About this file
This is a Quality Assurance Surveillance Plan (QASP) matrix for sanitary tank cleaning and sanitation services at Portsmouth Naval Shipyard. The document establishes performance standards, acceptable quality levels (AQL), surveillance methods, and government actions for contract non-compliance across three sanitary tanks.
The cleaning requirements mandate removal of all wastes and loose materials using only pressure washing and hand tools—no power tools permitted. Tanks must be cleaned to Society for Protective Coatings Surface Preparation Standard No. 1 (SSPC-SP 1). Tank-specific requirements include calcium buildup removal from Tank #1, removal of 100-400 square feet of loose and flaking epoxy paint from Tank #2 requiring exposure to steel substrate, and extensive degreasing via hand wiping with rags and degreasing agents on Tank #3. All anodes must be removed prior to final cleaning and turned over to Portsmouth Naval Shipyard personnel. Sanitization requires 24-hour advance notification to the Project Management Team and Code 133 Inspector, with the inspector observing the addition of specified quantities of sodium hypochlorite solution (5% or 10% concentrations vary by tank size). Tanks must sit for a minimum four hours with sanitizing solution, then either refilled with potable water for one hour or low-pressure washed. Bacteriological testing of washwater runoff is mandatory, with limits set at less than 235 CFU/100 mL for E. coli and less than 61 CFU/100 mL for enterococci. The Government action for any AQL failure includes requiring re-work at contractor expense, with an additional 5% penalty of the total Contract Line Item Number value assessed for each failure after the first inspection. Acceptable performance is rated on a descriptive scale ranging from Exceptional to Unsatisfactory, with Satisfactory defined as meeting all contractual requirements with only minor non-conformances and timely corrective actions.

| Ratings | Descriptive Rating System |
| Exceptional | Performance meets all contractual requirements and exceeds many to the Government’s benefit. Problems are rare, and the contractor takes highly effective corrective action. For this SOW, an "Exceptional" rating would require flawless execution, proactive communication of potential issues with well-researched solutions, and delivery of all components and documentation ahead of schedule. |
| Very Good | Performance meets all contractual requirements and exceeds some to the Government’s benefit. Problems are infrequent, and the contractor demonstrates effective corrective action. For example, all technical requirements are met, and the contractor provides exceptionally clear and well-organized documentation that facilitates rapid government review. |
| Satisfactory | Performance meets all contractual requirements. This is the expected level of performance. Any non-conformances are minor, and the contractor's proposed corrective actions are timely and effective. The work is technically sound, and all deliverables are submitted on time. |
| Marginal | Performance does not meet some contractual requirements. A "Marginal" rating may be assigned if a significant non-conformance is identified (e.g., failure to meet a key test parameter like breakaway torque) or if required documentation is repeatedly late or incomplete. This reflects a need for increased government oversight. |
| Unsatisfactory | Performance does not meet most contractual requirements, and recovery is not likely in a timely manner. This rating would be assigned for multiple significant failures, a critical uncorrected deficiency, or a failure to provide required OQE, which jeopardizes the entire project. |
