For: NAVFAC Atlantic, Norfolk, VA Contract Number: N62470-22-R-9000 Contract Description: CNIC Housing Condition Assessment Program IDIQ Contract for NAVFAC Contractors Name: CNIC Housing Condition Assessment Program
1. PURPOSE
This Quality Assurance Surveillance Plan is a government-developed document used to determine if the contractor’s performance meets the performance standards contained in the contract. The QASP establishes procedures on how this assessment/inspection process will be conducted. It provides the detailed process for a continuous oversight process:
• What will be monitored?
• How monitoring will take place?
• Who will conduct the monitoring?
• How monitoring efforts and results will be documented?
This QASP does not detail how the contractor accomplishes the work. Rather, the QASP is created with the premise that the contractor is responsible for management and quality control actions to meet the terms of the contract. It is the Government’s responsibility to be objective, fair, and consistent in evaluating performance.
The Government may review and revise the QASP on a regular basis and the contractor shall cooperate with government quality insurance activities so long as the government’s actions reasonably relate to the determination of whether the contractors performance is in compliance with contractual requirements.
2. GOVERNMENT ROLES AND RESPONSIBILITIES

Contracting Activity: Naval Facilities Engineering Command Atlantic (NAVFAC Atlantic)
b. Contracting Officer Representative (COR) – The COR is responsible for technical administration of the contract and shall assure proper Government surveillance of the contractor’s performance. The COR is not empowered to make any contractual commitments or to authorize any contractual changes on the Government’s behalf.
Assigned COR: To be assigned at award.
3. PERFORMANCE STANDARDS
Performance standards define desired services. The Government performs surveillance to determine if the contractor exceeds, meets or does not meet these standards.
This section details the method(s) used to verify Contractor compliance with the Statement of Work (SOW). The key elements of this process are the Contractor’s quality program and Government identified performance objectives. The Performance Requirements of the contract dictate the suggested inspection and surveillance requirements the COR shall accomplish on a periodic basis. The procedures on how to document performance in a correct and effective manner are included below.
4. QUALITY ASSURANCE (QA) SURVEILLANCE METHODS
The intent of the QA surveillance methods is to gain confidence in the Contractor’s ability to provide satisfactory services. This surveillance method is subject to change based on the Contractor’s performance. The COR shall use the surveillance methods listed below in the administration of this
QASP.
a. 100% Review. This level of review entails consideration of all performance requirements and deliverable established by and for the Performance Objective within a given timeframe.
b. Random Monitoring. Random monitoring consists of unspecified observations of some or all requirements and deliverable established by and for the Performance Objectives within a given timeframe.
c. Passive Monitoring. Passive monitoring includes observations by other Government personnel to be substantiated by the COR that relate to some or all requirements and deliverables established by and for the Performance Objectives within the given timeframe. The handling of complaints is included in this method of surveillance.
d. Periodic Review. The COR conducts periodic performance reviews with the contractor. At this time, the QASP Performance Objectives can be reviewed, assessed for their applicability and modified with the addition or subtraction of Performance Objectives as conditions warrant.
e. Special Audits. The COR must be alert of conditions that would warrant a special quality audit.
Any time the COR observes that a functional area is out of acceptable tolerance or that the technical expertise is not available, a request for a functional area quality audit should be addressed to the contractor.
5. DOCUMENTING PERFORMANCE
a. ACCEPTABLE PERFORMANCE The Government may document positive performance. Any report may become a part of the supporting documentation for any contractual action.
b. UNACCEPTABLE PERFORMANCE When unacceptable performance occurs, the COR may inform the Contractor. This will normally be in writing unless circumstances necessitate verbal communication. In any case, the COR shall document the discussion and place it in the COR file.
When the COR determines formal written communication is required, the COR may prepare a Contract Discrepancy Report (CDR), and present it to the contractor’s program/contract manager. The contractor shall acknowledge receipt of CDRs in writing. The CDR will specify whether the contractor is required to prepare a corrective action plan to document how the contractor shall correct the unacceptable performance and avoid a recurrence. The CDR will also state how long after receipt the contractor has to present this corrective action plan to the COR.

Complete records of all inspection work performed by the Contractor shall be maintained and made available to the Government during contract performance and for as long afterwards as the contract requires.

(f) If the Contractor fails to promptly perform the services again or to take the necessary action to ensure future performance in conformity with contract requirements, the Government may (1) by contract or otherwise, perform the services and charge to the Contractor any cost incurred by the Government that is directly related to the performance of such service or (2) terminate the contract for default.
