About this file

This is a Past Performance Questionnaire template designed for evaluating contractors bidding on a Blanket Purchase Agreement (BPA) with Naval Surface Warfare Center Indian Head Division (NSWC IHD) for Special Industrial Manufacturing Equipment and Parts covering vacuum pumps, mixers, and grinders across multiple PSC/FSC codes (4310, J036, 3660, 3695, 4330, and 3665). The questionnaire collects information from government agencies and activities that have previous experience with prospective offerors, including the contractor's name, contract order number, item description, cost, and the duration of the evaluating agency's relationship with the contractor.

The questionnaire evaluates past performance across five primary criteria: Customer Satisfaction (responsiveness to needs, personnel qualifications, cost estimation accuracy); Timeliness (task completion within requested timeframes); Technical Success (understanding of SOW requirements, first-time task completion, problem resolution); Program Management (subcontractor management and cost/schedule/performance control); and Quality (reliability and quality of services delivered). Contractors are rated as either ACCEPTABLE or UNACCEPTABLE based on whether prior contract performance met or exceeded requirements with minor issues and effective corrective actions, or failed to meet requirements with serious problems and ineffective corrective actions. The questionnaire also requests subjective recommendations regarding contractor suitability for similar contracts and disclosure of any special or unique problems. Completed questionnaires should be submitted to Rebecca Collins at Naval Surface Warfare Center, 4081 N. Jackson Road, Building 841, Indian Head, MD 20640-5115.

************************************************************************************** RATING

Please use the following guidelines to answer the questions. The Offeror’s Past Performance will be considered ACCEPTABLE or UNACCEPTABLE as detailed below:

EVALUATION CRITERIA

ACCEPTABLE-The Offeror’s performance of previously awarded relevant contract(s) met or exceeded contractual requirements. The assessed prior performance was accomplished with very minor problems or some problems for which corrective actions taken, or proposed to be taken, by the Offeror were, or are expected to be, mostly effective. Performance over completed contracts ranged from adequate to the highest quality or exhibited a trend of becoming so. The Offeror’s past performance record leads to an expectation of successful performance.

UNACCEPTABLE-The Offeror’s performance of previously awarded relevant contract(s) did not meet some contractual requirements and recovery did not occur within the period of performance. The assessed prior performance reflected some serious problem(s) for which the Offeror either failed to identify or implement corrective actions in a timely manner, or for which the corrective actions implemented or proposed to be implemented were, or are expected to be, partially or mostly ineffective. Performance over completed contracts was consistently of mediocre or poor quality or exhibited a trend of becoming so. The Offeror’s past performance record leads to an expectation that successful performance will not be achieved, or might be difficult to achieve, or that it can occur only with increased levels of Government management and oversight.
