Additionally, the table in Section 4.4 indicates two RNs per shift for each weekend day, but calculates only 48 hours. Shouldn’t the number read 96 hours to cover both Saturday and Sunday? Therefore, the total hours required per week would be 300. Taking into consideration the extra day for leap year, shouldn’t the total hours on the pricing sheet read 15,648?
Answer 3: Please see the revised Statement of Work in Section L, Enclosure 3 – Lot A, Statement of Work for Registered Nurse, Labor & Delivery and the revised pricing sheet will be posted to FBO and shall be submitted in accordance with Section L.
SECTION A - SOLICITATION/CONTRACT FORM
