QUALITY ASSURANCE SURVEILLANCE PLAN (QASP)

1.0 PURPOSE

This Quality Assurance Surveillance Plan (QASP) defines the Government’s approach to measuring the Contractor’s performance against contractual objectives. The QASP establishes the methods used by the Government to measure the effectiveness of the Contractor’s quality program as defined within the Quality Program Plan (QPP). The QASP provides quality expectations and uses decision thresholds to reduce the risk of non-conforming material reaching the Fleet.

The QASP provides the Government with actionable criteria to ensure that the Contractor's quality efforts are timely, effective, and comply with contractual requirements. The QASP is not intended to duplicate the Contractor's quality efforts. The QASP is provided to support the Contractor's efforts by establishing surveillance and quality performance criteria that demonstrates that contractual hardware not only passes individual Factory Acceptance Tests (FAT), but also is a product of a controlled quality process.

1.1 SCOPE

The QASP establishes the surveillance and quality performance criteria used by the Government to demonstrate that MK54 Common Parts Kits, MK54 MOD 0 Processor Group Assemblies (PGA) and W4 Cables, and MK54 LWT Exercise Fuel Tanks associated hardware, as well as related Spares and Production Support Material (PSM) meet contractual requirements.

1.2 AUTHORITY

Authority for issuance of this QASP is provided under Federal Acquisition Regulation (FAR) Subpart 46.4 and the Inspection of Supplies and Services clauses in the contract.

2.0 ROLES AND RESPONSIBILITIES

Procuring Contracting Officer (PCO) – The PCO ensures the Contractor receives impartial and equitable treatment under the contract, and is ultimately responsible for the final determination of the adequacy of the Contractor’s performance.

The PCO will appoint a COR as the government authority for performance management. Additional government personnel may serve as technical inspectors, depending on the complexity of the services measured, as well as the contractor’s performance, and will provide their inputs and observations to the COR.

Contracting Officer Representative (COR) – The COR is responsible for technical administration of the contract, ensures proper Government surveillance of the Contractor’s performance, and provides disposition of Contract Data Requirements List (CDRL) submittals. The COR is not empowered to make any contractual commitments or to authorize any contractual changes on the Government’s behalf. Any changes that the Contractor deems may affect contract price, terms, or conditions shall be referred to the PCO for action.

Subject Matter Expert (SME) – The Government SME is responsible for technical review of the CDRL submissions.

QUALITY ASSURANCE SURVEILLANCE PLAN (QASP)

3.0 METHODS OF SURVEILLANCE

The primary means of surveillance for the criteria outlined in the QASP is through CDRL deliverables. The Government, at its discretion, may independently verify the provided information using available Government resources in accordance with contract and FAR requirements.

3.1 PERFORMANCE OF SURVEILLANCE

The Government SME will evaluate contractor performance through review of CDRLs submitted by the Contractor in accordance with the contract Statement of Work (SOW). The CDRL reviews are used to determine how the Contractor is performing against documented contractual performance objectives.

3.2 SURVEILLANCE LEVEL ADJUSTMENTS

The COR and PCO may periodically review the QASP to determine if changes in surveillance levels or methods are necessary and update the QASP accordingly. When a change in the Contractor’s required work (i.e., a change of design or programmatic rebaseline) or change in Contractor’s performance results in a change of program risk, surveillance changes may be considered.

4.0 DEFINITIONS

Factory Acceptance Test (FAT) – Final testing portion of tests and/or processes detailed in the Test Procedure - Factory Acceptance Test Procedures CDRL.

Failure Analysis and Corrective Action Report (FACAR) – A Contractor analysis and corrective action reporting process for non-conformances, as described in the Failure Analysis and Corrective Action Report (FACAR) CDRL.

Key Quality Indicator (KQI) – Process indicator or other quantitative measurement criteria with defined thresholds upon which delivery of supplies is based. KQIs provide indication of the expected quality level at subsequent tests or inspections.

Nonconformance – A product that departs from a specified requirement with respect to one or more features, dimensions, properties, or parameters that may affect subsequent operations, form, fit, or function. A nonconformance may be detected by any process, inspection, or test.

Catastrophic Non-Conformance - A failure that would create safety hazards to personnel and or loss of system platform.

Critical Non-Conformance - A failure that will result in mission loss.

Major Non-Conformance - A failure that would degrade in-water performance or cause reduction in availability.

Minor Non-Conformance - A failure that would degrade performance or cause reduction in availability but would not result in degraded in-water performance.

Systemic Failure – Identical failure symptoms of three (3) different units, of the same design, under test.

5.0 NON-CONFORMANCE THRESHOLDS

5.1 CATEGORY I NON-CONFORMANCE THRESHOLDS: FAT

Non-conformances will be categorized as Category-I non-conformances if they are discovered during FAT testing and the criticality of the mission impact is categorized as a Major, Critical, or Catastrophic non-conformance as defined above in section 4.0, and provided within the related FACAR submission(s). Category-I non-conformance thresholds are based on awarded deliveries per production year.

DRAFT

QUALITY ASSURANCE SURVEILLANCE PLAN (QASP)

Category-I non-conformances are applicable to each individual build-to-specification Unit Under Test (UUT). The Government will review all non-conformances and may exclude them from the total counted towards the threshold at their discretion. Category-I non-conformance thresholds are intended to monitor hardware quality at the Contractor’s Facility.

Table 1. Allowed Number of Open CAT I Non-conformances

5.2 CATEGORY II NON-CONFORMANCES: Factory Escapes

Non-conformances will be categorized as Category-II non-conformances if they are discovered after delivery to the government during the contractual warranty period and the criticality of the mission impact is categorized as a Major, Critical, or Catastrophic non-conformance as defined above in section 4.0, and provided within the related FACAR submission(s). Category-II non-conformance thresholds are based on awarded deliveries per production year. Category- II non-conformances are divided between MK54 MOD0 PGA & W4, MK54 Common Part Components, and MK54 LWT Exercise Fuel Tanks. The Government will review all non-conformances and may exclude them from the total counted towards the threshold at their discretion. Category-II non-conformance thresholds are intended to monitor hardware quality in the fleet.

Table 2. Categorized Factory Escape Threshold

5.3 TOTAL ALLOWABLE FACTORY ESCAPES

All non-conformances discovered after delivery to the government during the contractual warranty period will count towards the Total Allowable Factory Escapes. Total Allowable Factory Escapes are based on awarded deliveries per production year. Total Allowable Factory Escapes are divided between MK54 MOD0 PGA & W4, MK54 Common Part Components, and MK54 LWT Exercise Fuel Tanks. Non-conformances discovered within the first 100 MK54 MOD0 PGA & W4, the first 100 MK54 Common Part Components, and the first 50 MK54 LWT Exercise Fuel Tanks delivered on the contract are excluded from total allowable factory escapes. Total Allowable Factory Escapes is intended to monitor schedule impacts at the Intermediate Maintenance Activities (IMAs).
