EVALUATION FACTORS – COMMERCIAL ITEMS:
(1) Factor 1 Past Performance/ Experience:
(i) Solicitation Submittal Requirements:
(1) Provide a list of up to 3 contracts similar in size and complexity of this solicitation completed during the past 3 years and all contracts currently in process must be provided. Contracts listed may include those entered into by the Federal Government, agencies of state and local governments and commercial customers. Projects that more closely resemble the scope of the solicited scope of work may be rated more highly. Complete and include with your proposal, Past Performance Form, for each project referenced.
The Past Performance Questionnaire ( NAVFAC Past Performance Questionnaire, Form PPQ-0) included in the solicitation is provided for the offeror to submit to the client for each project the offeror includes in its proposal for Factor 1 – Past Performance/Experience. Ensure correct phone numbers and email addresses are provided for the client point of contact. Completed Past Performance Questionnaires are to be mailed or faxed from the client back to the Offeror and the Offeror shall submit with their technical proposal. In order for the client evaluation to be considered, the Past Performance Questionnaires must be submitted no later than the proposal due date. If the offeror is unable to obtain a completed PPQ from a client for a project(s) before proposal closing date, the offeror should complete and submit with the proposal the first page of the PPQ (Attachment ‘J-0200000-19”), which will provide contract and client information for the respective project(s). Offerors should follow-up with clients/references to ensure timely submittal of questionnaires. If the client requests, questionnaires may be submitted directly to the Government’s point of contact, Debra S. Dills via email at debbie.dills@navy.mil prior to proposal closing date. Offerors shall not incorporate by reference into their proposal PPQs previously submitted for other RFPs.
However, this does not preclude the Government from utilizing previously submitted PPQ information in the past performance evaluation.
Also include performance recognition documents received within the last three years such as awards, award fee determinations, customer letters of commendation, and any other forms of performance recognition.
(2) Evaluation of past performance will often be subjective. The evaluation of past performance will include the contractor’s history of reasonable and cooperative behavior, commitment to customer satisfaction, record of conforming to applicable law and industry standards, quality of workmanship, record of recommending and/or implementing innovative approaches and/or technologies; record of controlling costs;
safety performance record, and adherence to schedules. Key personnel and subcontractor’s past performance may also be evaluated.
(3) The Government reserves the right to contact any other sources of information that may have knowledge or information on an offeror’s relevant past performance history.
(4) Offers, which lack relevant past performance history, will receive a "No Rating" for this factor. Any offeror’s proposal which has no relevant past performance history, while rated neutral, may not represent the most advantageous proposal to the Government and thus, may not be competitive when compared to proposals of other offerors.
(ii) Basis of Evaluation:
The degree to which past performance evaluations and all other past performance information reviewed by the Government (e.g., PPIRS, performance recognition documents, and information obtained for any other source) reflect a trend of satisfactory performance considering:
 A pattern of successful completion of tasks;
 A pattern of deliverables that are timely and of good quality;
 A pattern of cooperativeness and teamwork with the Government at all levels
(task managers, contracting officers, auditors, etc.);
 Tasks that are identical to, similar to, or related to the task at hand;
 A pattern of continually providing the highest level of customer satisfaction; and A respect for stewardship of Government funds.
 Proposals that fail to provide required and complete information may be rated lower.
(2) Factor 2, Performance Narrative:
(i) Solicitation Submittal Requirements:
Contractor shall submit a performance/construction narrative detailing how the work will be accomplished. The following questions/answers must be included.
• Provide your proposed Integrated Maintenance Program (IMP) (Schedule)
• What frequency do you propose for maintenance to ensure continuous, trouble free
VTE systems?
(ii) Basis of Evaluation: The Integrated Maintenance Program shall clearly identify all task required to complete all work requirements and schedule of such.
Factor 3, Management Approach:
(i) Solicitation Submittal Requirements:
Contractor shall clearly demonstrate a feasible approach for controlling and managing the entire project and offeror’s general management and administration structure. The following questions/answers must be included:
• How will incoming work be received, assigned to an employee, scheduled, etc., without adversely impacting existing, uncompleted work.
• What is your plan to ensure personnel have the necessary training and certification to accomplish the specialty work requirements specified? How do you propose to ensure this training and certification is maintained current?
• Explain how call backs will be received during and after regular working hours, how work will be scheduled and assigned to an employee, and how emergency calls will be responded to after regular working hours.
• Based on a four week month how many days will an elevator mechanic be on center each week? (minimum of 1 required)
• What is your approach to effectively coordinate with the VTE users in order to accomplish work within specified time limits and per work schedules? How is this approach customer-service oriented?
• How many people will be on site working together conducting annual and semi-annual inspections in the presents of a government furnished NAVFAC Certified Elevator Inspector.
• Subcontractor Management, for work that is to be subcontracted, provide any proposed subcontractor’s name/and or proposed subcontractor selection and management processes that will be used to ensure consistent satisfactory performance of subcontracts. If work is to be self performed, please indicate so in your proposal.
• Management Plan shall include:
(1) Contractor’s Safety Plan
(2) Activity Hazard Analysis
(3) Copy of State Business License and Certifications
(ii) Basis of Evaluation:
Management ability will be evaluated as follows: Evaluation will focus on the offeror’s ability to accomplish the work including subcontractors and work at multiple sites simultaneously. The offeror’s quality control organization and procedures are more than adequate to assure high quality performance. The proposal clearly details a plan responding to unforeseen/unknown conditions. The contractor clearly demonstrates the ability to establish, communicate progress, and perform to a construction schedule.
(4) Factor 4, Safety:
(i) Solicitation Submittal Requirements: The Offeror shall submit the following information: (For a partnership or joint venture, the following submittal requirements are required for each contractor who is part of the partnership or joint venture; however, only one safety narrative is required. EMR and DART Rates shall not be submitted for subcontractors.)
(1) Experience Modification Rate (EMR): For the three previous complete calendar years 2008-2010, submit your EMR (which compares your company’s annual losses in insurance claims against its policy premiums over a three year period). If you have no EMR, affirmatively state so, and explain why. Any extenuating circumstances that affected the EMR and upward or downward trends should be addressed as part of this element. Lower EMRs will be given greater weight in the evaluation.
(2) OSHA Days Away from Work, Restricted Duty, or Job Transfer (DART) Rate: For the three previous complete calendar years 2010-2013, submit your OSHA Days Away from Work, Restricted Duty, or Job Transfer (DART) Rate, as defined by the U.S.
Department of Labor, Occupational Safety and Health Administration. If you cannot submit an OSHA DART Rate, affirmatively state so, and explain why. Any extenuating circumstances that affected the OSHA DART Rate data and upward or downward trends should be addressed as part of this element. Lower OSHA DART Rates will be given greater weight in the evaluation.
(3) Technical Approach for Safety: Describe the plan that the Offeror will implement to evaluate safety performance of potential subcontractors, as a part of the selection process for all levels of subcontractors. Also, describe any innovative methods that the Offeror will employ to ensure and monitor safe work practices at all subcontractor levels. The Safety Narrative shall be limited to two pages.
(ii) Basis of Evaluation: The Government is seeking to determine that the Offeror has consistently demonstrated a commitment to safety and that the Offeror plans to properly manage and implement safety procedures for itself and its subcontractors. The Government will evaluate the Offeror’s overall safety record, the Offeror’s plan to select and monitor subcontractors, any and innovative safety methods that the Offeror plans to implement for this procurement. The Government’s sources of information for evaluating safety may include, but are not limited to, OSHA, NAVFAC’s Facility Accident and Incident Reporting (FAIR) database, and other related databases. While the Government may elect to consider data from other sources, the burden of providing detailed, current, accurate and complete safety information regarding these submittal requirements rests with the Offeror. The evaluation will collectively consider the following:
- Experience Modification Rate (EMR)
- OSHA Days Away from Work, Restricted Duty, or Job Transfer (DART) Rate
- Offeror Technical Approach to Safety
- Other sources of information available to the Government
(1) Experience Modification Rate (EMR): The Government will evaluate the EMR to determine if the Offeror has demonstrated a history of safe work practices taking into account any upward or downward trends and extenuating circumstances that impact the rating. Lower EMRs will be given greater weight in the evaluation.
(2) OSHA Days Away from Work, Restricted Duty, or Job Transfer (DART) Rate: The Government will evaluate the OSHA DART Rate to determine if the Offeror has demonstrated a history of safe work practices taking into account any upward or downward trends and extenuating circumstances that impact the rates. Lower OSHA DART Rates will be given greater weight in the evaluation.
(2) Technical Approach to Safety: The Government will evaluate the narrative to determine the degree to which subcontractor safety performance will be considered in the selection of all levels of subcontractors on the upcoming project. The Government will also evaluate the narrative to determine the degree to which innovations are being proposed that may enhance safety on this procurement. Those Offerors whose plan demonstrates a commitment to hire subcontractors with a culture of safety and who propose innovative methods to enhance a safe working environment may be given greater weight in the evaluation.
Notes:
1. The three elements of safety, as listed above, are not sub-factors. The evaluators should collectively consider all elements of safety when assigning an overall adjectival rating for this factor. The TEB will need to make a qualitative determination of the rating for this factor, taking into account the risk ratings for EMR and DART and the subjective evaluation of the narrative.
2. Evaluators will not average the rating from the three years for the EMR and the DART.
3. The following should be utilized by evaluators as a general guideline to evaluate the EMR and DART rates:
Risk EMR Very Low Risk Less than 0.6 Low Risk From 0.6 to less than 0.8 Moderate Risk From 0.8 to 1.0 High Risk Greater than1.0 to 1.1 Extremely High Risk Greater than 1.1
