About this file
This is a proposal cover sheet for a Mini-RFP issued by the Oregon Department of Transportation for the Norton Ave and Larkspur Path School Connection project. The A&E and related services sought are intended to advance the project from the Design Acceptance Package phase to Final Plans, Specifications, and Estimates, with the proposer potentially providing Professional of Record and Construction Engineering Support services during construction if required. Proposers must hold a minimum qualification of being an Oregon Registered Professional Engineer. The cover sheet requires the proposer to provide firm name, primary contact information, email address, and telephone number.
The cover sheet functions as a certification document in which the proposer's authorized representative attests to receipt of all addenda, accuracy of proposal contents, compliance with non-discrimination requirements, possession of sexual harassment and discrimination prevention policies (if contract value exceeds $150,000), absence of undisclosed liquidated debt to the State of Oregon, acceptance of all Mini-RFP terms and conditions, and completion of required Conflict of Interest disclosures. Proposers must confirm either that they have no conflicts of interest or that all required disclosures have been submitted separately. The authorized representative must sign and date the cover sheet and check an "APPROVED" checkbox to bind the proposer and certify authority to submit the proposal on behalf of the firm.
