MANDATORY SITE VISIT:
A site visit is required for this solicitation to be eligible for award. The contractor shall visit the project site; collect required data about existing structures, surroundings, appurtenance and all items that will be affected by this project or related to this project; and verify existing conditions.
The contractor shall be responsible for the verification of the actual site condition and the accuracy of this information to ensure adequacy of the design and successful construction.

1.0 GENERAL
1.1 General Description:
Project Title: HANGAR-111 replace failing, antiquated lighting systems with new energy efficient lighting to meet mission requirements.
Project Description: Removal of failing, antiquated High Bay lighting systems and replace with new energy efficient lighting to meet mission requirements.
Contractor to provide lump sum price to replace noted light fixtures as called out in the below scope of work for both hangar bays.
1.2 General Conditions:
1. Work shall be performed in accordance with scope of work.
2. No foreign nationals allowed.
3. Government tenant to provide staging area for materials and equipment near work area.
4. Government tenant to provide access to area for workers, material deliveries, and equipment.
5. Prior to any work starting on-site the contractor will provide a narrative technical proposal outlining how the contractor will perform the project, how the project will be phased, and what measures will be taken by the contractor so that construction of the project will have minimal effect on the building operations.
6. All work to be in accordance with and not limited to the following:
a. OSHA and Army Corp of Engineers Safety Manual, EM-385-1-1 Safety requirements, Current Edition.
b. Safety and Health Standards for Construction Industry (29 CFR PART 1929 2007)

9. Secured fire alarm. Contractor shall notify the contracting officer representative IO days in advance.
10. Submit an Inventory Adjustment Form for each piece of equipment removed and each piece of equipment installed to the NAWCAD point of contact (POC).
11. The contractor shall visit the project site; collect required data about existing structures, surroundings, appurtenance and all items that will be affected by this project or related to this project; and verify existing conditions. The contractor shall be responsible for the verification of the actual site condition and the accuracy of this information to ensure adequacy of the design and successful construction.
1.3 Location: NAVAL AIR STATION PATUXENT RIVER, MD Hangar 111.
1.4 Time of Completion: Work shall be prosecuted diligently and shall be completed for use within the time indicated in the award document. Duration time is calendar days not workdays. Completion date will be 1!!..days from date of award.
Contractor is required to furnish proof of any cause for delay and shall request an extension of the completion date in writing through the Contracting Officer.
1.5 Site Visit: See solicitation for site visit details. Contractor shall verify all conditions, dimensions and methods on site(s) at time of site visit.
1.6 Payment: Payment will be made in one lump sum after completion and acceptance of the work by the Government Representative. Progress payment decisions will be on a case-by-case basis. All invoices shall be submitted through WAWF.
1.7 Government Furnished Materials/Utilities: All material, equipment, labor, and supervision will be supplied by the contractor with any exceptions as noted below. The contractor shall provide and maintain, at his/her expense, the necessary service lines from existing Government outlets to the site of the work.
Utilities specified above will be furnished at no cost to the contractor. In the event that the Government is unable to provide the required types of utilities, the contractor shall, at his/her expense arrange for the required utilities. No foreign nationals allowed.
1.8 Execution of Work: Work shall be performed during normal duty hours 0600-1630, Monday through Friday. Work performed after normal working hours shall be coordinated with the NAWCAD POC. No work shall be performed on Government holidays or weekends unless expressly allowed by the Officer in Charge, NAWCAD Contracts.
1.9 Contractor's Production Report: The contractor is required to submit a "Contractor
Production Report". The form shall be completed daily and be provided
41Page electronically to the NAWCAD POC, by 10:00 AM the following day.
The existing commands in Hangar 111 require knowledge of work being performed.
Information regarding working in/ through building spaces, power outages (large or small) and the effect on the building and mission needs to be communicated.
Therefore; the contractor shall have to provide a written schedule of daily work, three weeks before starting the project. The schedule is to provide the following information:
• The date, phasing, time of day and duration that work will be performed for each of the tasks and outages.
• Room/area locations in which the work will be performed and which the outage will effect.
• Additional information is to be provided as per the contracting officer's requirements.
The schedule is to be updated with the contracting officer POC on a daily basis.
1.10 Submittals: The Government Representative shall approve submittals prior to the start of work. The contractor shall submit the following cut sheets with the cost proposal within 15 days after award:
1. All material and equipment including but not limited to: 400Hz system equipment and material required for installation of systems.
2. Activity Hazard Analysis in accordance with EM 385-1-1.
3. Fall protection plan.
4. Manufacturer's Data/Catalog Cut sheets for equipment/materials.
5. Work Schedule.
6. Inventory Adjustment Form.
7. Prior to any work starting on site the contractor will provide an updated technical narrative outlining how the contractor will perform the project, how the project will be phased, and what measures will be taken by the contractor so that construction of the project will have minimal effect on the building operations.
8. Preconstruction Submittals
Within 36 days of contract award but not less than 30 days prior to commencing any work on site, the Contractor shall submit the following for review and approval.
(a) Shop Drawings

(b) Nameplates Nameplate illustrations and data shall be approved by the Contracting Officer before installation.
(c) Provide a one-line drawing showing branch/feeder circuits and include equipment. Provide the following information on drawing:
(i) Approximate conduit length
(ii) Conductor sizes: provide conductor sized per NEC 70, show voltage drop calculations, note adjustments to conductor size for installation having more than three current-carrying conductors and correction for ambient temperature.
(iii) Conduit size and material type (EMT, PVC, RMC etc...).
(iv) Major junction/splice boxes: indicate sizes of boxes and if any splicing of conductors.
(v) Fire seal assembly insulation drawings and their ratings. Provide existing wall fire rating for wall that are being penetrated.
(d) Product Data
Six copies of annotated descriptive data to show the specific model, type, and size of each item. Catalog cut sheets shall also indicate the UL listing. The data shall be highlighted to show model, size, and options that are intended for consideration. Data shall be adequate to demonstrate compliance with all contract requirements. Product data for all equipment shall be combined into a single submittal. Provide an equipment list identifying the type, quantity, make, and model number of each piece of equipment to be provided under this submittal. The equipment list shall include the type, quantity, make and model of spare equipment and those shown on the shop drawings.
(e) Operation and Maintenance (O&M) Instructions Six copies of the Operation and Maintenance Instructions. The O&M Instructions shall be prepared in a single volume or in multiple volumes, with each volume indexed, and may be submitted as a Technical Data Package. Manuals shall be approved prior to training. The Interior Fire Alarm and Mass Notification System Operation and Maintenance Instructions shall include the following:
(i) Five copies of the Manufacturer Data Package.
(ii) Operating manual outlining step-by-step procedures required for system start-up, operation, and shutdown. The manual shall include the manufacturer's name, model number, service manual, parts list, and preliminary equipment list complete with description of equipment and their basic operating features.
(iii) Maintenance manual listing routine maintenance procedures, possible breakdowns and repairs, and troubleshooting guide. The manuals shall include conduit layout, equipment layout and simplified wiring, and control diagrams of the system as installed.
(iv) A final Equipment List shall be submitted with the Operating and Maintenance (O&M) manual.
8. As-Built Shop Drawings

The drawings shall show the system as installed, including deviations from both the project drawings and the approved shop drawings. These drawings shall be submitted within two weeks after the final Government test of the system. At least one set of the as-built (marked-up) drawings shall be provided at the time of, or prior to the final Government test. Contractor may choose to work with NAWCAD ISE team to create the final drawings.
Refer any questions to NAWCAD POC.
1.11 Clean-Up of Site, shall be performed on a daily basis. Contractors are not permitted to use Government dumpsters or other contractor's dumpsters for disposal of debris. Daily and final clean up required. The contractor shall ensure that all trash and debris created by this project is cleaned up and removed off site. Customer to provide area for a dumpster TBD for duration of job.
1.12 Foreign Object Debris (FOD), is any particle or substance, alien to an aircraft or system, which could potentially cause damage. Contractor and their employees shall controlling and containing FOD generated by the construction.
1.13 Hot Work: A hot work permit is required as needed. A permit may be obtained through the Government inspector with 48 hours prior notice. Hot work in electrical or mechanical room space will require a gas-free test prior to the Government fire inspector granting approval. In accordance with the Government fire inspector's requirements, the contractor shall perform this test daily. The Government fire inspector shall approve the instrument used.
1.14 Dust Control: The amount of dust resulting from demolition shall be controlled to prevent the spread of dust to occupied portions of the construction site and to avoid creation of a nuisance in the surrounding area. Use of water shall not be permitted when it will result in, or create, hazardous or objectionable conditions such as ice, flooding and pollution.
1.15 Inspection of Work: Contractor shall inspect all work in accordance with FAR
52.246-12 "Inspection of Construction."
1.16 Lockout/Tagout: The contractor shall apply, monitor and remove (upon completion of work) proper lockout and tag out devices on all equipment undergoing repairs in accordance with the latest version of the U.S. Army Corps of Engineers Safety and Health Requirements Manual, EM-385-1-1.
1.17 Warranty: All equipment and parts furnished and installed by the contractor shall be warrantied against defects and workmanship for a period of not less than one year.
Additionally covering the manufacturers' warranty of 5 years for light fixtures from the date of acceptance at no additional cost to the Government. Manufacturer's warranty for all parts will take effect upon the expiration date of the contractor's one-year warranty.
1.18 NOTE: Any changes shall be submitted back to a contracting official for approval prior to execution of any changes.

1.19 Attached Drawings and documents: The following referenced drawings, sketches, pictures, reports and other documents accompany this statement of work and are a part thereof. Drawings are the property of the Government and shall not be used for any purpose other than that contemplated by the statement of work.

2.0 DETAILED DESCRIPTION
Project Description: This is a REPAIR project intended to provide new 400 Watt LED high bay lighting systems as a replacement for the existing failing high bay lighting systems. The contractor shall provide all labor, material, equipment, and other incidental work required to complete this project. Other incidental work required includes any and all items necessary to ensure a complete, usable and maintainable final product including the demolition work not specifically stated elsewhere in this Statement of Work (SOW). Work includes, but is not limited to, the following:
Note: Lockout/tag out required when removing/installing, working on or relocating electrical devices.
Note: All debris requires removal from building the same day as produced.
DEMOLITION
2.1 Removal and proper disposal of existing high bay light fixtures. Quantity and locations of each fixture per site visit instructions.
INSTALL
2.2 Electrical
a) The contractor shall locate as required: outlets, panels, equipment, light fixtures, conduit, etc..., in the field with the contracting officer POC. No electrical equipment/device shall be located so that the lowest part of the box, conduit or equipment is below 20" above hangar finished floor.
b) The contractor is to perform their installation as per the latest National Electrical Code
(NEC)/ National Fire Protection Association (NFPA 70) and all applicable NFPA codes which are applicable to this Scope of Work. Additionally as per manufacturer's requirements.

c) Contractor is to review and follow all building codes (to include but not be limited to NEC/NFPA 70 Chapter 5 special Occupancies) with regards to location, installation and construction of equipment, conduits and devices installed within aircraft hangar.
d) All equipment, large junction boxes, disconnects and panels, shall be properly marked using engraved plastic signage appropriate for the environment in which the equipment is located. Include equipment name, voltage, name of panel from which the equipment is fed, location of feeder panel, and circuit breaker number.
e) Provide grounding and bonding throughout the installation as required by the latest
National Electrical Code NFPA 70.
f) Contractor to assume that all wiring and controls are in good working order for the purpose of pricing this task.
g) Provide new 400Watt LED fixtures equal to, or better than NICOR HBC4-400 Watt High
Bay. Fixtures to meet the NAWCAD Hangar lighting modernization and standardization instruction/requirements. Contractor to confirm required branch circuit voltage.

3. ADMINISTRATIVE AND SAFETY REQUIREMENTS
3.1 Directives: The contractor and his employees shall comply with all referenced regulations, directives, and Instructions. Applicable Department of Defense (DOD), Secretary of the Navy (SECNAV), Chief of Naval Operations (OPNAV), and other directives, instructions, and regulations are available from the Contracting Officer.
3.2 Station Regulations:
3.2.1 The contractor and his employees shall become acquainted with and obey all
Government regulations as posted or as requested by the Contracting Officer.
3.2.2 The contractor shall participate actively in the activity energy conservation program. The contractor shall comply with the base energy conservation program and shall become familiar with its requirements. Use of high-energy consuming tools or equipment shall be approved by the Contracting Officer prior to use.
3.3 Fire Protection: The contractor and his employees shall know where fire alarms are located and how to tum them on. The contractor shall handle and store all combustible supplies, materials, waste and trash in a manner that prevents fire or hazards to persons, facilities, and materials. Contractor employees operating critical equipment shall be trained to properly respond during a fire alarm or fire in accordance with activity instructions.
3.4 Environmental Protection: The contractor shall comply with all federal, state and local environmental protection laws and the regulations and standards. The contractor shall coordinate all environmental protection matters with the Contracting Officer. If a regulatory agency assesses a monetary fine against the Government for violations resulting from contractor's actions, the contractor shall reimburse the Government for the fine and related costs. The contractor shall clean up any oil or chemical spills resulting from his operations at his own expense. The contractor shall not create a nuisance or hazard to the health of military or civilian personnel.

3.5 Disposal: Debris, rubbish, non-hazardous waste and non-usable material resulting from the work under this contract shall be disposed of by the contractor at his expense off Government property, unless otherwise directed. In either case, the contractor shall dispose of all hazardous waste in accordance with all applicable environmental law, including but not limited to, the Resource Conservation and Recovery Act and its associated state and local regulations. Prior to disposal of any hazardous waste, the contractor shall obtain approval from the Contracting Officer.
3.5.1 The contractor shall recycle all metal being removed from this project. This would include but is not limited to all flashing and edging. Segregate and recycle all debris generated by the work and remove off station to a licensed facility. Debris shall be removed and transported in such a manner that prevents spillage on streets or adjacent areas. Local regulations regarding hauling and disposal shall apply.
3.5.2 The contractor shall provide copies of all landfill and disposal tickets to the government Environmental Specialist within five days of removal from base.
3.5.3 Submit a report with invoice that includes the following:
a. Amount (by weight) and type of waste materials disposed of in a landfill or incinerator. Provide destination and the tip fee per ton.
b. Amount (by weight) and type of materials salvaged for sale, salvaged for reuse, and recycled. Provide destination and revenue generated for each material.
3.6 Safety Requirements and Reports
All work shall be conducted in a safe manner and shall comply with all requirements of the latest version of the U.S. Army Corps of Engineers Safety and Health Requirements Manual, EM-385-1-1. The contractor shall be responsible for instructing his employees in appropriate safety measures and shall not permit them to place machines and other equipment in traffic lanes or other locations in such a manner as to create safety hazards. The contractor shall employ a competent person as defined in EM-385-1-1 to function as the Site Safety and Health Officer (SSHO).
The following paragraphs denote minimum safety requirements. Contractor shall consult EM-385-1-1 for additional information or any safety issues not discussed in this specification.
3.6.1 Personal Protective Equipment Minimum Requirements:
a. Head Protection: All persons working in or visiting hard-hat areas shall be provided with and required to wear a Type I or Type II, Class G head protection meeting the requirements of ANSI Z89.l. Hardhat areas are those areas with potential hazard of head injury.
b. Clothing: Employees shall wear clothing suitable for the weather and work conditions: the minimum for construction sites shall be short sleeve shirt, long pants (excessively long or baggy pants are prohibited).

c. Protective Footwear: Employees shall wear safety-toed boots meeting ANSI Z4 1 while working on construction sites.
d. Eye and Face Protection: All eye and face protection shall meet the requirements of ANS I/ASSE Z87 .1, employees shall also use eye side protection.
e. High visibility vests: Vests will be worn when working around construction equipment or when working on highways.
3.6.2 Display of Safety Information: Within one calendar day after commencement of work, erect a safety bulletin board at the job site. The following information shall be displayed on the safety bulletin board in clear view of the on-site construction personnel, maintained current, and protected against the elements and unauthorized removal.
a. Map denoting the route to the nearest emergency care facility.
b. Emergency phone numbers.
c. Copy of AHA's.
d. OSHA 300A form.
3.6.3 Emergency Medical Treatment: Contractor shall arrange for their own emergency medical treatment. The Government has no responsibility to provide emergency medical treatment.
3.6.4 Use of Hand and Power Tools: Hand and power tools shall be of a manufacturer listed by a nationally recognized testing laboratory for the specific application for which they are to be used. Use, inspection and maintenance:
a. Hand and power tools shall be used, inspected, and maintained in accordance with the manufacturer's instructions and recommendations and shall be used only for the purpose for which designed.
b. Hand and power tools shall be inspected, tested, and determined to be in safe operating condition before use. Continued periodic inspections shall be made to assure safe operating condition and proper maintenance.
c. Hand and power tools shall be in good repair and with all required safety devices installed and properly adjusted. Tools having defects that will impair their strength or render them unsafe shall be removed from service.
d. Loose and frayed clothing, loose long hair and dangling jewelry (including dangling earrings, chains and wristwatches) shall not be worn while working with any power tools.
e. Guarding - Power tools designed to accommodate guards shall be equipped with
121Page such guards:
• All guards shall be functional.
• Reciprocating, rotating, and moving parts of equipment shall be guarded if exposed to contact by Employees or otherwise create a hazard.
3.6.5 Portable Extension Cords: Portable extension cords shall be sized in accordance with manufacturer ratings for the tool to be used and shall be protected from damage. All damaged extension cords shall be immediately removed from service. Portable extension cords shall meet the requirements ofNFPA 70.
3.6.6 Ladders: All portable ladders shall be of sufficient length and shall be placed so that workers will not stretch or assume a hazardous position. The top or top step of a stepladder shall not be used as a step unless it has been designed to be so used by the manufacturer. Ladders may be used as work platforms only when use of small hand tools or handling of light material is involved.
3.6.7 Lockout/Tag Out: Coordinate all control activities with the designated Government representative. Apply, monitor and remove proper lockout and tag out devices.
3.6.8 Sanitation Requirements: Provide drinking water and toilets as needed for construction personnel.
3.6.9 Fire Protection: Provide two ABC fire extinguishers at the work site to guard against potential fires.
3.6.10 Fuel-Powered Tools: When fuel-powered tools are used, they will not be fueled while running, while hot, or near an open flame.
3.6.11 Machinery and Mobile Equipment: The contractor shall complete an equipment checklist for any construction equipment e.g. backhoes, lift trucks, bobcats, etc. that will be used on site.
3.6.12 Excavation: All digging requires the contractor to contact the NAVFAC utilities department David Dement@ (301) 757-4882 min. 14 days prior to any excavation.
3.6.13 Confined Space: For confined space areas follow procedures outlined in Section 34 of
EM-385-1-1.
3.6.14 Activity Hazard Analysis: The principle purpose of an Activity Hazard Analysis
(AHA) is to reduce the overall risk of the hazards associated with construction work.
An AHA will be developed by the contractor for every operation involving a type of work presenting hazards not experienced in previous project operations, or where a new work crew or subcontractor is to perform work.
The analysis shall identify and evaluate hazards and outline the proposed methods and techniques for the safe completion of each phase of work. At a minimum, define
131Page activity being performed (can use the Definable Features of Work). A Definable Feature of Work (DFOW) is:
• A task, which is separate and distinct from other tasks.
• Has the same control requirements and work crews.
• Sequence of work.
• Specific safety and health hazards anticipated (slip or trips, cuts, dust or chips in eyes).
• Control measures (to include personal protective equipment) to eliminate or reduce each hazard to acceptable levels.
• Equipment to be used (hand tools, backhoe).
• Inspection requirements (list the inspection requirements for the activity to ensure the controls are working and equipment is inspected to ensure proper operation).
• Training requirements for all involved (any unique training required to make the established controls work) and the competent person in charge of that phase of work.
The AHA shall be continuously reviewed and, when appropriate, modified to address changing site conditions or operations. The analysis should be used during daily inspections to ensure the implementation and effectiveness of the activity's safety and health controls. Activity Hazard Analysis shall be updated as necessary to provide an effective response to changing work conditions and activities.
3.6.15 Accident Reports and Notifications
3.6.15.1 Accident Reports: For recordable injuries and illnesses, and property damage accidents resulting in at least $2,000 in damages, the prime contractor shall conduct an accident investigation to establish the root cause of the accident, complete the Navy Contractor Significant Incident Report (CSIR) and provide the report to the Contracting Officer within five calendar days of the accident. The Contracting Officer will provide copies of any required or special forms.
3.6.15.2 Accident Notification: Notify the Contracting Officer as soon as practical, but not later than four hours after any accident meeting the definition of recordable injuries or illnesses or high visibility accidents, property damage equal to or greater than $2,000.
Information shall include contractor name, contract title, type of contract, name of activity, installation or location where accident occurred, date and time of accident, names of personnel injured, extent of property damage (if any), extent of injury (if known) and brief description of accident (to include type of construction equipment used, PPE used, etc.). Preserve the conditions and evidence on the accident site until the Government investigation team arrives on site and Government investigation is
141Page conducted.
3.6.15.3 Definition of a Recordable Injury or Illness: Any work related injury or illness that results in:
a. Death, regardless of the time between the injury and death, or the length of the illness;
b. Days away from work;
c. Restricted work;
d. Transfer to another job;
e. Medical treatment beyond first aid;
f. Loss of consciousness;
g. A significant injury or illness diagnosed by a physician or other licensed health care professional, even if it did not result in a. through f. listed above.
3.6.15.4 Monthly Exposure Report: This report is a compilation of employee hours worked each month for all site workers, both prime and subcontractors, and shall be recorded daily on the Contractor Production Report. The contractor's workspace may be inspected periodically for OSHA and Navy violations. Abatement of violations will be the responsibility of the contractor and/or the Government as determined by the Contracting Officer. The contractor shall assist the Safety Office escort and the federal or state OSHA inspector if a complaint is filed. Any fines levied on the contractor by federal or state OSHA offices due to safety/health violations shall be paid promptly.
3.6.16 If the contractor fails or refuses to promptly comply with safety requirements, the Contracting
Office may issue an order stopping all or part of the work until satisfactory corrective action has been taken. No part of the time lost due to any such stop order shall be made the subject of claim for extension of time or for excess costs or damages to the contractor.
3.6.17 The Safety Office will investigate all complaints of unsafe or unhealthful working conditions received from contractor's employees, federal civilian employees, or military personnel. The Safety Office will notify the Contracting Officer of the results of such investigations.
3.7 Passes and Badges
3.7.1 Passes and Badges: Contractor employees and representatives performing work under this contract are required to be either United States citizens or documented legal residents (status verified by prime contractor). All contractor employees shall obtain the required employee and vehicle passes. Each employee is required to wear the Government issued badge over the front of the outer clothing. Failure to obtain security and base access passes shall not be a cause for a contract performance time extension. The contractor is required to immediately tum in all terminated employee's badges to the issuing office. Application for and use of badges will be as directed. Obtain access
15IPage to the installation by participating in the Defense Biometrics Identification System (DBIDS) credentials. Information on DBIDS requirements can be found here:
<https://www.cnic.navy.mil/om/dbids.html>. Immediately report instances of lost or stolen badges to the Contracting Officer.
3.7.2 All contractor employees, including subcontractors, and subcontractors' employees, suppliers, and suppliers' employees shall be required to comply with the latest edition ofNASPAXRIVINST 5510.15 "REGULATIONS FOR ADMISSIONS TO THE NAVAL AIR STATION, PATUXENT RIVER, MARYLAND COMPLEX". A copy of the regulations may be obtained from the PAX RIVER Pass and ID Office. All contractors who do not have base access will need to submit a contractor base access request using (Attachment 6 - Contractor Base Access Request Form) as detailed in Section L Instructions to Bidders. Nothing in the contract shall be construed in any way to limit the authority of the Commanding Officer to prescribe new, or to enforce existing security regulations governing the admission or exclusion of persons and the conduct of persons while aboard the station, including but not limited to, the rights of search of all persons or vehicles aboard the station. Coordinate with the Contracting Officer for specific security and access requirements.
3.7.3 Access to Buildings/Occupied Buildings: The contractor may work in or around existing occupied buildings. The contractor is responsible, via the Contracting Officer, to obtain access to building and facilities and arrange for them to be opened and closed.
Do not enter the building(s) without prior approval of the Contracting Officer. Keep the existing buildings and their contents secure at all times. Provide temporary closures as required to maintain security. Contract personnel shall not be permitted in security-regulated buildings or areas unless cleared by the Security Officer.
3.8 Identification of Contractor Employees
3.8.1 The contractor shall provide to the Contracting Officer the name or names of the responsible supervisory person or persons authorized to act for the contractor.
3.8.2 The contractor shall furnish sufficient personnel to perform all work specified within the contract.
3.8.3 Contractor employees shall conduct themselves in a proper, efficient, courteous and business-like manner.
3.8.4 The contractor shall remove from the site any individual whose continued employment is deemed by the Contracting Officer to be contrary to the public interest or inconsistent with the best interests of National Security.
3.8.5 All contractor/subcontractor employees working under this contract shall be identified by a distinctive nameplate, emblem, or patch attached in a prominent place on an outer garment. Employee identification shall not be substituted for station required passes or badges.
3.9 Identification of Contractor Vehicles: The company name shall be displayed
161Page on each of the contractor's vehicles in a manner and size that is clearly visible. All vehicles shall display a valid state license plate and safety inspection sticker, if applicable, and shall be maintained in good repair.
3.10 Permits: The contractor shall, without additional expense to the Government, obtain all appointments, licenses, and permits required for the prosecution of the work. The contractor shall comply with all applicable federal, state, and local laws. Evidence of such permits and licenses shall be provided to the Contracting Officer before work commences.
3.11 Proof of Citizenship: No employee or representative of the contractor shall be admitted to the site of work unless satisfactory proof of citizenship is furnished, or, if an alien, legal residency with in the United States is confirmed. No foreign nationals are allowed as part of the contractor installation labor force.
3.12 MINOR MATERIALS AND WORK:
Minor materials and work not specifically mentioned herein but necessary for the proper completion of the specified work shall be furnished without additional cost to the Government. The contractor shall visit the project site; collect required data about existing structures, surroundings, appurtenance and all items that will be affected by this project or related to this project; and verify existing conditions. The contractor shall be responsible for the verification of the actual site condition and the accuracy of this information to ensure adequacy of the design and successful construction.

RECORD OF CATEGORICAL EXCLUSION
SUBJECT OF THE PROPOSED ACTION:
DESCRIPTION OF THE PROPOSED ACTION:
This is a repair project intended to provide new 400 Watt LED high bay lighting systems as a replacement for the existing failing high bay lighting systems. The contractor shall provide all labor, material, equipment, and other incidental work required to complete this project.
See the Statement of Work for details.
SUMMARY OF ADDITIONAL ENVIRONMENTAL REQUIREMENTS:
These action items must be added to the specifications section of the contract or resolved before contract award.
Historic Preservation
• Hl 11 is a contributing element to the Flight Test/Tactical Historic District. This undertaking will not adversely affect this historic district, or any other historic property.
Solid Waste and Recycling
• Contact the Installation Environmental Program Director with the list of items and quantities that can be recycled. The Contractor must provide copies of all weight and landfill tickets to the Environmental Specialist within five days after removal from base.
Asbestos and Lead
• Structures or buildings that were constructed prior to 1988 have the potential to contain asbestos and/or lead based paint. If the proposed project involves any type of renovation, alteration, or demolition, asbestos or lead based paint abatement may be required. Contact the Asbestos Program Manager for further guidance.
• If the work area is found to contain regulated asbestos-containing material (RACM)*, the project activities must comply with the requirements of 40 CFR 61, Subpart M (National Emissions Standard for Asbestos - Demolition and Renovation) and 29 CFR 1926.1101 (OSHA Asbestos in Construction), as well as Maryland Title 26 - Department of the
Environment regulations (COMAR 26). The regulations require that each owner or operator of a demolition or renovation activity involving RACM remove all such material from a facility being demolished or renovated before any activity begins that would break up, dislodge, or similarly disturb the material or preclude access to the material for subsequent removal.
• If there is a release of asbestos fibers, or if there is a potential for release of lead based paint dusts, contact the Air Quality Program Manager for assistance and additional information.
• If abatement is required on a project, an abatement plan is required by a State of Maryland Licensed Project Designer. All asbestos- and lead paint-containing waste shipping manifests shall be signed by a designated and qualified government representative.
Program Manager Comments:
Follow the above action items and incorporate as appropriate into all specifications (e.g., RFPs SOWs) associated with this project. Additional information is provided below:
*RACM includes "a) friable asbestos material; b) Category I nonfriable ACM that has become friable; c) Category I nonfriable ACM that will be or has been subjected to sanding, grinding, cutting, or abrading; or d) Category II nonfriable ACM that has a high probability of becoming or has become crumbled, pulverized, or reduced to powder by the forces expected to act on the material in the course of demolition or renovation operations ... " [40 CFR 61.141]
Hazardous Materials
• The Contractor will ensure proper handling, storage and proper disposal of all hazardous material (e.g., products used by the project that require SDS).
• The Contractor is responsible for ensuring that personnel are trained in the Hazardous Communication (HAZCOM) Standard prior to hazardous material usage.
• The Contractor will maintain records throughout the course of the project of all manpower and material expenditures and will ensure accountability for material in the Contractors possession.
• The use of hazardous materials in this project may be subject to the installation's regulatory reporting requirements under the Emergency Planning and Community Right- To-Know Act (EPCRA). The following information shall be submitted to the HazMat/EPCRA Program Manager as soon as possible:
o SDS and product specs for all hazardous materials to be applied/used.
o General description of the intended use for each product.
o Total amount used (preferably weight).
o Span of dates the products are used.
o Maximum amount of product that will be on site AT ONE TIME.
Hazardous Waste
• Contractors are responsible for cost and removal of all regulated waste (Hazardous Waste, Universal Waste, Non-RCRA Regulated Waste) generated by the project in accordance with a11 federal, state, local, and Navy regulations.
• Contact the Regulated Waste Program Manager to assist in waste stream determination.
Examples of commonly generated regulated waste include, but are not limited to:
oil/fuel/lubricant material and contaminated debris, lead contaminated debris, lead paint, solvent/paint/thinner/stain material and contaminated debris, fuels/oils/lubricants, mercury switches, batteries, aerosol cans, and compressed gas cylinders.
• Waste lamps and ballasts shall be handled as universal waste or hazardous waste dependent upon their condition. Lamps/bulbs including fluorescent, HID, neon, high-pressure sodium, CFL, metal halide, LED, or mercury vapor are considered universal waste when in undamaged condition and shall be managed accordingly. Broken lamps are hazardous waste and shall be managed accordingly.
• Ensure all regulated waste is properly stored, labeled/marked, and disposed in accordance with the following local, state, and federal regulations:
o Title 40, Code of Federal Regulations, Parts 260-279 o COMAR, Title 26.13 o OPNAVINST 5090.1
• All waste disposal documentation (manifests/shipping papers) require the signature of the Regulated Waste Program Manager or someone authorized by the Regulated Waste Program Manager prior to shipping off base.
Follow the above guidance throughout the project for all waste generated and contact the Regulated Waste Program Manager for further guidance as needed.
• Ensure contractors working on new construction projects remove all unused hazardous materials and waste generated by the project.

PRESENCE OF NORMAL CIRCUMSTANCES:
This proposed action will not affect public health or safety. No wetlands or migratory bird populations will be affected by this action, and no listed threatened or endangered species are located in the vicinity. Hl 11 is a contributing element to the Flight Test/Tactical Historic District. This undertaking will not adversely affect this historic district, or any other historic property. According to various archeological surveys, no archeological sites are known to exist in this area. The proposed action does not have the potential for significant environmental effects on hazardous waste sites, according to the Federal Facilities Agreement. This action will not involve effects on the human environment that are highly uncertain, involve unique or unknown risks, or that are scientifically controversial. Finally, the proposed action does not establish a precedent nor threaten a violation of Federal, State, or local environmental law.
GENERAL CONFORMITY RULE DETERMINATJON:
(1) In accordance with Section 176(c) of the Clean Air Act, the action is located in an attainment area (St. Mary's County, MD) for NAAQS levels PM10, NO2, SO2, Carbon Monoxide, Ozone, and Pb, and is not subject to the conformity rule. This project will also meet the Northeastern Transport Region levels of 50 tons/year for Ozone.
CONCLUSION:
Subject proposed action can be categorically excluded from further National Environmental Policy Act (NEPA) processes per OPNAVINST 5090.1, Reference (c), Section 10.3.14, Table 10-6:
(8) Routine repair and maintenance of buildings, facilities, vessels, aircraft, ranges, and equipment associated with existing operations and activities (e.g., localized pest management activities, minor erosion control measures, painting, refitting, general building/structural repair, landscaping, grounds maintenance)
Polychlorinated Biphenyls (PCBs)
• Wastes containing PCBs at concentrations greater than 49 ppm must be disposed of as a hazardous waste. Disposal may require submittal of a lab analysis. Contact the Regulated Waste Program Manager to have samples taken or to obtain a lab analysis.
• Lamp ballasts may contain PCBs, ballasts manufactured prior to 1979 are assumed to contain PCBs unless "No PCB" is clearly stated. PCB-containing lamp ballasts shall be managed as Universal Waste.
There are no apparent PCB wastes associated with this project, however, contact the Regulated Waste Program Manager if PCBs are encountered.
