1 Background

1.1 Puget Sound Naval Shipyard & Intermediate Maintenance Facility (PSNS & IMF) uses the PL 14-195 High Integrity Container (HIC) to collect ion exchange resin media, and activated carbon which has become saturated with radioactive particulate. The HIC containing Low Level Radioactive Waste (LLRW) must be transported in a protective cask from PSNS & IMF, Bremerton, WA to US Ecology Washington Inc. facility at Hanford Reservation, Richland, WA.

PSNS & IMF does not possess the required cask nor the special trailer that transport it, hence the requirement for contractual support.

2 Scope

2.1 The contractor shall provide the cask and transportation service to transport HIC containing LLRW from PSNS & IMF, Bremerton, WA to US Ecology Washington Inc., Richland, WA in full compliance of this Statement of Work.

3 Objective

3.1 The objective is to safely transport the PL14-195 HIC from PSNS & IMF, 1400 Farragut Ave, Bremerton, WA 98314 to US Ecology Washington INC, ¼ Mile West of 200 East Area, Hanford Reservation, Richland, WA 99352 for disposal.

4 Tasks

4.1 Vendor shall perform a thorough review of the structural, mechanical, and electrical components of the Tractor and Trailer prior to leaving the vendor site.

4.2 Vendor shall provide documentation and photographs to PSNS & IMF in accordance with section 5.5 of this Statement of Work (SOW).

4.3 Vendor shall transport Shipping Cask and corresponding trailer to PSNS & IMF for loading.

4.4 Vendor shall transport the loaded cask to US Ecology (Hanford, Washington) for disposal of the PL14-195

HIC.

4.5 After unloading the PL14-195 HIC, the vendor shall transport the cask from the disposal site back to the vendor’s facility.

4.6 NOT USED

5 Delivery

5.1 Cask Transportation Trailer

5.1.1 The transportation trailer must be designed to carry the fully loaded shipping cask, which weighs 57,350 lbs. The maximum bed height of the transportation trailer shall be limited to 60 inches.

5.1.2 The vendor shall obtain a current Washington State Commercial Vehicle Safety Alliance (CVSA) level 1 inspection for the cask transportation trailer within 1 week of delivering the trailer to PSNS & IMF. Vendor shall provide certification of the CVSA inspection of the trailer upon arrival at PSNS & IMF.

5.1.3 The vendor shall obtain a current Washington State Commercial Vehicle Safety Alliance (CVSA) level 1 inspection for the tractor that will transport the cask to the Hanford Reservation. The vendor shall obtain this inspection within 1 week of the tractor’s arrival at the shipyard. Vendor shall provide certification of the CVSA inspection of the tractor upon arrival at PSNS & IMF.

5.1.4 In addition to meeting all regulatory requirements specified in paragraphs 5.1.2, 5.1.3, and FEDERAL MOTOR CARRIER SAFETY REGULATIONS of 49 CFR Chapter III, Subtitle B, Appendix G, the tractor and transportation trailer will comply with the following standards of professional appearance and mechanical reliability:

5.1.4.1 Tractor and trailer shall be clean consistent with normal road dirt and current weather conditions. Paint shall be relatively uniform in color and be mostly intact consistent with normal wear and as noted below for rusting.

5.1.4.1.1 Rusting that has corroded the surface, such that the large areas have thick rust scale (large areas of metal thinning) that appears to compromise the structural integrity of metal parts, is not acceptable. Rusting that has significant amounts of pinhole corrosion that appears to compromise the structural integrity of the metal parts is not acceptable. Rust on the rims of the wheels must be only be minimal surface rust.

5.1.4.2 Tractor and trailer shall not have visible evidence of a collision, such as dented fender or bent back bumpers, split fiberglass cowlings or covers, dented radiator grill work, tears or holes in the trailer head board or box (indicating a severely shifting cargo event), etc. Denting of trailer rub rails and headboard or box consistence with normal use is acceptable.

5.1.4.3 DOT stickers, labels, and signage (placards) shall be legible and in good condition consistent with normal use.

5.1.4.4 There shall be no missing or damaged decking leaving holes in the trailer, flat bed, deck etc. Repairs and deck scrapes, dents, minor missing wood due to normal usage from loading and unloading cargo are acceptable.

5.1.4.5 There shall be no holes in head boards. Denting consistent with loading and unloading is acceptable.

5.1.4.6 Structural load bearing or tie down parts of the tractor or trailer shall not have cracks which could compromise the structural integrity of the body parts or could result in loss of parts during transportation.

5.1.4.7 Cracked windshields are not acceptable.

5.1.4.8 Windshield wipers shall clean windshields without leaving residue or streaks. .

5.1.4.9 All lights (e.g. headlights, tail/brake lights, turn signals, etc.) shall illuminate when activated and shall have correct, undamaged lens covers.

5.1.4.10 Tires shall be inflated to pressures specified by the tire manufacturer according to conditions and application, and have tread depth greater than 4/32” (steer tire), and 2/32” (all other tires). Tires shall not display uneven tread wear, sidewall damage, fatigue, or cracking, separations, cuts, bead damage, lack of tread, inner liner defects, punctures, improper repairs, or other conditions that would lead to failure. The vendor is responsible for all costs resulting from tire failure.

5.1.4.10.1 Tires shall not be flat or have obvious damaged treads or side walls, such as cuts or tears exposing fibers, missing treads sections, or imbedded sharp objects. No recapped/retread tires.

5.1.4.11 There shall be no leaking fluids such as anti-freeze, fuel, oil, hydraulic fluid, brake fluid, etc.

5.1.4.12 Air suspension system shall be in good working order, and shall not present the following:

5.1.4.12.1 Tears, holes or deterioration of rubber components and support systems.

5.1.4.12.2 Air leaks in the system.

5.1.4.12.3 A condition which causes the vehicle height to sag or to have an inability to control ride height.

5.2 Shipping Cask

5.2.1 The shipping cask must be a top-loading cask capable of transporting a single PL14-195 High Integrity Container (HIC). The cask shall meet the US DOT specification 7A, Type A packaging requirements of Title 49 Code of Federal Regulations. The shipping cask must also meet additional requirements as specified in paragraphs

5.2.1.1 through 5.2.1.3.

5.2.1.1 The inside diameter of the shipping cask shall be a minimum of 77 inches to a maximum of 79 inches.

The inside height of the shipping cask, with the primary lid installed, shall be a minimum of 80 inches to a maximum of 81 inches, as shown in Figure 1.

5.2.1.2 The shipping cask shall be directly bolted to the transportation trailer using a minimum of sixteen 1 ¼ inch steel bolts through the base plate, as shown in Figures 1 and 2. The bolt pattern may be similar to what is shown in Figure 2. However, the configuration of the bolts must allow the cask to be firmly attached to steel structural components of the trailer.

5.2.1.3 The shipping cask shall include a metal rain cover which must be securely attached to the cask and transportation trailer using a minimum of 4 chain ratchet binders.

5.3 Radiological Requirements

5.3.1 Radiological Contamination

5.3.1.1 The vendor shall inspect the shipping cask for radiological contamination (both loose and fixed) prior to delivery to Puget Sound Naval Shipyard.

5.3.1.2 The vendor shall measure gamma radiation fields with an AN/PDR-27 or Teletector model 6112 (IM-260/PDR probe), or equivalent instrument.

5.3.2 Loose Radioactive Contamination

5.3.2.1 The vendor shall base detection for loose radioactive contamination on dry swipes. The vendor shall take dry swipes over an area of 100 square centimeters (100 cm2) using a 1-1/8 inch diameter filter or paper disc.

5.3.2.2 Loose radioactive contamination on internal and external surfaces of the shipping cask and transport trailer shall not exceed the values listed in paragraphs 5.3.2.2.1 and 5.3.2.2.2 for beta-gamma and alpha contamination.

5.3.2.2.1 The vendor shall use dry wipes for detecting loose beta-gamma radioactivity. The vendor shall measure the dry swipes within 1/2 inch with a Geiger counter E 140N equipped with a DT-304/PDR probe, or equivalent.

The maximum loose beta-gamma emitting radioactive contamination shall be less than 450 picocuries/100 cm2 (1000 disintegrations per minute/100 cm2), equivalent cobalt-60.

5.3.2.2.2 The vendor shall use an AN/PDR-56, or DT-681/PDQ alpha survey meter or equivalent for detecting loose alpha radioactivity. The maximum loose alpha emitting radioactive contamination shall be less than 50 picocuries/100 cm2.

5.3.2.2.2.1 The vendor shall count swipes for a minimum of 30 seconds if using the DT-681/PDQ.

5.3.3 Fixed Radioactivity

5.3.3.1 The vendor shall measure fixed radioactivity on external surfaces and internal surfaces on the shipping cask and transport trailer using a Geiger counter E 140N equipped with a DT-304/PDR probe or equivalent instrument for beta-gamma radioactivity equivalent cobalt-60 and an AN/PDR-56, or DT-681/PDQ or equivalent for alpha radioactivity.

5.3.3.2 The externals of the shipping cask and transport trailer shall not exceed 450 picocuries/probe, beta-gamma radiation, and shall not exceed 50 picocuries/probe alpha.

5.3.3.2.1 The vendor shall provide PSNS & IMF with results of external surface radiological surveys on a Fixed Radioactivity Survey Report (FRSR) at least 10 working days prior to transporting the shipping cask and trailer to PSNS & IMF. The vendor shall not commence transporting the shipping cask and trailer to PSNS until PSNS & IMF evaluates and approves the survey results.

5.3.3.2.2 The vendor shall provide PSNS & IMF with results of internal surface radiological surveys on a Fixed Radioactivity Survey Report (FRSR) at least 10 working days prior to transporting the shipping cask and trailer to PSNS & IMF. The vendor shall not commence transporting the shipping cask and trailer to PSNS & IMF until PSNS & IMF evaluates and approves the survey results.

NOTE: A cask with fixed internal radioactivity levels less than 0.1 mRem/hr is preferred.

5.3.3.2.3 Provide the data via email to the Contracting Representative who will forward it to the government Technical Experts to evaluate the data and provide acceptance/rejection via reply-to-all within one week of receipt.

5.4 Lifting, Certification, and Testing Requirements

5.4.1 The vendor shall load test the lifting attachments for the shipping cask in accordance with the requirements of and within the periodicity of ANSI N14.6-1993 and the following:

5.4.1.1 The vendor shall provide the cask with two or four lift lugs, equally spaced on the cask body. The lifting and handling attachments (lift lugs, padeyes, etc.) shall be designed, fabricated, and installed to a minimum strength of 3:1 yield and 5:1 ultimate. The safety factor is based on the worst case stress based on the designed lifting configuration using the maximum allowed sling angles from vertical with the fully loaded cask at 57,350 lbs.

(combined weight of cask and payload). The Cask Empty Weight is 39,650 lbs., and the Maximum Rated Payload Capacity of Cask: 17,700 lbs.

5.4.1.2 For the cask, the load testing shall be 150% (+5%,-0%) of the combined cask weight which is the maximum weight of the assembled shipping cask and the maximum payload.

5.4.1.2.1 If the cask has two lift lugs, the vendor shall load test to 150% (+5%,-0%) of the maximum loaded weight for a minimum of ten minutes. The load test shall incorporate the additional forces based on the designed lifting configuration using the maximum allowed sling angles from vertical.

5.4.1.2.2 If the cask has four lift lugs, the vendor shall load test two diametrically opposed lift lugs to 150% (+5%,-0%) of the maximum loaded weight for a minimum of ten minutes. The load test shall incorporate the additional forces based on the designed lifting configuration using the maximum allowed sling angles from vertical.

The contractor shall repeat the load test for the remaining two diametrically opposite lift lugs.

5.4.1.3 The vendor shall provide the cask’s primary lid lifting assembly with two (up to three) lift lugs. The vendor shall load test the primary lift lugs to 150% (+5%,-0%) of the maximum weight (with secondary lid installed) for a minimum of ten minutes. The load test shall incorporate the additional forces based on the designed lifting configuration using the maximum allowed sling angles from vertical.

5.4.1.4 The cask’s secondary lid lifting assembly shall be provided with one (up to three) lift lugs. The vendor shall load test the secondary lid assembly lift lug(s) to150% (+5%,-0%) of designed weight for a minimum of ten minutes.

5.4.1.5 The cask’s protective (rain) cover assembly shall be provided with one (up to three) lift lugs. The vendor shall load test the protective cover assembly lift lug(s) to 150% (+5%,-0%) of the weight of the assembly for a minimum of ten minutes.

5.4.1.6 If any lift lug(s) are provided with a permanently-installed shackle (e.g. rain cover shackle), the vendor shall load test the shackle(s) to 200% (+5%,-0%) of the designed weight limit for a minimum of ten minutes.

5.4.2 The vendor shall perform a magnetic particle inspection of the entire lifting attachment and attachment welds for each shipping cask lift lug, primary and secondary lid lifting assemblies, protective (rain) cover lifting assembly, and any permanently-installed shackles in accordance with the equivalent NAVSEA technical publication T9074-AS-GIB-010/271 with acceptance criteria to MIL STD 2035A (Class 3 for welds), prior to service period.

5.4.3 The vendor shall perform the load tests, inspections, and the magnetic particle inspections prior to the service period with sufficient time that the certifications shall not expire prior to the end of the service period. There must also be enough time left before certification expiration to allow for schedule delays which could extend the service period up to four months.

5.5 Required Documentation and Equipment

5.5.1 The Vendor shall provide two complete sets of certification data. One set shall accompany the material and one set shall be forwarded to the contracting officer a minimum of 10 working days prior to the vendor commencing transport of the cask and trailer to PSNS & IMF.

5.5.2 The Vendor shall forward documents and listings of the applicable procedures and engineering drawings (e.g. cask user procedures for primary lid installation/removal, cask safety analysis report), radioactivity survey data, condition of the cask, rain cover, and trailer (with photographs) and the procedures to certify the cask for US DOT specification 7A, Type A, to the contracting officer a minimum of 10 working days before shipment of the cask and trailer to PSNS & IMF.

5.5.3 The Vendor shall forward a Certificate of Compliance (COC) certifying the Shielding Casks and Trailers provided under this contract comply with this Statement of Work and its associated requirements. The vendor shall also forward the safety analysis report demonstrating that construction methods, design, and materials are compliant with the US DOT 7A, Type A packaging requirements, to the contracting officer a minimum of 10 working days before shipment of the cask and trailer to PSNS & IMF.

5.5.4 The Vendor shall forward certifications and testing methods of the load testing to the contracting officer a minimum of 10 working days before shipment of the cask and trailer to PSNS & IMF.

5.5.5 Vendor shall forward certification of satisfactory non-destructive testing to the contracting officer a minimum of 10 working days before shipment of the cask and trailer to PSNS & IMF.

5.5.6 Vendor shall forward all documentation specified in 5.5.1 through 5.5.5 to the contracting officer.

5.5.7 The vendor shall provide, with the transportation cask and trailer, the following extra sets of fasteners (bolt, washer, lock washer, and nut, as applicable, to secure one bolted connection):

- 1 set of primary lid fasteners

- 1 set of secondary lid fasteners

- 1 set of cask base plate to trailer fasteners

6 Government-Furnished Property – None

7 Security Requirement

7.1 UNCLASSIFIED

7.2 Contractor personnel shall comply with all current badging and security procedure requirements for gaining access to Department of Defense (DoD) Installations/Government Sites. Access to Navy Installations or Sites may only be gained by obtaining a badge (either permanent or temporary) from the Visitor Control Center (VCC) in building 981 (Pass & ID). It is the contractor’s responsibility to check for, and obtain changes and updated information from the VCC on a continual basis.

7.3 The following Identification Cards (ID) are authorized by Naval Base Kitsap and Naval Station Everett to be used by visitors 12 years old or greater, while being escorted by a Civil Servant or Military member holding a current CAC Card, effective 2 February 2016:

- Drivers licenses from states other than Washington, Illinois, Minnesota, Missouri and New Mexico (unless WA, IL, MN, MI and NM licenses are the “Enhanced” version)

- U.S. passport

- U.S. passport card

- DHS trusted traveler cards ( Global Entry, NEXUS, SENTRI, FAST)

- Permanent resident card

- Border crossing card

- DHS-designated enhanced driver’s license

- Federally recognized, tribal-issued photo ID

- HSPD-12 PIV card

- Foreign government-issued passport

- Canadian provincial driver’s license or Indian and Northern Affairs-Canada card

- Transportation Worker Identification Credential (TWIC)

7.3.1 Contractor employees are required to have an individual identification badge as well as one (1) of the four (4) credentials listed below in their possession to gain access to job sites:

7.3.1.1 Common Access Card (CAC):

7.3.1.1.1 Contractors who have a need to access a government computer system may be eligible for a contractor CAC; most contractors are not eligible for a CAC. Contact the Contracting Officer’s Representative (COR) or Contracting Officer’s Security Officer for assistance as needed.

7.3.1.2 Single Day Pass:

7.3.1.2.1 Visiting vendors/contractors requiring infrequent access may obtain daily passes directly from the individual Navy Installation by submitting identification credentials for verification and undergoing a criminal screening/background check. A new pass is required to gain access each day. Passes are issued by Naval Base Kitsap (NBK). The Federal Bureau of Investigation’s (FBI) National Crime Information Center (NCIC) criminal database and FBI Terrorist Watch List will be checked daily prior to entry. In addition, NBK must check the Navy’s Consolidated Law Enforcement Operations Center (CLEOC) database to ensure an unescorted visitor has no objectionable criminal record, is not on the Terrorist Watch List and has not been previously debarred from entry onto any Navy Installation.

7.3.1.3 Defense Biometric Identification System (DBIDS) Program: Commander, Navy Installations Command (CNIC) has established the Defense Biometric Identification System (DBIDS) for access control to CNIC Installations via Entry Control Points (ECP). DBIDS is an enterprise identity management and perimeter installation access control solution in which Contractor personnel who enroll, and are approved, are subsequently granted access to the installation for a period up to three years, or the length of the contract, whichever is less, and are not required to obtain a new pass from the Base Pass and Identification Office for each visit. There are no fees associated with obtaining a DBIDS credential.

7.3.1.3.1 The Government performs background screening and credentialing. Throughout the year the Contractor employee must continue to meet background screening standards. Periodic background screenings are conducted to verify continued DBIDS participation and installation access privileges. DBIDS access privileges will be immediately suspended or revoked if at any time a Contractor employee becomes ineligible.

7.3.1.3.2 Access to Installation. All Contractor personnel shall obtain access to the installation through enrollment and registration into the Defense Biometric Identification System (DBIDS). The Contractor shall provide the Contracting Officer with the name of their designated Service Contractor Administrator (SCA) for enrollment in DBIDS. Contact the Contracting Officer’s Representative (COR) or Contracting Officer’s Security Officer for assistance as needed. Once enrolled, the Contractor must provide the DBIDS Registrar with an approved employee list and then direct their employees to register into DBIDS.

7.3.1.3.3 DBIDS Credentials. Contractor employees shall furnish a completed copy of the SECNAV 5512/1 form to obtain the required background check and visit the local Navy Installation Visitor Control Center to obtain a DBIDS credential once approved. The SECNAV 5512/1 form and additional information about DBIDS can be found at: https://www.cnic.navy.mil/om/dbids.html.

7.3.1.3.4 The Contractor may initiate the adjudication process when a background screen failure results in disqualification from participation in DBIDS and the Contractor employees do not agree with the reason for disqualification. The Contractor may also apply for a waiver when a background screening failure results in disqualification from participation in DBIDS. The Commanding Officer will be the final waiver determination authority.

7.3.1.3.5 The Contractor shall immediately collect employee DBIDS credentials and notify the Contracting Officer in writing:

- That an employee has departed the company without having properly returned or surrendered their DBIDS credentials.

- That there is a reasonable basis to conclude that an employee, or former employee, might pose a risk, compromise, or threat to the safety or security of the installation or anyone therein.

7.3.1.3.6 DBIDS Paper Passes. In the event that a visitor, vendor, or Contractor employee elects not to participate in DBIDS, the individual will be issued DBIDS paper pass from the Base Pass and Identification Office in order to access to the installation. The time standard for the validity of a pass to access an installation will be not more than thirty (30) days and may be further restricted based on local policy or by Federal, DOD, Navy, and CNIC guidance.

The Government will not be responsible for any cost or lost time associated with obtaining paper passes or added vetting or inspections incurred by non-participants in the DBIDS credentialing process.

7.3.1.4 Transportation Workers Identification Credential (TWIC):

7.3.1.4.1 Trucking and Longshoremen industry, to include Military Sealift Command (MSC) personnel are authorized to use this credential for unescorted access so long as the holder also has in his/her possession an original Bill Of Lading indicating that the delivery of goods is within the perimeter of the host installation, or in the case of MSC personnel, the holder has in his/her possession, a current Merchant Mariner identification card.

7.4 For contractor individuals that require higher level access requirements such as Nuclear Work Areas (NWA), Controlled Nuclear Information Areas (CNIA) areas or access to the Main Machinery Rooms of nuclear propelled ships or submarines, the Contractor’s Facility Security Officer ( FSO) shall; (1) submit each individual’s information into the Joint Personnel Adjudication System (JPAS) Visit Request program with Security Management Office (SMO) number 002515, and (2) Submit a completed DD254 (ensuring that block 10b is checked), and that the Naval Nuclear Propulsion Information (NNPI) security clause is listed in either the Security Section of the Statement of Work (SOW) or in the list of effective clauses for review by the Industrial Security Office.

Additionally, a separate Visit Request List Form (PSNS&IMF Form 5512) must be submitted to the Contracting Officer’s Security Officer.

7.5 The contractor shall prepare a Visit Request List Form with Cover Letter on Company Letterhead for all prime contractor and subcontractor personnel required to access Navy Vessels in the CIA. The visit request will be submitted via email to the Contracting Officer’s Security Officer at psnsimf.400scrty.fct@navy.mil. The cover letter will be a formal request for badging and access during the contract or delivery period of performance. The letter shall include (1) the purpose of access, (2) dates of validity (period of performance), (3) contract number, and

(4) name of subcontractors to be employed. The cover letter shall be signed by the Prime Contractors Facility Security Officer (FSO), and shall be received by the Contacting Officer’s Security Officer NO LESS THAN FIVE

(5) FULL WORKING DAYS PRIOR TO THE REQUESTED START DATE (8-10 calendar days prior is recommended). If red badges are required (for contractor personnel to access Restricted Data/Naval Nuclear Propulsion Information-NNPI) there are additional screening and higher level approval requirements, which could take considerably longer. Therefore, personnel identified in the Visit Request List must have a valid requirement to gain access to Restricted Data/NNPI. The prime contractor shall bear full responsibility for the accuracy of the Cover Letter and Visit Request List. Note: Submitting a cover letter and/or list with incomplete information, errors or a blanket list of all company personnel (especially for all red badges) can significantly delay processing requests and result in not being authorized access in time to begin the period of performance. With good cause, as determined by the Contracting Officer or appropriate Security Personnel, access requests may be refused or amended at any time. Updates to the Visit Request List shall be made on an as needed basis. The C400 security office may be contacted at (360) 476-0418 for questions regarding visit request processing.

7.6 The Visit Request List will be organized ALPHABETICALLY BY LAST NAME and contain the following information in column form (a spreadsheet template can be obtained by contacting the Contracting Officer’s Security Officer or COR).

- DOB (Date of Birth)

- POB (City & State)

- SSN (Social Security Number)

- CITIZENSHIP (Country)

7.7 Contractor employees must be U.S. citizens to access PSNS & IMF owned or controlled spaces unescorted, to include ship's engineering spaces. Foreign Nationals will be issued "Escort Required" white badges only. Additionally, an approved special security plan indicating the proposed visit locations and route to and from the location must be utilized. This requirement adds 3 working days to the visit request process. U.S. citizens working for Foreign Owned or Controlled Companies requiring access, unless they fall under a Defense Security Service (DSS) security clearance, will be issued Green badges only suitable for general CIA access. Access by U.S.

citizens working for Foreign Owned or Controlled Companies into more stringently controlled areas such as Nuclear Work Areas (NWA) require a security plan and escorts. Ensure personnel employed by Foreign Owned or Controlled Companies are clearly identified in Visit Request documents and in communications with the Contracting Officer's Security Officer. Security Clearance information, including confirmation of required red badges, shall be submitted to the Contracting Officer's Security Officer.

7.8 Badges will be picked up at the Naval Base Kitsap Visitor Control Center (VCC) in building 981. Each Contractor employee is REQUIRED to present proof of citizenship for on-site verification (not retention) at the VCC prior to obtaining a badge. This is a requirement for access into the CIA at PSNS&IMF, Bremerton, WA. All contractor employees requesting access shall be U.S. citizens. Acceptable proof of citizenship includes Original Birth Certificate (not a copy) or Current U.S. Passport.

7.9 Lack of a proper ID badge will result in denial of access to the Naval Installation, facility and/or the ship, resulting in a delay at the Contractor’s expense. If Contractor employees are providing a document with no photo, they shall also be required to present a current authorized official issue photo ID listed in paragraph 7.3 above.

7.10 Badges must be displayed on the outermost garment above the waist at all times. The only exception would be while the contractor personnel are engaged in work which could cause a safety hazard if the badge were worn while performing the work.

7.11 Contractor personnel are authorized to retain temporary badges until the badge expiration date or termination of employment. Upon badge expiration date, termination of employment, or completion of contract, the Contractor is required to immediately notify the Contracting Officer’s Security Officer and return all badges.

7.12 Lost or misplaced badges shall be reported immediately to the Contracting Officer’s Security Officer and COR. If after hours report lost or misplaced badges immediately to the Navy Emergency Services Command (NESCOM) at 360-476-3393.

7.13 Immediately notify the Contracting Officer’s Security Officer and the COR of any access problems or security concerns.

7.14 To grant Contractor Vehicle Access to the PSNS & IMF CIA, a list of vehicles, a copy of each vehicle registration and a justification for access shall be submitted for processing via PSNS & IMF Form 5530/25.

Contractors are required to stop at PSNS & IMF Pass & ID Office with proof of valid, current Driver’s License, vehicle registration and proof of insurance, prior to receiving access.

7.15 Contractor owned computers and associated equipment (e.g. cables, removable storage media, etc.) must be inspected by Code 109 prior to bringing them into the CIA and shall have a PSNS&IMF AIS Security Survey for Visitor Computer Equipment Form PSNS&IMF 5239/49 (Rev. 8-07), approved by Code 109, on site at all times.

7.16 Contractor Employees shall adhere to PSNS&IMF NOTICE 5239.6 PORTABLE ELECTRONIC

DEVICE (PED) GUIDANCE.

7.16.1 Lap-top Computers and/or cell phones equipped with cameras are STRICTLY PROHIBITED inside all PSNS & IMF owned or controlled spaces or property (the CIA, DMF & YOKO Enclaves, etc.) and subject to seizure by Installation Police and Shipyard Security Personnel.

7.16.2 ALL portable electronic devices are STRICTLY PROHIBITED onboard submarines (e.g. phones, mp3 devices, computing devices, recording equipment, removable storage media, etc.).

8 Place of Performance/Delivery Address

8.1 PSNS & IMF, 1400 Farragut Ave, Bremerton, WA 98314

9 Period of Performance

9.1 The three months (90 calendar days) service period for the shipping cask and transport trailer shall start the day the equipment leaves the vendor’s site while in transit to PSNS & IMF.

9.2 Service accrual will cease when the PL14-195 HIC is unloaded at the disposal site.

9.3 The cask and shipping trailer will spend approximately two months at PSNS & IMF while being loaded with a PL14-195 HIC and two weeks in transport between PSNS & IMF and the US Ecology with two weeks for unplanned delays.

9.4 PSNS & IMF will notify the vendor of any delays of loading the PL14-195 HIC at PSNS, or offloading the PL14-195 HIC at US Ecology.

9.5 The expected period of performance of the 87 consecutive calendar days is between 6 July 2021 through 30 September 2021.

9.6 PSNS & IMF shall notify the contractor of the specific date of period of performance no later than 15 days prior to the onsite need date for cask and transportation trailer.
