About this file
This is a Certificate of Eligibility and Debarment document issued by the State of Maine for a one-time purchase of special purpose tool cabinets. The document requires bidders to certify their eligibility to participate in the procurement by confirming they are not presently debarred, suspended, proposed for debarment, or declared ineligible by any Federal Department or Agency. Bidders must also attest that they have not been convicted of or had civil judgments rendered against them within the three-year period preceding the proposal for fraud, criminal offenses, anti-trust violations, embezzlement, theft, forgery, bribery, falsification of records, false statements, or receipt of stolen property in connection with any public transaction or contract. Additionally, bidders must certify they are not presently indicted or criminally or civilly charged with any of these offenses and have not had any public transactions terminated by default within the three-year period preceding the application.
Bidders are required to acknowledge their understanding that they must promptly inform the State of Maine of any information that subsequently contradicts the certifications provided in paragraphs one through four. Should a bidder be unable to certify to all statements, the bidder must provide a written explanation to the State of Maine. The document requires the authorized representative of the bidding company to sign and date the certification, including their printed name and title, thereby attesting to the truthfulness and accuracy of all statements made in the certificate.
