MARKET RESEARCH QUESTIONNAIRE

Mobile Mammography Services – Federal Correctional Institution –Waseca, Minnesota

The Federal Bureau of Prisons is conducting market research in compliance with Federal Acquisition Regulation, Part 10. The Federal Bureau of Prisons intends to establish a contract for the provision of Mobile Mammography Services for female inmates located at the Federal Correctional Institution (hereinafter referred to as FCI) Waseca, MN. The physical address for FCI Waseca is 1000 University Drive, SW, Waseca, MN 56093.

The Government contemplates awarding of an indefinite-delivery, requirements-type contract with firm-fixed session resulting from the future solicitation. The contract period shall consists of a 12 month base period with four (4) 12 month option periods for renewal. It is anticipated that eight one-hour sessions will be needed three times per year. One session is defined as one hour in length.

Sessions will typically be scheduled every other month between the months of April and September.

Sessions will be scheduled between the hours of 7:30 a.m. and 4:00 p.m., Monday through Friday, excluding Federal Holidays. The contractor will be notified in advance of the requested date of service and number of inmates to be examined. For further description of the services, read the attached Statement of Work (SOW).

Services include, but are not limited to,

• Evaluation of patients are referred by Health Services staff.

• Perform digital breast screening mammograms.

• Provide professional interpretations. Fees for professional services shall be included in the exam price. Reports must be typewritten of computerized format. Reports must be received within seven days of service, unless abnormalities are noted. FCI Waseca Health Services must be notified of abnormal reports within 48 hours of service. The contractor shall provide a copy of the mammogram image on CD for each mammogram performed.

The mobile unit’s digital imaging capabilities shall allow the operator to readily complete approximately 25 patients per session. The number of patients requiring services will determine the length of session. The contractor will be responsible for equipment transportation, delivery and set up at facility, tractors, trailers, drivers, complete maintenance/service coverage on all equipment, training and staffing insurance, site planning and consultation, marketing, support, etc.

The purpose of this Market Research is to gather data from this survey is for informational purposes only in regards to the upcoming solicitation. If you are interested in providing the services or retrieving the solicitation, please answer the following questions.

The North American Industrial Classification System (NAICS) code for Mobile Mammography Services is 621512, which sets the small business size standard at $16.5 million (below $16.5 million in gross revenues is considered small, above $16.5 million in gross revenues is considered large). Any subcontracts for the provision of services must contain the requirements of the clause at FAR 52.237-7, “Indemnification and Medical Liability Insurance,” including the maintenance of medical liability insurance. All potential quoters are advised if a solicitation is issued it will include the clause at FAR 52.212-4 (Feb 2007) for Commercial Items, paragraph (t) System for Award Management (SAM).
